25.01.2022 Ankle sprains are hot in the clinic again! Gentle pain-free lower limb muscular stretching and ankle joint ROM exercises should be encouraged within the first 72 hours of injury. This will influence normal joint, neural and soft tissue afferent input, joint kinematics and motor patterning. Basic ROM exercises should be coupled with stretch exercise regimes to improve the function and mobility of the ankle joint. Normal motions must be maintained or re-trained as early as the... injury is capable (pain free) to minimize the risk of re-injury. It is of most importance to adopt pain free motions during the acute stages of ankle sprains, to avoid movements through planes of injury. This will prevent injured tissue irritation and prevent further damage to injured tissue.



25.01.2022 As Cam McEntyre heads off to World Junior Champs he gives a great message about being able to separate your sporting life from your home/social life. Mental and physical stress from training/competition can become intense and if not well managed by switching off can lead to burnout and injuries. It is important to have a balanced lifestyle off the field to relax away from the sporting environment. This enables athletes to recharge themselves so they can be 110% on for the next training session or competition. Switching off is a very important message I give to my athletes and patients to avoid mental exhaustion resulting in negative effects on performance.

25.01.2022 Rehabilitating the Ankle Part II - Balance and Proprioception Proprioception is the bodies awareness of the position of your joints in space. Input from pressure and sensory receptors in the ankle joint and foot is relayed to the brain to determine the position of the joint. When injured, these receptors are disturbed and a loss of proprioception is likely. Balance exercises stress these proprioceptive receptors, and when placed under exercise stress offer an effective method... of ankle healing and rehabilitation. Static balance and proprioceptive exercises should start with the foot in a stable position and progress accordingly to positions of relative instability. This benefits by adding further stress to reactivate the sub-optimally functioning neuromuscular system of the injured ankle. Progressing exercises needs to be gentle and appropriate in accordance to the injuries tissue healing rate and functional ability. The progression to unstable surfaces become necessary to stress contractile rate for force development, reactive stability and the foots ability to act as an effective mobile adaptor. (refrerence) Examples of unstable surfaces include foam boards, wobble boards, balance disc, balance beams, bosu balls, trampettes or sand. Integrating unstable surface proprioceptive training for as little as five minutes per day in an ankle rehabilitation program has proven a significant reduction in ankle injury risks through a competitive sports season. Additionally, if the strong foot position is applied, the quality of the exercises can be specifically enhanced through the use of a softer pliable surface. *Note - Prematurely rehabilitated ankles are at risk of irritating or damaging healing tissue if the correct progression of rehabilitation has not been followed. Adequate strength progressions must be completed prior to avoid irritating or further ankle injury.

24.01.2022 Is it Plantar Fasciitis? There are numerous structures in the foot which could be contributing to foot and heel pain than just the plantar fascia. Constant weightbearing from daily activities such as walking, running or even prolonged periods of standing at work often cause biomechanical changes which affect the function of the foot. Check out all the muscles that influence the bottom of the foot! The foot is highly rich with proprioceptive feedback and when altered by chan...ges to foot biomechanics drastically effect balance and awareness. Joints become fibrotic and immobile, muscles short and hypertonic and pain is the outcome. When treating the foot, it is important to identify and address any joint mechanical issues from heel to toe. The many muscles that affect foot arch(es) stabilization and toe movements need specific attention. Not to forget the lower leg muscles and their tendinous insertions that influence foot mechanics from afar! Top it off by influencing good foot behaviour with rehab exercises! 20.01.2022 Rehabilitating the Ankle Part IV - Advanced Proprioception Return-to-play mechanisms require repetitive and sports-specific training to develop and ingrain specific neural pathways. Technical training targeted at sports performance can accelerate an athletes return to sport and reduce the risk of re-injury. The introduction of multi-lateral, multi-surface jumping/hopping exercises becomes necessary to integrate a specific level of proprioceptive stimulation and intrinsic stre...ngthening to ankle joint rehabilitation. These dynamic proprioceptive exercises will prepare the ankle and lower limb for weight bearing impacts and multi-plane dynamic loading. By this stage of rehabilitation, it is not an unreasonable expectation of athletes to have progressed back to field training routines, where straight line running and light jogging is incorporated.

17.01.2022 Performance Testing Why do we test? Testing provides us with feedback on how an athlete is tracking through the training program. I test my athletes after a training cycle 3-4 times a season. This allows me to see how they are tracking with strength/power output, how they have improved or declined, what areas they need to work on and if the athlete is where they need to be at that stage of the season. Overall, it tells us what condition the athlete is in after that stage or... training/competition and provides a benchmark of what may be their best form. For example, if we have completed a heavy strength block the athlete should be lifting pbs when testing or close to them. However, is this applicable to their sport? What is their next phase of training going to consist of? I have tested athletes post major competition and found that the core lifts (ie squat) were not as strong but specific lifts (split snatch) were in PB form. Most importantly S&C results need to translate to the results in their sport. Does the program ultimately correlate to improving sporting performance? Testing should consist of the same exercices/movements/technology each time to have consistent and comparable results. Creating an appropriate testing protocol is vital. Exercises I use for testing may include Clean, Snatch, Squat, Bench, Chins, V-sitsin30s etc and/or a variety of specific movements applicable for the sport. 10.01.2022 Rehabilitating a Rolled Ankle Part 1. Rehabilitation strategies should begin during the same time if the ankle sprain is not so significant that it presents as too antalgic (heavy limping). Weight bearing such as walking, is important as early as possible to reduce swelling and increase ankle motion. Stretching and basic range of motion actions are also encouraged. Strengthening exercises are recommended 24-48 hours post injury avoiding the planes of injury or as soon as pai...n free motion can occur. The ROM stage of rehabilitation is a good indicator to whether pain free resistance is added as a progression. These non-weight bearing exercises will improve strength to muscles influencing ankle stability and movement. Part II coming soon - Balance and Proprioception.

Rehabilitating/strengthening the shoulder rotator cuffs/shoulder joint requires ideal scapular posture. Weak parascapular muscles and overactive superior/anterior muscles causes Scapular Diskinesia. ... 03.01.2022 Rehabilitating the Ankle Part III - Resisted Proprioception This group of exercises is essential to ankle rehabilitation, providing a low force, low impact exercise with high proprioceptive stimulation to strengthen the ankle joint. The injured ankle will be stationary throughout the exercise duration, however it shall be challenged to remain stable from external forces created by the opposite leg. The foot and lower limb muscular require isometric and eccentric contractions ...to maintain static equilibrium against multi-directional forces and varying resistance, enhancing the rehabilitation of muscles and neural pathways of the injured ankle. Any type of resistance band or tubing as appropriate to use, although the right selection of resistance is most important to ensure the exercise is completed perfectly. This will increase the effectiveness of the exercise and enhance the neuromuscular systems ability to rehabilitate the injured ankle.
