22.01.2022 I get asked from time to time to give concrete examples of systemic racism in maternity care. Here's one that came across my desk today. I've been looking at ...one of the Anti-D leaflets handed out in the UK. Not only is it full of pictures of white people, but it says that. "About 85% of people have the RhD Factor, and the other 15% do not." I have been researching this area for more than 20 years, so I can tell you that that figure is only true for people of European descent. In fact, one per cent of people of Asian descent, eight percent of people of African descent and 15 percent of people of European descent are rhesus negative. The fact that we give out information leaflets containing data and pictures which only represent and relate to white people is systemic racism. 21.01.2022 Some babies are born very purple. About 10 seconds after birth, they will take their first breath, which will inflate their lungs and start the oxygenation process. ... As they continue to breathe air, this color will turn more red, and then into pink. The hands and feet may retain a bluish hue for a bit longer, which is normal. 17.01.2022 That cord There is a lot of impatience in maternity care when it comes to birthing the placenta. It is often rushed, managed and medicated. The cord is often clamped and cut after a few minutes (or less) and an injection (pitocin- which you can decline) given to help release the placenta.... Once baby is born tho, the placenta is still at work - supplying baby with oxygenated blood. Up to about a third of babys blood is still in the placenta, so it continues to transfer this to the baby through the cord. Clamping and cutting the cord before the placenta has finished pulsing means that baby is denied some of its blood volume. This blood is beneficial for iron stores, brain and myelin development. Baby will also benefit from increased stem cells. Usually the cord will continue to pulse for 2-20 minutes. But everyone is different and sometimes the process takes much much longer. On the weekend I attended a beautiful home birth where the cord pulsed for an hour and a half! Yes 90 minutes! It was 2 hours before the placenta was born. The midwives patiently watched and waited. Birth is so individual. It does not fit into boxes, schedules, ideals, protocols, rigid rules or timeframes. Birth is the teacher. 17.01.2022 Many have seen a dehydrated umbilical cord keepsake before. Many mums choose to have this done when I’m processing their placenta for encapsulation. I wanted to show this one because it’s a great example of a very well drained cord as a result of delayed/optimal cord clamping.... You can clearly see the outlines of the 3 cord vessels: 2 arteries and 1 vein. There are a few spots that have clotted or residual blood in them which is normal but the vast majority of this cord is empty This means it's all gone into the baby Where it belongs and where it's physiologically designed to be. If you haven’t learnt about optimal cord clamping and ‘waiting for white’ please look into it. Once you know the importance of your baby beginning life on land with its full blood volume (not just two thirds) you’re more likely to do all you can at birth to ensure ain't nobody severing that lifeline before its the right time! Be your baby’s advocate and remind your caregivers of the oath of ethics (whether its Hippocrates oath or not) .... first, do no harm
16.01.2022 If you're currently pregnant and finding yourself wondering how your vagina and peri will fare through the birth, you're not alone! It's a super normal concern - and a valid one at that! From 20 years of experience, I can confidently say it's not all up to chance.... I'm a big believer in the body's ability to birth babies. As a doula I've attended births of 10 pounders that leave not a trace. I birthed my 9 pound baby complete with a nuchal hand after a 1.5 hour labour, with an intact peri. But I've also seen mums of 7 pounders receive episiotomies or experience 2nd degree tears. And pretty much everything in between. What I do know is that there are M-A-N-Y factors around birth that influence how things unfold for your lady bits. So I've put together a list of these *influencers* plus some things to consider both before and during the birth itself. 16.01.2022 Why do I Iove using herbal tinctures? By definition, a tincture is a liquid herbal extract made by soaking herbs in solvents such as pure grain alcohol, which helps extract the active ingredients from the herbs. The liquid is strained out and the herbs are discarded. A tincture can be made of one herb or a combination of herbs. It is considered to be one of the most effective tincture forms because the alcohol itself extracts the medicinal properties of the plant better t...han any other solvent. Alcohol also has great preservative properties and when stored properly, alcohol based tinctures can be kept for up to 2 years. The fact that they are an alcohol based preparation has some great benefits (note: you can also get glycerine based tinctures, great alternative for certain herbs, children, those who prefer/cannot have any alcohol). The alcohol extraction method allows for a high potency preparation. Which means, you don’t need to take much to get the medicinal effect. This can vary from drop dosing to mls. Because of the alcohol the absorption is rapid in the gut. They can also be taken under the tongue, with a little water, for rapid entrance into the bloodstream. Tinctures are also easy and convenient to use. Some herbs are used to have an immediate/direct effect, (e.g digestive system) whilst others are used for their nutritive and accumulative effects ie. you may take them continually for several weeks before results are anticipated. It is a big relief to find out the baby is not breech (buttocks coming through the pelvis before the head is born), especially when so many breech babies are born by cesarean these days. ... Senses are intensified in order to connect you to the feel and smell of your baby.... Putting a hat on your baby can disrupt this process. Skin to skin is sufficient in keeping your baby warm. You NEED to smell your baby. Your baby NEEDS to smell you. It’s vital for trust, bonding, nursing, and birthing the placenta. Your baby is attracted to your scent, searching for the familiar smell of amniotic fluid that can be found on your areolas and armpits. 13.01.2022 Diastasis recti happens when the left and right abdominal muscles weaken and stretch to the side, as in pregnancy. The condition is incredibly common (60-70%) of women who’ve been pregnant experience some degree of diastasis recti), but even so, most women don’t know how to get rid of it. Typically, solutions involve surgery or intense workouts, but those aren’t necessarily the best options for everyone. In fact, they can sometimes lead to the condition returning - or worsening.

09.01.2022 Did you get to see your placenta? After having 2 previous caesarean births, Brooke had her VBA2C and she got to see her placenta for the first time. We kept baby Hallie's cord attached until the placenta was birthed - she had a physiological placenta birth also (meaning we waited until her body was ready to release her placenta, which was around an hour after her birth).... Caesarean mums, did you know you can request your baby and placenta remain connected? A number of my clients have had the experience of cutting the cord together when back in their maternity room, when the environment is calm and private - reclaiming another cultural ritual of a parent being the person to 'release' the baby into the world and welcoming them to their family. 08.01.2022 It is our natural instinct as mammals to birth in the dark. We need the combination of natural sunlight during the day and natural darkness at night to produce melatonin. While melatonin is the hormone that regulates our sleep, during labor it synergises with oxytocin to enhance birthing waves.... This is why often, birth surges are stronger at night ( and we birth workers are mostly night workers) For this reason, darkness is one of the vital conditions that help ensure a smooth birth. There can be candles, lava lamps, string lights, but ultimately darkness is what creates an ambiance of softness and comfort. Security and peace. This tiny bubble of time, the hours after birth, are some of the most magical hours I have ever witnessed. 05.01.2022 My go-to herbal support for THIRD STAGE OF LABOUR and IMMEDIATE & EARLY POSTPARTUM I often use the COMPLETE Postnatal Herbal Tea with my doula clients, particularly (but not exclusively) at homebirths. As mum is getting pushy or actively pushing I’ll get this herbal tea on the brew. At homebirths I like to use a good ol’ teapot or a plunger for this. This is something we chat about during our prenatal visits so I often know what they have at their home and where things a...re stashed! At hospital births I’ve been known to pack my own plunger and fetch boiling water from the parents room or staff’s urn By the time baby has arrived, the tea has been well brewed and is a good drinking temperature for mum. I will offer it through a drinking straw. It can work absolute wonders in bringing placentas out smoothly and all in one piece Then mum can keep brewing it in those first few days after birth to help take the edge off the afterpains and curb her postnatal blood loss, as the uterus contracts back down to pre-pregnancy size. The Mama’s After Pains Relief is for use from the day of the birth. I often tell mums to have their first dose with their first decent meal after baby have arrived. This also contains anti-spasmodic and tonifying herbs specific to the uterus. Research reports acupressure as beneficial in labour and reduces caesarean sections. It has also been reported to reduce the need for epidurals and caesarean sections when used as part of an antenatal intervention.
01.01.2022 Most breastfeeding mothers can drink caffeine in moderation. Some babies, particularly those under 6 months, may be more sensitive to mum’s caffeine intake. Babies whose mothers avoided caffeine completely during pregnancy seem to react more to caffeine in mum’s diet. Even if baby is sensitive to the caffeine now, he may not be when he’s a little older - so if you do have to stop or limit your caffeine intake, you can try again when baby is older....
