14.01.2022 Q Fever Treatable and Preventable - NDL Article There are some great things about living in rural and remote Australia, but Q Fever isnt one of them. Q Fever is an infectious disease caused by the bacterium Coxiella Burnetii which is spread from animals to humans. The disease has a wide range of severity, including significant impact on short- and long-term health.... The highest risk is from handling cattle, sheep, and goats. In particular handling birthing fluids, faeces, urine, and waste products. However, the bacteria can be blown in the wind through contaminated dust, or caught from other animals such as rabbits, kangaroos, or ticks. It is thought that 6% of people living on farms and 3% of people living in rural areas will be exposed to Q Fever. Classic acute Q Fever appears as a flu-like illness with high fever, headache, aches and pains, with lung and liver inflammation. In its severe form, it can affect the heart or appear as meningitis. Five percent go on to develop chronic infection which may result in damage to blood vessels, heart valves, liver, or bones. Ten percent develop Q-Fever Fatigue Syndrome which can limit activity for months to years. Prompt recognition, diagnosis and antibiotic treatment (generally doxycycline) can reduce the length of illness and the likelihood of complications. Transmission is reduced by good hand hygiene and personal protective equipment (including P2 masks). However, vaccination is the most effective way to prevent infection and is recommended for those who work in, live in, or visit high-risk environments. Two appointments are required for vaccination. The first includes a brief medical consultation and assesses for prior exposure by skin-prick and blood test. Fewer wet nappies or reduced urine output is a clear sign of dehydration, along with dry skin with reduced turgor, sunken eyes, dry mouth, and drowsiness.

09.01.2022 Sleep is an active process that is essential for our body and mind. It is of such importance that reduced sleep is associated with deterioration in immunity, blood sugar control, obesity, cardiovascular disease, Alzheimers disease, mental health and sports performance. However, sleep for many of us is a difficult thing to achieve. The reasons for this vary from individual-to- individual. Major contributors include stress and poor sleep hygiene. Some people may physical cond...itions including obstructive sleep apnea which impact their sleep. Sleep hygiene refers to the practices and habits that promote good nighttime sleep quality and promote daytime alertness. This includes having a good routine (day / night pattern, and evening wind-down), have the bedroom cool, dark & quiet, avoiding caffeine, nicotine & alcohol, exercising early in the day, and intentional relaxation (including meditation, yoga, and mindfulness exercises.) If you are interested to learn about the science of sleep. Matthew Walkers book Why We Sleep is an excellent place to start. You will find information on the 90min sleep cycles we experience, the different purposes of REM and non-REM sleep, sleep disorders, the role of adenosine and melatonin and how that is all influenced by lifestyle choices and medicines. Our bodies are primed to adapt to challenges we set it, and remarkably, it is moving that makes you better at moving. The words movement, exercise and fitness conjure all sorts of images from bodybuilders to marathon runners . The truth is that these images are often the extremes of a single aspect of fitness. Going to these extremes is amazing but neither r...equired for health nor represent the spectrum of physical activity. Movement and fitness include the general physical skills of cardiovascular/respiratory endurance, stamina, strength, flexibility, power, speed, coordination, agility, balance, and accuracy. Fitness, and therefore health is improved as we develop across all these domains. If youre good at some, take time to work on others. Long-distance walking, running and cycling improve cardiovascular and respiratory endurance and stamina. Shorter distances improve power and speed. Rowing and swimming use more and different muscles. These activities improve insulin sensitivity, help maintain weight, and reduces the risk of diabetes and cardiovascular disease. Resistance training with weights, body weight and resistant bands improve muscular strength. Physically active jobs and activities like gardening will also develop strength. Over time this improves bone density and reduces the risk of osteoporosis. Various sports improve coordination and agility, as well as having significant social and mental health benefits. They naturally incorporate high-intensity impulse training in a fun and engaging manner. Yoga and gymnastics improve flexibility and balance. Consistency is important. Be active on more days than not - 3-6 sessions a week, lasting 30-60min. Good technique keeps it safe. Be smart about loading or difficulty. If youre not sure ask a professional fitness instructor or exercise physiologist. Routine is the enemy of adaptation. Mix it up. Challenge yourself. Being intentional about moving today, will help keep you moving tomorrow, the next day, and into the years to come.

08.01.2022 In 2017 the "Pap smear test" was replaced with the new "Cervical Screening Test." This is a smarter test that looks for the causative HPV virus before cancer changes have even begun. Most women aged 25-74 should be tested every five years. The risk of a woman being diagnosed with cervical cancer by age 85 is 1 in 162. Death rates of cervical cancer in Australia have halved since the National Cervical Screening Program began in 1991. 05.01.2022 "Doctors are being asked to provide medical clearance to patients who have completed a 14-day period of self-isolation due to COVID-19. NSW Health has provided advice stating that doctors are unable to issue medical clearance certificates because there is no testing that can be conducted to predict whether or not patients will become unwell if they are currently symptom free (view NSW Health coronavirus FAQs). The only type of certification a GP can provide is to the effe...ct that, on the day of assessment, the patient was symptom free (this relies on a patient history and the patient being honest) and had no clinical features to indicate a viral infection. These are relatively limited but would include normal temperature and heart rate, nil respiratory distress, clear chest, no rhinorrhoea or pharyngitis." If a patient presents with the requisite travel history and provides the GP with a history of having been in isolation for 14 days, a doctor could write a letter to this effect: I assessed on which they state is 14 days since their self-isolation. On assessment today, they did not present with any clinical features suggestive of a viral infection. This decision was reached to provide additional protection to vulnerable populations (patients and staff) due to the increased risk of community transmission. We are still working through how this applies to General Practice, but if 1.5m social distancing is not possible then masks will be required.
