25.01.2022 PIGMENTATION Not all pigmented lesions are suitable for laser treatment which is why we insist that all patients see our doctors first for medical assessment to make a diagnosis and management plan prior to booking any treatment. This lovely patient had a pigmented lesion on her right cheek that is known as a solar lentigo and is a result of sun exposure and solar damage. Unfortunately traditional laser treatment was not suitable which is why I used Enlighten III picoseco...nd laser @cutera.anz to remove her lesion. Picosecond laser works differently to other laser technology, causing a mechanical breakdown of the pigment allowing your body to then remove the fragmented particles. As picosecond laser causes significantly less heating of the skin, it is a safer option for patients with darker coloured skin or other risk factors. 23.01.2022 MELANIN Did you know that our skin colour is determined by the presence of multiple pigments within our skin: melanin (brown), carotene (yellow), oxygenated haemoglobin (red) and deoxygenated haemoglobin (blue/purple). Of these, melanin is the most important in determining skin colour. The process of melanin production is called melanogenesis. It is a complex process with many steps: melanocytes are the cells responsible for producing melanin... melanin is packaged into granules called melanosomes melanosomes are transported by ‘arms’ of the melanocyte called dendrites the transported melanosomes are delivered to the surrounding cells called keratinocytes as the keratinocyte matures, it migrates to the surface of the skin taking the melanin with it All of these stages of melanin production, processing, packaging and transportation can be altered leading to increased pigmentation, or hyperpigmentation. 21.01.2022 CAUTION Social media influencers and celebrities are often used to promote and market skin care products to the public, claiming that these are the products that they personally use. The reality is that they often do not use these products regularly and in some cases have only trialled them for a short period of time.... But what is even more important is that it really makes no difference what they are using on their skin because your skin is completely different - it is your skin and not their skin. So whilst it is great that they may be very happy with their skin care, we caution you not to purchase a product solely because an influencer or celebrity endorses it. 18.01.2022 BEFORE & AFTER PHOTOS Cosmetic social media accounts are often littered with before and after photos and understandably so. Before you have any cosmetic procedure you are likely to want to see what results are being achieved by a particular clinic or clinician. Interestingly though, the quality of these photos varies widely. After photos are often in better light with patients smiling or in more advantageous positions. We do not routinely take photos after dermal filler p...rocedures. We take repeat imaging at F/U appointments to monitor changes over time. Immediate after results are obviously extremely important to us, but what matters even more, is what our patients look like in animation or with movement. This cannot be captured with still photography. It is not uncommon for us to see patients who have been treated elsewhere with dramatic before and after photos, but in real life and with facial movement, their filler is immediately obvious and distracting. This in our opinion is not acceptable nor aesthetically pleasing. 18.01.2022 MELASMA Melasma is often a difficult condition to treat but one of the most rewarding. Most people would recognise melasma as a condition of facial pigmentation but an increasing understanding of the role of inflammation/vascular component has led to better treatment options and outcomes with longer lasting results. Whilst there is no one approach to melasma, with each patient requiring a different treatment plan, there are general principles to follow. These include: Avo...idance of triggers Reducing melanin production Increasing cell turnover Targeting and removing pigment Reducing inflammation Unfortunately I see many patients who have tried various commonly marketed dermal treatments that have only exacerbated their melasma in the long term. So whilst this is an extremely distressing condition and I understand that patients are keen to remove their pigment quickly, my advice is to seek medical advice and to approach management systematically and sensitively. Not to rush out and try a little bit of this and a little bit of that. My lovely patient here has had a significant reduction in her pigmentation after only 3 months of prescription topical skin care. No peels. No laser. No dermal treatments. No downtime. Of course the journey doesnt stop here. Melasma is a lifelong chronic medical condition and therefore takes dedication and commitment. At the end of his training and after passing an exam, he will be awarded the hard earned title of a medical fellow of the college. What works for one skin type is not necessarily effective nor safe for those with skin of colour. 12.01.2022 PIGMENTATION This lovely lady presented wanting to improve her skin quality and more specifically to remove her pigmentation. The small pigmented lesion on the tip of her nose showed a minor suspicious change under dermoscopy examination. Before proceeding to treat her pigmentation we performed a biopsy of the lesion. The results showed that it was a lentigo maligna, which is an early form of melanoma that has not yet spread or become invasive. This lesion needs to be completely removed by surgery. This is a perfect example of why we insist on a consultation and examination of your skin prior to any laser treatment or other skin treatments. Even after skin treatments, if any lesions do not respond in the way that we expect, or they return after treatment, then we insist on a biopsy. Safety first! Drs Jennifer and Bernard Leung

06.01.2022 PARTNERSHIP We are often asked by our patients and our colleagues How can you possibly work with your spouse?. They are often surprised to hear that not only do we enjoy working together but that we prefer it this way. We first met each other through our medical practice and we have continued to work together to this day. There are only a handful of cosmetic doctor couples like us in the industry. Working together and running our practice together has its unique challe...nges. We never have a day off. Holidays invariably involve studies, conferences, and practice troubleshooting whilst away. Evenings are spent educating ourselves and thinking of ways to do things better. Of course we have the occasional disagreements. BUT, we live and breathe our profession. We get to share our successes and failures each day together. We know exactly what each other has been through that day. We share our passion for our craft and drive for perfection. We wouldnt have it any other way. In fact, as our aesthetic practice has developed over the years we are now able to produce results that were not achievable in the past. Our focus is now heavily on facial aesthetics taking into account every layer of the face and how it ages and changes with time. We have more recently started managing and treating our new patients together and the feedback has been overwhelmingly positive. These are the patients who have seen some of the most dramatic and long lasting results. So what we now offer is the ultimate in facial assessment and treatment. When we take on a patient it is for life. It is our privilege to be a part of each of our patients aesthetic journey. In short, we love working with each other. When we think about cleansing our face, there are a multitude of products on the market - foaming cleansers, milk cleansers, scrubs, exfoliating cleansers and cleansers for different skin types and skin conditions. Most of us appreciate that the main purpose of cleansing our skin is to remove makeup, pollution, suncream and other toxic elements in the environment. If left on your skin, these external factors c...ontribute to increased skin sensitivity, inflammation and ultimately premature skin ageing. In fact, at the recent Australasian Society of Cosmetic Dermatologists conference that we attended in Sydney, the increased prevalence of skin sensitivity and inflammatory skin conditions was discussed and partly contributed to the fact that we as a nation are now using many more products on our skin, many of which are potentially harmful. The original thinking behind this was our observation that the cosmetic industry has grown exponentially, often driven by commercial interest, and regulations have not always kept up.
