23.01.2022 How do we work out what nerve is causing the problem ? To get a good idea of what nerve we can use dermatomes, myotomes and reflexes to help us. Dermatomes: See pic below. This is the area of skin that the nerve supplies and gives us a fairly good idea of what level we are dealing with. So if you have pins and needles or numbness in the thumb its a good bet that C6 nerve is a problem.... Myptomes: This is where we test your muscle strength to see if the nerve is affected or getting compresssed. People who struggle to get on their tip toes and have an acute episode of back and leg pain may have an S1 nerve root issue.

21.01.2022 Achilles Heel Tendinopathy is the term used to describe pain at a tendon. It is not really an inflammatory issue. Current thinking is that it is more of a "reactive" tendon that has a failed healing response and doesnt tolerate force as well. Thats when it becomes painful. This image is one way of classifying achilles tendinopathy. depending on its location.... 1. Intramuscular 2. Free tendon or mid portion can be divided into three regions A. Proximal B. Middle C. Distal 3. Calcaneal or insertional tendonopathy is where the tendon joins into the heel.

This is a fairly extreme example of muscle guarding. the patient on examination ahs a classic stiff shoulder but under anaesthesia has full range. From this it is highly unlikely that the shoulder is "stiff " due to a contracted and fibrotic capsule.... More likely higher centres are involve , i.e the subconscious brain. See more

After breaking his arm this patient developed a classic frozen shoulder presentation. Loss of external rotation > abduction> flexion... In the space of 3-4 weeks of physio and exercise there has been a big change in functional range of this stiff shoulder .
