24.01.2022 Prematurity and high narrow palates It is well known that premature infants are at greater risk of developing obstructive Sleep Disturbed Breathing (SDB). Thes...e sleep disorders are well linked to increased risk of developmental, neurocognitive and behavioural problems in children. We also know that narrow high palates are a risk factor for SDB. Last year, a research study of 244 premature infants exploring the links between high arch palates, the development of SDB and developmental outcomes at 2 years, was published by Huang et al. in the journal Sleep Medicine. *** What were the findings? 62% of pre-term babies had a high narrow palate at birth, compared to 10% of the control group (infants born 37-40 weeks). The high palate persisted during the two year follow up period. At the end of two years, 79% of the premature infant group had Obstructive Sleep Apnoea (OSA) compared to 10% of the control group. Within the subgroup of premature infants with narrow high palate, the incidence of OSA was 84.7%. Neurodevelopmental outcomes were assessed at 6, 12, 18 and 24 months using Bayley Scales of Infant Development and the Denver Developmental Screening Tests (DDT). There was a greater proportion of children with developmental delay in the premature infant group at every stage. Within the premature infant group, there was a significantly greater proportion of children with developmental delay in the narrow high palate group compared to the normal palate group. Overall, the data supports that high narrow palate is an important risk factor in the development of abnormal breathing. *** Why is there a link with prematurity and high palate? The last three months of pregnancy are critical for the fetus to train normal reflexes and prepare for the functions of sucking, swallowing, chewing and nasal breathing. Prematurity interrupts this training time, leading to incomplete muscle development. This includes the tongue, which needs to function correctly to stimulate the midpalate suture and properly develop the palate structure. The palate is the floor of the nose. When it is high and narrow, the nasal passages are narrowed and there is increased resistance to nasal breathing. It promotes nasal disuse and the development of mouth breathing over time. Mouth breathing is also associated with poor stimulation of palate development. The cycle of dysfunction and poor structural development is perpetuated. The palate is also an important part of the skeletal framework that supports our collapsible tube of airway muscles. A narrow palate offers reduced support. The upper airway is more prone to collapse during sleep when its muscles (including the tongue) are more relaxed. *** As a dentist involved in with managing adult OSA, I see the narrow high palate in many of my patients. I often find it despairing to listen to the impacts it has had on peoples health, mood and quality of life and wish the problem had been intervened earlier. I believe normalizing both orofacial muscle functions and palate structures as early in life as possible is a very worthwhile goal. Of great interest to me is that the same group of researchers has previously shared data demonstrating that myofunctional therapy exercises to stimulate more normal oral function helps normalise palate development and breathing over time. (Huang et al. 2012) They excluded a group of children who were having myofunctional therapy from the premature infant group in the 2019 published study for this reason. I have a special interest and am closely following the research in this area. I hope to learn more approaches with interdisciplinary colleagues to help this high-risk group in the future. I also hope to see more focus on prevention as early as conception and specifically through addressing maternal sleep disturbed breathing, a known risk factor in pre-term births. #AirwayMatters

Heat stroke can be... fatal for all animals, especially for pets who have short snouts, are overweight, or have thick/dark coats. No dog should be exercised or put in a hot car when the temp exceeds 34 degrees. Today temperatures in Sydney may exceed 40 degrees. Breeds like Frenchies, Pugs, Staffies, Spaniels, Boxers and Mastiffs are at a much higher risk of heat stroke. If you can, put out water bowls for wildlife and keep an eye out for any birds on the ground - if you’re able to catch a bird easily then it probably needs medical attention. If you see any bats on the ground please do not touch them - contact WIRES or Sydney Wildlife immediately and provide shade with an umbrella if possible.

12.01.2022 Research findings have revealed that the way children breathe affects their brain. Lack of oxygen and sleep deprivation have a major impact on a childs behavio...ur. When less oxygen is able to reach the brain and sleep is compromised, the childs ability to pay attention and concentrate at school is reduced, which may be mistaken for ADHD. As neuroscientist Matthew Walker states: It is clear that a tired, under-slept brain is little more than a leaky memory sieve, in no state to receive, absorb or efficiently retain an education Walker, M. Why we Sleep 2017. The National Sleep Foundation recommends 9-11 hours of sleep a night for school-aged children and 8-10 for teenagers, but Sleep Quality is as important as quantity. Disrupted sleep can reduce the time spent in deep restorative sleep and stage 4 REM sleep which is vital because it stimulates the areas of the brain that are essential in learning and memory. As of 2016, 6.1 million children aged 2-17 years living in the U.S. had been diagnosed with ADHD. MOUTH BREATHING DISRUPTS SLEEP Babies are born nose breathers, its the natural way to breathe. Children who mouth breathe are more likely to snore and develop Obstructive Sleep Apnea (OSA), a condition where breathing pauses for 10- 30 seconds or even longer (AASM). When this happens, the brain is alerted that there is a problem and arouses the child many times during the night, which makes them tired and irritable the next day. WHAT CAUSES MOUTH BREATHING? A child with a constantly open mouth will very likely grow into an adult with flatter facial features, less prominent cheekbones, a longer face, droopier eyes, a narrower palate and even a small lower jaw. Maintaining nasal breathing can prevent negative growth patterns like these. If the cause is huge tonsils then removing them might be an option. If the problem is structural and a child cant close their lips over flared front teeth then the solution may be orthodontic treatment. A thorough dental or ENT consultation will help determine whether mouth breathing is a problem. CHECK YOUR CHILD IN THE MORNING. Getting up in the morning should not be a daily battle. 03.01.2022 Teeth grinding in children Did you know sleep bruxism or teeth grinding during sleep can be a red flag for breathing disturbances? Episodes of teeth grinding co...mmonly occur in the transition from deep sleep toward light or REM sleep. They are associated with brief arousals lasting a few seconds where the brain, heart and breathing are reactivated and there is a rise in the muscle tone. These episodes of increased jaw muscle activity are thought to offer some protection against the complete collapses of the airway that define Obstructive Sleep Apnoea (OSA). New thinking is that rather than being related to psychological stress, teeth grinding can be a response to the physiological stress of the threat to breath. A study by Australian dentist Dr Nisch Singh demonstrated that the degree of sleep bruxism related tooth wear in childrens teeth was associated with their Respiratory Disturbance Index (RDI) a measure of their degree of OSA plus the more subtle breathing disturbances related to increased effort of breathing to maintain an open airway. The results of this study suggested that sleep bruxism was a marker for breathing disturbances, but there was a closer relationship with the effortful breathing than the complete collapses of the airway. This means that it is important to pay attention to your childs sleep and breathing, and be aware of other red flags that could indicate unrestorative sleep. If there are any underlying breathing disturbances, these may be addressed. One of the common risk factors for obstructive breathing in young children are enlarged adenoids and tonsils. Research tells us that approximately 75% of children who grind their teeth and have their adenoids and tonsils removed will stop teeth grinding. That leaves a quarter who will continue (Morton 2008, Efketkharian et al 2008). We know that sleep breathing disturbances are multifactorial and removing of adenoids and tonsils is not curative for most children, particularly in the long term. These problems require a team approach. A dentist with a special interest in airway will assess for other risk factors. For example in this child with severe tooth wear related to teeth grinding, we assess The palate the deep thumb-print indent represents a narrowed nasal floor and airway with increased resistance to nasal airflow. Their oral rest posture in this picture we see open mouth posture and a tongue thrust or lowered tongue posture. If a child has their lips apart during the day, we know that this will be the case at night when all the muscles are more relaxed. As the jaw drops open, it rotates backwards and so does the tongue, impinging the airway space. The way the tongue functions optimal function includes the capacity for the tongue to suction to the palate - this is the most stable position for breathing and the airway during sleep. We assess for tongue control, tone and tongue ties that could be restricting this function. What about sleep bruxism and medications? There are also certain stimulant and anti-depressant medications where teeth grinding is a known side-effect. It can still be helpful to rule out any underlying breathing disturbance. There is a growing evidence base suggesting an association between ADHD and mood disorders in children with obstructive sleep. Ideally, it would be good to rule out any sleep breathing problems and unrestorative sleep before committing to long term medications. #airwaymatters
