23.01.2022 Some Frequently Asked Questions: Level 2 guidelines Are these guidelines mandatory? These guidelines are not mandatory at this stage. The framework published by the ADA on Monday 23 March provides guidance to dentists providing treatment during the COVID-19 pandemic. The ADA is the not the regulator, so this framework is a guideline to assist members.... What treatments are considered essential services? The framework outlines examples of Emergency, Urgent and Routine dental care. As noted in the framework, this is not intended to be a definitive list, and dentists should use their clinical judgement to determine where a particular treatment sits, taking into account the individual patient circumstances and risk. Do we have to stop all procedures that produce aerosols? No. Aerosol generating procedures increase the risk of transmission for COVID-19, so the advice to dentists under Level 2 is to minimise the provision of aerosol generating routine dental treatment until otherwise advised. Dentists should understand the risk to themselves, their staff and patients, and use their clinical judgement to determine what treatment procedures to provide. You should also ensure that you take all reasonable steps to reduce aerosols, including the use of rubber dam and high-speed evacuation. You should also consider what alternative treatment modalities might be used to reduce aerosols. What if I have patients already booked in for treatment? You need to consider whether that treatment required meets the criteria for routine, urgent or emergency to see where is fits within the framework. If treatment can be postponed for up to 4-6 months without impacting on the patient and if the treatment is likely to cause significant aerosols, then consider postponing treatment. You need to consider the potential risk to you and your staff, as well as your patients. It is important that you use your clinical judgement. What about [particular] treatment? The ADAVB cannot advise you on specific dental conditions or treatment procedures. Dentists MUST exercise their clinical judgement using the framework and guidelines, taking into account the particular circumstances of the patient and the risk involved. What happens next? We expect to see an escalation to Level 3 mandated by the government at some point in the future, so dentists must be prepared for that eventuality. It is also important to consider that in the context of treatment decisions that are made now. If routine treatment is suspended for 4-6 months under Level 3, then some routine dental conditions now may deteriorate significantly and require urgent or emergency care in the future. This should guide treatment decisions being made now.
