22.01.2022 Having been through a rotator cuff repair myself in which we commenced measurable submaximal loading within the first 2 weeks and had great function at 3 months it has become part of the way we approach our post op rot cuff protocols in those seeking early return to function. The key is measure what it can handle. It can’t not work so we need to find on the spectrum where an individual sits. Do nothing on one end. Do damage on the other. ... When prescribed a protocol exercise if you’re too close to the do nothing you will have sub optimal outcomes as the graft isn’t being loaded enough to improve the bind to the bone and strengthen the muscle belonging to the repaired tendon. Too much and yes you can re-tear. So make sure when you are treating a rotator cuff repair you know how to find that sweet spot and load appropriately. Don’t let it become weaker!!

18.01.2022 Always love the complexity of the knee and how it is controlled ... I also feel we don’t give enough cred to the role of the hammie’s. Perhaps one of the reasons we get so many lateral hamstring issues is the imbalance of this rotational component. To the Physios and other ahps’s out there do you measure strength rotational contributions to the knee?? Medial v lateral? How do you handle them? Do think foot and hip rotations have an effect? Do you bias you’re iso hammy stren...gth tests?? To the weekend warriors, Aspiring athletes and already made its with hammy and knee issues have you delved deeper into how your hammies work at the knee AND the hip AND the foot ... nothing worse than a repeat hammy strain ... happy to field some questions if anyone wants to direct message. 09.01.2022 What a weird time it is for all at the moment ... Every day something new or different. For me its the swings and roundabouts and the emotional roller-coaster I woke up very early this morning and got stuck into work - anything i could possibly do ... then the the self doubt... "What if i cant do anything?" "What if i let my team down?" So many things beyond my control. So I went and laid down and tried to breathe and relax. Then i went for a run - first one in a while due to a recent run of injuries ... Then the ideas started to come, then the the motivation and now the actions Point is - you can only control the controllables --> manage the mindset and create and commit to the routine Happy to chat

07.01.2022 Pre 6am work out. Long day ahead ... got to find the time to win the day #beabetteryou I am now 8 months post a para-umbilical hernia mesh repair. Yeah I’ve probably overdone it ... well I know. I can feel the bump in my belly. She has split open again ... Will it stop me this time? Should it stop me? ... I did loaded chin ups for the first time since ripping it 12 months ago. Pushed and pulled the sled. Ran 17km on Sunday ... I can feel it But I will go on

07.01.2022 Achilles tendonopathy and donuts!! Do we treat the donut or the hole??? ***Case study*** ... . . . Patient presents with Achilles tendon issues on both sides. struggling to ride her and difficulty with work (bus driver). 18 months of physio with no results doing some stretches, dry needling and calf raises . . . Patient obviously distressed is sent to P360. We assess her and say lets give this a solid 8 weeks of loading it heavy. After 2 weeks we do our first in-house 3D ultrasound scan and freaked a little ... we had never seen so many "holes"in the tendon (holes refer to big parts of degenerated or torn tendon) ... Was it worth treating the hole??? or do we just aim to improve function and work with what we have got . . . We discussed it and went for it ... heavy heavy loading targeted directly on the hole ... measured her maximal isometric contraction in kilograms and targeted the inside back portion of the tendon in 5 weeks we re-scanned and you can see what we found... well you don't need to be an expert to see the change . . . Is she better though??? Lets just say she is bangin out some of Darryl Braithwaites best work and has even started playing tennis "hail to the bus driver" She still has a lot of work to do but finally on the path to #beabetteryou . . . 05.01.2022 ** Please share - Excellent Case Story of a healed ACL tear without surgery from Perth Australia! ** A 16 year old female injured her left knee in early May 2...018 playing netball, with an ensuing MRI revealing a complete rupture of her left ACL, and a strain to her MCL. She was recommended to undergo a full knee reconstruction by two different Orthopaedic specialists, and her surgery was subsequently booked in to be performed two weeks later. However, as she was also completing high school exams, her parents elected to delay the surgery until 4 weeks post injury once her exams had finished. Her surgery was postponed a second time because of sickness, subsequently delayed until 6 weeks post injury, and then cancelled. During this time, her physiotherapist continued rehabilitation and strengthening, with the patient having no pain and was able to run, jump and hop with no instability. She played her first official game of netball at 9 weeks post injury, continuing to play each week until the season ended in September 2018. 6 month follow-up report and comparison scans below! 01.01.2022 Interesting concept and something I have been kicking around with for a while. Can we make it thicker? If we do does that make it stronger?... And if it’s stronger will it prevent rupture, enhance healing or improve graft reconstruction outcomes Would it give more confidence? I’m not convinced as yet ... What’s the strongest thing around the knee?? Maybe its the muscles??? Why don’t we focus on them? We need the muscles to fail to place the necessary fast heavy strain in a vulnerable position to see the ACL tear ... Learn to control the position. Muscle firing and patterns. Practice landing and changing direction especially when tired and fatigued Don’t put the acl in a vulnerable position or at least learn to control it then guess what it doesn’t matter what graft type, what animal or artificial product they’ve used, what position the tunnel is in and what thickness it is. If it helps lets thicken it. But let’s not forget that muscle training is still king No assessing means you’re guessing - know how your muscles work and how they function. #letstalkacl #beabetteryou
