18.01.2022 Non-Surgical Knee ACL Injury Management A common belief has been that having a torn ACL means you cannot return to pivoting and landing sports with an ACL reconstruction (ACLR). There have been a growing number of cases at all levels of activity, from leisure activities to professional sport. There are studies that compared surgical and conservative treatments which have found that ACLR is not a prerequisite to return to sport (Delince & Ghafil, 2013).... An older study by McDaniel and Dameron (1983) found that 76% of untreated patients with ACL ruptures returned to strenuous sport. The follow-up on average was 14 years. A study by Hurd et al in 2009 found that individuals who went with non-surgical management could return to high-level activities without subsequent problems of instability. Another study, Grindem et al 2012, found many individuals returning to running, jumping and cutting sports after undergoing non-surgical management. One of the long-held beliefs about ACL injuries is that once your ACL is torn, it cannot repair itself. There is scientific evidence to show that in fact the ACL can repair and regrow itself if it's given the opportunity! The time-frame for healing has usually been 6 to 12 months with regular strengthen training. It's important to note that the ACL doesn't repair in every case, and more research is needed to find out what factors may promote healing. Notable studies in which full thickness ACL ruptures healed include: - Ihara et al (1994), 20 out of 25 patients - Fujimoto et al (2001) 23 out of 30 patients - Costa-Paz et al (2012) 14 out of 14 patients - van Meer et al (2014) 30 out of 50 patients - Marangoni et al (2018) 30 out of 30 patients Overall, there is strong evidence that non-surgical management is able to facilitate a return to sports of all levels. It is inevitable that there will also be cases where surgery is required. My goal is to help people see that there is more than one option. Regardless if you go with non-surgical or require surgery for ACRL, starting an exercise rehabilitation program as early as possible will help with the eventual recovery.
