18.01.2022 If there was one ritual Id like to see banned before 40 weeks, its the stretch-and-sweep, aka membrane stripping. The routine stretch-and-sweep is performed d...uring a vaginal exam at any time beginning around 37 weeks. Mothers are seldom told it was done. A stretch-and-sweep involves having the providers gloved fingers manually stretch open the cervix and then strip the babys amniotic sack away from the cervical opening. The babys amniotic sack is normally adhered to the opening of the cervix in a way that prevents infectious agents from reaching the baby. Its one of pregnancys many safe-guards. Its believed that manually dilating the cervix and separating the sack from the opening of the cervix will release extra prostaglandins that could stimulate labour. Only prostaglandins dont trigger normal labour. Its a hormone that ripens the cervix. Its actually the babys lungs that trigger labour when they secrete a protein just when theyre ready for air-breathing. Then oxytocin from mamas brain brings on contractions. A membrane stripping is intended to "prevent induction". What your provider is saying to you (if they tell you) is: "I will perform a mechanical induction to start labour artificially so that you might avoid a chemical induction after your due date with IV oxytocin in the hospital." Yes. A stretch-and-sweep is an induction that is meant to "prevent" induction. It is meant to bypass your babys lungs and all of the safety mechanisms of human birth to get the baby out before the baby is ready. It can hurt. It can make you bleed. It can cause your water to break meaning you now are expected to go to the hospital for a chemical induction. It can introduce infection. Some studies say that doing a membrane stripping doesnt shorten pregnancy. Other studies say it can shorten pregnancy by a couple of days. 17.01.2022 "In a study of 3079 children evaluated at 10 years of age, gestational age at birth was linearly associated with global and regional brain volumes. Longer gesta...tional duration was associated with larger brain volumes; for example, every 1-week-longer gestational duration corresponded to an additional 4.5 cm3/wk (95% CI, 2.7-6.3 cm3/wk) larger total brain volume." What does that mean in lay terms? Simple. There are advantages to waiting for your baby to decide when to be born. And one of them is that waiting longer means their brain volume can develop more. So much so that a difference can still be seen ten years later. For every extra week the baby stays in your womb, the brain grows by an extra 4.5 cubic centimetres of brain volume. Another good reason to weigh up ALL the pros and cons of interventions such as induction of labour and elective caesarean section before making the decision that is right for you and your baby. 13.01.2022 Did you get an episiotomy? have you witnessed one being performed? Did you understand what was about to happen because the procedure was completely explained a...nd consent was sought?? Here is what I have seen in 12 years as a midwife: the woman is pushing her baby out, there is some kind of unwelcome delay from the perspective of the care provider and they choose to do an episiotomy (cutting through the vulva to mechanically enlarge the vaginal opening), the practitioner says Im going to cut you/give you a little snip to get this baby out, ok?... the woman may or may not give the practitioner the go ahead, she may or may not even know what little snip means ... but she gets an episiotomy anyway. Some practitioners believe that if a woman looks like she is about to have a significant perineal tear during childbirth then they should cut an episiotomy BUT we know that when we reduce episiotomy rates we also reduce the number of women experiencing severe perineal/vaginal trauma (seems common sense, but anyway, there is research on that) Making a decision on the spot, whilst pushing out a baby is not the time to decipher if you want an episiotomy or if you even need one ... make educating yourself about episiotomy a priority during pregnancy so you are clear about what you want and know before going in... and as always, your choice has supreme authority . 11.01.2022 Respect your body. Respect means caring for it, appreciating it, seeing it, recognizing the work it does, feeding it. You may not be happy with your body, you c...an have disappointments, you can struggle to love it. But to respect it you have to ask where those thoughts come from and are those values more important than the value your body brings? We're all in different places in our journey and I won't shame anyone struggling to love or respect their bodies. 06.01.2022 Wrapped up in love DID YOU KNOW that men’s hormones change hugely once they become fathers? For so long (and understandably!), we have focused solely on the h...uge rollercoaster of emotional and hormonal changes that women go through when they become new mothers. But men experience this too! Let’s start with testosterone. Testosterone is the ‘masculine’ hormone that we all know about - it’s role is mostly to motivate men to find female partners and to reproduce. Studies show that men with higher levels of testosterone tend to be more attractive to potential mates. So, the theory is, when men find their partner and have a baby, their testosterone levels drop as they’re no longer needed to be so high! Multiple studies have found that men’s testosterone levels are lowest just before and straight after their baby is born. And then there’s oxytocin... the wonder love hormone that we talk about so much! Studies in both animals and humans show that once a father, a man’s levels of oxytocin (the happy love hormone), estrogen and prolactin all increase! What’s more, fathers who interact more regularly and lovingly with their children have, unsurprisingly, much higher levels of oxytocin. I say to clients, its not like you need to rush anywhere, just let that cord pulsate ...until its finished its work to send all that oxygenated blood that your baby needs to thrive outside of the womb. #Repost @vintagedoula "We know so little. First it was 30 seconds, now ACOG recommends (gasp) 60 seconds, but studies show 5 minutes can change brain development. Now imagine if we actually just left it alone and allowed 5 minutes to turn into and indeterminate amount of time decided not by our egos or policies but only by the anatomy of the cord. What if we only clamped the cord once all the blood and stem cells transfused from the placenta to the baby, as is the position of most midwives, and is definable when the cord turns white, cold and limp, and the arteries and veins truly shut down... 04.01.2022 Homebirth isnt homebirth at all costs. And homebirth is not necessarily about the place of birth. Jodie and Tim planned a homebirth for their first baby... supported by Midwife Jo and myself. Jodie (a midwife herself) wanted above all to give birth in a safe environment avoiding any unnecessary intervention. Her labour was long and tough. She had utilised a TENS machine, the shower, the pool, heat packs, long walks with stairs, rebozo- we tried it all. Jodies back went into a crippling spasm in the shower whilst in established labour causing not only intense pain, but a sense of overriding anxiety. Many, many hours into labour, Jodie made the decision to go to hospital for pain relief. We had worked together at home as a team and we went to hospital as a team. We were greeted warmly and welcomed with kindness. Jodie had an epidural and got immediate relief. We dozed, we laughed, her labour continued to progress. And then, when it came time for her to push out her baby the beautiful hospital midwife handed Jo a pair of gloves and said- Youre her midwife. You should catch her baby. This is collaboration at its finest- something so small which is actually so significant- maintaining the continuity. Jodie pushed her baby girl out like a champion, right into Jos hands who placed her baby on her chest. Jodie came out of this birth wishing her labour wasnt so long and gruelling, but not wishing that she had made different choices in the labour she did have. Even though she didnt give birth at home, she did all the hard work in the place she felt most comfortable. Everyone knows birth doesnt always go to plan. And this is where the importance of care comes in- to have continuity of care with a midwife (and a doula, in this case), where you have explored every option, made decisions that were respected and supported and all of your caregivers worked together- is a great leap in the psychological and emotional recovery from a birth that didnt go the way you hoped and planned for. Homebirth is one of the only places in this country you can access true, one to one midwifery care. 02.01.2022 Epidurals... they have a place in birth, just like any intervention, but just like any intervention they also come with risks. Epidurals are not something to ta...ke lightly! A recent study comparing first time birthing people who had an epidural vs those that did not found that those having an epidural were: were more likely to require an instrumental birth significantly greater risk of 10 hours or more first stage of labour longer second stage of labour increased likelihood of receiving IV syntocinon to augment labour increased likelihood of receiving antibiotics increased likelihood of fever half as likely to be breastfeeding at three months postpartum No differences were observed between groups in neonatal outcomes. Making an informed decision is key. Just with any option with birth, theres no right or wrong decision, you decide whats right for you and your birth. The above are risks, not certaintys, but its important that you know them before choosing an option.
