25.01.2022 Let’s talk about shoulder bursitis! A common injury that is also very responsive to treatment. Shoulder bursitis is usually caused by overuse with poor mechanics. Note that overuse doesn’t necessarily mean it has to take months... it can happen with just one day of painting (for example) if the shoulder is weak enough.... There lies the actual problem. Weakness and poor mechanics, usually related to poor shoulder postures. By strengthening the rotator cuff and surrounding muscles we can provide a long lasting solution to this problem because not only are we making the muscles themselves stronger, we will also improve the shoulders posture and dynamic stability. 24.01.2022 Let’s talk about plantar fasciitis! A painful condition characterised by pain in the heel or in the arch of the foot. It may or may not be accompanied by a heel spur. First things first, plantar fasciitis is an overuse injury so it develops over a period of time. ... A common mistake I see is putting too much of an emphasis on stretching. Stretching the calves, the arch, rolling the arch with a ball etc. None of these methods offer a long term solution. The best way to fix plantar fasciitis and prevent it from returning is by doing STRENGTHENING exercises and assessing your footwear to wear something suitable for your foot. 23.01.2022 Earlier this month I was approached to be the recommended physiotherapist for the NSW police force in the Coffs Harbour area. Thanks to Mac for reaching out and recognising that mental health support is incredibly important to our police officers following injuries suffered on the job. The trauma they experience is unfortunately not really talked about in the mainstream. In fact, more and more police officers are shown in a negative light which is a shame.... I am thankful to show my support and be able to offer my services when needed.
23.01.2022 I wanted to share some important research with mothers that may be dealing with chronic pain. This study from 2019 looked at mothers with chronic pain and the impact it had on their 8-12 year old children. Findings:... Mothers with chronic pain tended to have higher number of adverse childhood experiences themselves (parental divorce, emotional, physical, or sexual abuse etc) when compared to mothers without chronic pain. Higher ACE scores in mothers corresponded with lower physical and social functioning, greater anxiety and depression symptoms, greater fatigue and sleep disturbances, and higher pain levels. Higher ACE scores in mothers also significantly correlated to child self-reported depressive symptoms. A link was found between maternal depression and child depressive symptoms. This study continues to show us the link between ACE’s, chronic pain, mental health and the impact it can have on our children. It is a vicious cycle but one that can be stopped by seeking the right treatment. 23.01.2022 Let’s talk about lower back disc injuries! I see these all the time and fortunately they respond really well to physio treatment. On the flip side, it’s unfortunate that there are still a lot of false beliefs about disc injuries. First off, they are very common and the vast majority are mild. It’s quite rare to have a serious disc injury. Secondly, discs degenerate naturally as part of aging.... 37% of 20 year olds have disc degeneration present on MRI’s. A 40 year old has a 50% chance of having a disc bulge on MRI. These numbers increase as we age. In practice, I find that disc injuries heal really well provided you give them the right environment. Treatments revolve around keeping the muscles loose and keeping people active! 21.01.2022 Ahhh yes, the good old core stability exercises. Raise your hand if you’ve been prescribed these types of exercises before by your physio Admittedly, I used to prescribe these too when I first started and didn’t know any better. I remember actually receiving lectures about core stability exercises in uni So why aren’t they any better than general exercise?... 1. It assumes that the pain comes from a physical defect when in fact it could be due to the central nervous system. This is usually the case with chronic pain. The biggest hurdles are things like fear, worry, anxiety and flare-ups NOT lack of core strength. 2. People are strong! Yes generally everyone COULD be stronger, but the reality is we are still healthy functioning human beings even if we do have back pain. 3. No one wants to sit at home doing core exercises on their floor... I don’t wanna do them and neither would anyone else. The best exercise is the one that actually gets DONE! You’re better off going for a walk with a friend, family member, or your dog at the jetty or your local beach. That way you’ll be exercising whilst enjoying life and genuinely feeling happy! 21.01.2022 Let’s talk about state management! What I see time and time again when working with chronic pain patients is a lack of proper state management. It’s a cycle of doing things that are harmful repeatedly and not thinking about the physical consequences. Let’s use an obvious example. Say I go out on a 3 day bender and drink heaps of alcohol, don’t sleep, eat poorly, dehydrate myself, etc. how do you think I’ll feel on the 4th day? Yea... pretty bad.... The emotional state has a significant impact on your physical state, more than you would expect. This is why working on state management is a crucial part to how I treat chronic pain. Similarly, your physical state impacts your emotional state. It’s why we feel so good after exercising. Last week I had a chronic pain patient mention his back pain had been worse but he did not know why. After speaking to him a bit more he mentioned his close mate had passed away a few days ago. Suddenly it clicked. The emotional states he had been in, anger, sadness, grief, anxiety etc. had a direct impact on his pain levels. There was no change physically to his back, but there was a very real and significant change to his emotional state. These are things that are not often talked about by medical professionals with their patients and they fall through the cracks and end up in the chronic pain cycle seeking saviours, surgeries, and magical pills to solve all the issues. 19.01.2022 If we want to get serious about mental health we need to discuss the root of these issues which often lies in childhood trauma. An ACE score of 4 or more means one is 3x more likely to develop chronic pain, 4x more likely to suffer from anxiety, 5x more likely to suffer from depression, and a whopping 37x more likely to attempt suicide when compared to someone with an ACE score of 0. There are numerous health risks associated with higher scores. Also it’s worth mentioning t...hat these are 10 of the most common traumas but there are certainly other types (I.e. war, bullying, etc.) Fortunately, these traumas can be overcome through different types of therapies available. 18.01.2022 I love dry needling! To answer some frequent questions that I get, yes they are acupuncture needles. However, dry needling is different than acupuncture. Dry needling is used to release tight muscles. Think of it like a very efficient, pin point, remedial massage. Does it hurt??? That depends! The most common feedback I get is that it felt weird which it does. The overwhelming majority of patients handle it without any problems. Best of all, it’s all over very quickly.... 18.01.2022 This study, and many others, detail the importance of psychosocial factors on injuries in teenage athletes. When’s the last time a physiotherapist asked you about your sleep schedule? Or your general well-being as you return to sport? The most interesting finding to me was that higher competence-based self esteem was linked to injury risk. This means that a teenage athlete who bases their self-esteem on their athletic ability and competition results actually has poorer injury... outcomes. This makes sense. When an athlete performs well they’re on top of the world. When faced with injuries or setbacks they may feel empty because they believe so much of who they are is based on their performance. This lack of confidence when they return to sport, or after a bad game, is what increases the risk of injury. A focus should be placed on educating athletes that the same traits that make them great on the field (leadership, discipline, work ethic, communication, resilience, etc.) make them great PEOPLE off the field. 14.01.2022 Let’s talk about sciatica! Sciatica is another common condition that responds quite well to treatment. Often, sciatica occurs as a result of nerve irritation that occurs in the lower back. This isn’t always the case as the irritation can happen in the glute muscles as well. Patients often describe a pain in their butt cheek and difficulty sitting. Depending on the source, I find releasing tight muscles and mobilising the sciatic nerve works well to reduce symptoms. As inflamm...ation can be a big source of the pain, an underrated tip is to drink plenty of water and maintain a good diet. They just put up with it, but there’s no doubt it interferes with their day to day life. It’s not until they’re gone they realise how much better they feel! Before one can manage their pain levels they must first master their moods, their emotions, their impulses, their thoughts, their behaviours etc. Each of these steps are goals themselves that have to be achieved before reaching the bigger goal.... 08.01.2022 Adverse Childhood Experiences (ACEs) have a profound negative impact on a persons life and yet it is something that health professionals know very little about. These types of trauma are relatively common and are predictors of poor health outcomes including substance abuse, increased promiscuity, mental illness, higher suicide rates, and chronic pain among other things. In my clinical practice, a lot of my patients who have chronic pain often have had multiple ACEs. This is w...hy simply doing hands-on work and exercise alone is not a solution. Education itself is only limited by what the practitioner knows and often times these areas are ignored. I can tell you this stuff wasn’t discussed in my masters degree nor was it in a recent chronic pain course I finished. It’s a long hard road to rehabilitation that involves multidisciplinary care. As a physio, I have an excellent opportunity to listen to a patient’s story and guide them in the right direction. Often times I am not their first contact with therapy but I certainly can make a difference by showing or reminding them how to break out of the chronic pain cycle. 07.01.2022 Scoliosis progress pic! These pictures were taken only 2 weeks apart and after only 3 treatments. Scoliosis (abnormal curvature of the spine) is very rarely a serious issue and about half of the population has a functional scoliosis due to poor postures and muscle imbalances. Good results can be achieved pretty quickly with the right treatment.... I used a combination of dry needling, massage, joint mobilisations and also prescribed the patient a specific set of exercises he could perform at home. 06.01.2022 Before we dig deeper into chronic pain we have to establish what it is and what influences it. Chronic pain is any pain that lingers on 3 months after the initial injury. Generally speaking, most tissues in our bodies will fully heal within 3 months. Pain that remains after that is deemed to be chronic and is more likely to come from the central nervous system as opposed to tissue damage. This type of pain is significantly influenced by psychosocial factors such as your bel...iefs about pain, childhood experiences, mental health, stress, your friends and family, etc. Note that this does not mean the pain isn’t real, it very much is. It’s the SOURCE of the pain that is in question. In order to provide a quality treatment, we MUST address these psychosocial factors. Failure to do so will lead to an ineffective treatment that will have little impact on pain and disability. Suicide rates for men are increasing and I have personally been close to a few young men that chose suicide as a permanent solution to their temporary problems. I want to do my part to offer help. 05.01.2022 Let’s talk about the role the superego/inner critic has on chronic pain and mental health. The superego is essentially the inner critic. The little voice inside the head that tells us when we’ve made the wrong choice, makes us feel guilt or shame, or generally points out our flaws. What I find is that in a lot of cases of chronic pain or poor mental health, that critic is on overdrive. Patients often explain to me how they feel worthless or that the pain has taken over thei...r life. They have listened to this inner critic for so long that they believe everything it tells them. A lot of this stems from childhood. If a parent or other authoritative figure only ever notices the negative traits of the child, that child then begins to think that’s all they are. They view themselves as a bad object.
