23.01.2022 Merry Christmas everyone! I will be on leave from Thursday 24th Dec until Tuesday 5th Jan. I would like to take this time to thank all of the lovely people I have seen this year, for making my job the best job in the world. 2020 has been the toughest year so far, and watching so many new families adjust to life with a newborn, during a pandemic is a testament to how strong we all can be! I invite you all to share a picture on this post of your 2020 baby!! I will start off with my 3 little people. Merry Christmas

23.01.2022 Im Back!! As of this week, I will be commencing a home visiting service again- in accordance with the current Covid 19 regulations. I will still be offering video and phone consultations to those postnatal patients who request or prefer this service until September. To ensure the safety of everyone, any sickness must be reported before the visit and where applicable we will remain 1.5m away from each other to assist with social distancing. Happy to be back in the driving seat!

Im sorry for all the madness I put you through, for all the times Ive screamed at you in the middle of the night when the baby woke up for the... 10th time, for all the date nights we have to cancel and for all the times I forgot to tell you how amazing you are, just because I was too exhausted to speak. . . Im sorry for all the times Ive pooped with the door open, even though I promised wed always do our best to keep the romance alive, Im sorry for all the times Ive kept score of who changed the most diapers or whos turn it was to do the dishes. . . This isnt our forever my love, this is just our beautiful chaotic present. This is the hard part, the part where were rushing through life, trying to be enough for each other and our kids - the part where we grow, the part where we test our relationship to the fullest. . . Its the part where I need you the most, the part where I need you to love me a little extra when I least deserve it, because its still me, underneath the sleep deprived monster you meet in the morning, its still me, its still us, it all started with you and it will all end with you. . . One day well miss this, even the chaos, we will have time to actually go on a date night again, our kids wont need us as much then, but I will always need you my love. So thank you - for choosing me day after day even though Im not always my best self, if our love can be this great today, in the middle of all the chaos, can you imagine how beautiful it will be in 10 years when we have time to finish the laundry? @saracelinaa . 21.01.2022 Due to the recent COVID restrictions, I have put home visits on hold for the time being, but I am still very much working and providing care via video and phone conferencing- at no charge (conditions apply). 20.01.2022 Another day another update. Everyone will need a bit of patience over the next few days whilst we figure all the new directives out ... For those coming into th...e rooms you will need to check in at the front of the hospital as usual but now you will need to scan the the MySAGov app for the QR code check in. We have been advised that as the hospital will have one we don’t need another one at our front door (phew!!). Single use 3 ply hospital masks are now required as per SA Health - cloth masks are not ok ..... Whilst we will need to see how it plays out in the hospital the directive does say that mask need to be worn by EVERYONE whilst in the presence of others (presumably non family) - however I can only assume that this won’t apply in labour for those deemed at low risk of covid (which to be fair should be everyone that gets through the front door as if you are high risk you will be diverted elsewhere) .... I don’t really understand this change to be fair but I don’t get to decide what we do If anyone has any concerns about this please call our rooms in the morning - a phone consult is always an option in the next few weeks - hopefully it won’t last longer than that Hang in there peeps - we are all in this together Addit - the Apple version of the app won’t be ready until tomorrow afternoon so before then it’s a manual sign in or a normal QR scanner will work and you get redirected to the sa gov site to put in your info See more

18.01.2022 Just to keep everyone updated about the maternity visitor restrictions - as of today 2 people are allowed to visit per day (anytime, for however long they like,... can come and go) and these 2 people are allowed to be different each day (rather than the same 2 for the whole stay as it has been). Sibling visits remain unchanged at this stage. We are still awaiting an update on antenatal classes and are considering doing some in our rooms as an adjunct if the hospital is unable to find a platform that is suitable/acceptable - stay tuned See more

13.01.2022 When a mother says shes tired, thats all she meant ! She did not say she wants to drop her child and forget that the child exists. When a mother says she wan...ted some time alone, just her, thats all she meant . She didnt say shes sorry to be a mother and that motherhood was a mistake in her life. When a mother says she needs help to get things done, thats exactly what she meant. She is not incapable. When a mother makes noddles for dinner, it does not mean she does noddles everyday of the week and that her daughter/son is a child who does not know vegetables or meats. When you arrive at a mothers house and face disorganization, it doesnt mean that everyday the house is disorganized. When a mother says shed love to go out with her friends, thats all she meant. She doesnt want to go back to being single/ not a mother and like it as if she had responsibility. When a mother says she worried or afraid, thats what she meant! She didnt say shes going to falter and much less a coward. When you listen to a mother screaming, it doesnt mean she just screams, probably she has spoken 300 times with normal voice tone. When you see a nervous mother, on the edge of madness, doesnt mean everyday shes in that situation. Do not invent untruths to judge someone, let alone a women who every day, at all times, gives up her own life to live life of another being that she knows is much more important than her. In the world there is no human being able to give up and donate as much as a mother. And we deserve a lot of understanding and fewer trials

12.01.2022 NO. @kickscount Despite what you may have heard from a friend, relative or even a health professional (as I've been told in some circumstances), an unborn baby ...does not 'slow down' towards the end of pregnancy. . Never ignore reduced baby movements. Never feel that you are 'bothering' your care provider. Always trust your instincts. By around 26-28 weeks your baby may develop a regular pattern of movements. If at any stage you feel that baby either hasn't moved for some time or is moving but a lot less that normal, call your care provider straight away for advice. Babies will have periods of sleep and being awake when they are active. This will continue right up until they are born. . If you are genuinely concerned about lack of OR reduced movements, don't sleep on it and see if baby 'wakes up a bit' in the morning. . Even if you have previously been checked over but you still feel that baby isn't moving as normal, still seek advice. Remember, babies DO NOT move less at the end of pregnancy. Their movements may CHANGE from sharp kicks to sliding and rolling movements which may not feel as intense so it can be easy to assume they are not moving as much. You may also feel the baby's head twist and turn in your pelvis. This is still activity and the frequency of these movements should still remain the same. An unborn baby does not suddenly decide not to move their limbs or head as much at the end of pregnancy. . I'm always pretty frank when talking about baby movements because it's so important to seek advice if you have noticed a change. The majority of babies will be absolutely fine when getting checked over but occasionally reduced movement is a sign that baby is in distress and needs to be born. See more

06.01.2022 There is no safe level of caffeine intake in pregnancy. That is the conclusion of this narrative review of caffeine safety in pregnancy (BMJ Evidence Based ...Medicine, Open Access) which a patient brought to my attention very recently after hearing about it on the news. I felt that it required clarification to avoid concern. The single author concluded that, after finding 48 studies (37 observational studies and 11 meta-analyses), caffeine intake in pregnancy significantly increases the risk of miscarriage, stillbirth, low birth weight, childhood leukaemia and childhood overweight/obesity. The author then goes on to recommend that all worldwide guidelines (including American, UK and Australian) stating the safety of caffeine in doses<200mg/day (approximately 2 cups of coffee) should be revised to say there is no safe level of caffeine in pregnancy. Fortunately, there is no need to panic. This paper is far from as conclusive as it might make you believe, but serves as a good example of how language can be used to persuade when evidence does not. Firstly, this study is not a properly conducted systematic review or metanalysis. Literature from only PubMed and Google Scholar were searched. The search was not systematically or transparently documented and there was no reference to the PRISMA guidelines (a minimum set of items for reporting systematic reviews and meta-analyses). This is important as it means that a significant number of studies will have been missed in the search, introducing significant reporting bias (studies with results showing caffeine is safe in pregnancy may not have been included). The review looks at both observational studies and meta-analyses and it is not clear whether non-independence has been accounted for that is, some of the individual studies may have been included in the reported metanalyses. This would result in duplication of the data and an overestimation of the effect size. Because the review was not systematic, there is no detailed analysis of confounding factors (ie. Women who drank alcohol or smoked as well as drank coffee during their pregnancy which could also effect the outcomes). The variation between the studies have not been reported or investigated (therefore, cannot be compared or summed). This heterogeneity has not been identified or reported. There is no consideration of publication bias (when only studies with positive results tend to get published), and even more importantly, no analysis of bias in each individual study (which is mandatory for any meta-analysis). The author appears to try to make conclusions based on the number of studies that found an effect instead of the number of participants that were effected. It also doesn't account for the quality or lack of quality of each study. Even if a lot of studies find an effect, if they are not well designed then the effect may not exist. This paper seems to be written without any balance, making one concerned about the agenda of the author. The author states that the safety of caffeine may be backed by caffeine corporations (coffee and soft drink manufacturers). Indeed, a quick Google search of the authors name shows that he has written two books about the dangers of caffeine. While this does not mean that this report can be discounted, it does give some insight into the motives of the author. Interestingly, one randomised controlled trial which found no affect of caffeine in pregnancy is briefly mentioned in the discussion of the study, but quickly discredited. SUMMARY: In short, this article is really an opinion piece. Dont put down your coffee cup just yet. It is safe to consume <200mg/day of caffeine during your pregnancy. In these times of media frenzy, please read your evidence and advice with care.
