ROLE OBJECTIVES: Provide quality, timely and effective care that reflects the unique needs of each client and/or Participant Communicate well with clients and Participants with varied needs and backgrounds Promote the rights and privacy of our clients and Participants, in accordance with relevant policy and legislation Work for the continuous improvement of our service to ensure it’s the best it can be, in line with Aged Care Accreditation. Participate in ongoing professional development. Qualifications Must have: Minimum qualification - Certificate III in Individual Support or another relevant qualification. Current First Aid Certificate & CPR. Current police check Working with children card Location: SALE, VIC 3850 Specific task description: Monday (6pm-7pm) personal care to assist with showering. Wednesday (4.30pm-6pm) weekly assist with domestic care and from (6pm-7pm) personal care to assist with showering. Thursday assistance with social outings (support worker will need to drive the client’s car) for 3 hours and 45 minutes. Friday (6pm-7pm) personal care to assist with showering. This year, we’d like to take this opportunity to raise public awareness on #whywecare by highlighting the diversity of carers and the roles they have been taking. Carers can be health professionals, family members or friends who provide support and care to someone who needs help taking care of themselves to live independently and healthily. This role provides information, support, advice, advocacy and coordination services ensuring that the design and implementation of service options enhance customer's independence, control, wellbeing and quality of life in accordance with Consumer Directed Care principles. The key duties include: Assisting people with disa...bilities to remain living at home for as long as possible Support new NDIS participants to connect to appropriate individualised services Conduct assessments, develop support plans, undertake review, implement and facilitate clinical and care interventions Liaise effectively and positively with external stakeholders including carers, acute, respite and other community services, medical and allied health professionals Provide robust budget management, ensuring that accurate budgets are maintained so the customer can be supported as required Demonstrated understanding of needs and issues of people with a disability from CALD backgrounds and their carers Strong track record on project management and/or establishing new services Strong experience in community engagement and relationship building See more

05.01.2022 Language is a powerful tool in shaping and reflecting our thoughts, beliefs and feelings. A person is not defined by their disability, and it is important to use language that acknowledges this. Disability friendly languages can be used to create a sense of security, empowerment, pride, identity and purpose amongst people with disability. In AusCare we acknowledge and respect people with disability. We believe in empowering them and promoting the rights of people with disability by using disability friendly languages, regardless of their age, type of disability or cultural background. - Scenario 2
