25.01.2022 ACL RECONSTRUCTION IS NOT SUPERIOR TO A STRUCTURED REHAB PROGRAM Frobell (2010) in the Knee Anterior Cruciate Ligament Nonoperative vs Operative Treatment (KA...NON) Trial randomised 141 subjects with ACL tears to rehabilitation plus early reconstruction, or rehabilitation plus the option for delayed reconstruction. At 2 year follow-up there was no significant difference between the groups in pain, symptoms, function in activities of daily living, function in sports and recreation, knee related quality of life, or return to pre-injury or higher level of sport. Further analysis at 5 years found no significant difference between groups for meniscus surgery, time to meniscus surgery, change in KOOS scores, or radiographic knee osteoarthritis (Frobell, 2013). Filbay (2017) performed an analysis of the KANON Trial to determine prognostic factors for 5 year outcome following ACL rupture. 24.01.2022 ACL SURGERY HAS RISKS Following ACL injury most patients are encouraged to have surgery to restore their knee anatomy, however many patients are not we...ll informed of the possible risks of surgery and thus are unable to make an informed decision. There are multiple complications which can occur as a result of surgical reconstruction! Gobbi (2015) report infection occurs in 0.37% on average within 7.5 - 61.7 days post surgery. Whilst the rate of infection is not high, infection is a very serious which can have serious consequences. 22.01.2022 ACL SURGERY DOES NOT PRESERVE YOUR MENISCUS, REDUCE REINJURY, OR PREVENT OSTEOARTHRITIS Individuals are often told that they need ACL surgery or they are at g...reater risk of meniscal injury, at high risk of subsequent knee injury, and at greater risk of developing osteoarthritis, and that the surgery will mitigate these risks. This is misleading, and untrue! Risk of reinjury is very high, even post-surgery, with estimates up to 25% re-rupture rate over 15 years, and up to 16% within 3 months of return to sport (Bourke, 2012; Kyritsis, 2016) Frobell (2013) examined 121 patients at 5-year follow-up who had been randomised into either rehabilitation plus early reconstruction, or rehabilitation with the option of delayed reconstruction. At 5 years they found no significant difference in the development of osteoarthritis, meniscal injury, or time to meniscal injury. 21.01.2022 ACL SURGERY DOES NOT GUARANTEE RETURN TO SPORT A common expectation from patients is that having ACL surgery will guarantee the ability to return to pre-injur...y level sport. Unfortunately this is not the case, and many patients do not return to any sport, even less return to pre-injury level sport, few return to competitive sport, and very few return to pre-injury performance. Ardern (2014) performed a systematic review and meta-analysis of fifty-seven studies evaluating 5770 participants, at an average follow-up period of 40 months only 65% of participants had returned to pre-injury level sport, and only 55% had returned to competitive sport. Webster (2018) evaluated the rate of return to level 1 (cutting, pivoting, jumping) sports of 1440 patients within 12 months following ACL reconstruction. They found the overall return to level 1 sport within 12 months was 39.5%. Athletes 25 years or younger were more likely to return to sport than older athletes with 48% returning within 12 months, and males were 1.7 times more likely to return than females, however, the rates were still significantly low. Liptak (2017) examined the return to sport rate, and return to performance rate of elite AFL players following ACL reconstruction between 1990 and 2000. They found the return to elite AFL was 74%, 26% of the athletes who returned to sport suffered reinjury. Furthermore, on average, players had not returned to pre-injury level performance within 3 years. Lai (2018) performed another analysis of return to AFL following ACL reconstruction between 1999 and 2013. They found that 77% of athletes returned to the AFL for at least 1 game, however, 30% of athletes suffered reinjury. As much as I would love to guarantee to a patient that they will return to pre-injury level sport, we need to be realistic and set realistic expectations. While it is possible, it takes serious dedication to rehab, and is probably harder, and will take longer than most patients/ athletes expect. What we need to emphasise more, and provide more for patients and athletes is Education Psychological support Evidence based rehab guided by performance outcomes Graduated RTS

19.01.2022 YOUR ACL CAN RE-ATTACH AND HEAL Previously it has been believed that the ACL cannot heal due to poor blood supply or because synovial fluid within the jo...int capsule inhibited clot formation which is required for the healing process to occur. There has actually been evidence since the 90s that confirms the ACL has the ability to spontaneously heal without surgical intervention. Ihara (1996) evaluated 50 complete ACL ruptures at 3, 11, and 24 months post injury with MRI. At 3 months post-injury 74% of cased had healed and demonstrated a well defined normal-sized straight band Costa-paz (2012) demonstrated re-attachment and healing of ACL ruptures may occur in up to 100% of patients without any intervention at an average follow-up of 30 months (range 25-36 months) post injury. Furthermore, all patients had a clinically stable knee, and returned to the same or almost the same level of pre-injury activity. Roe (2016) found 18/19 patients who had sustained a complete ACL rupture had healed at 1 year, and 1 patient demonstrated bridging of the ACL with scar tissue formation. Ihara (2017) examined 105 patients with complete ACL tears at 6 months post injury to evaluate the healing capacity. 18.01.2022 YOU HAVE A CHOICE! Many patients believe that surgery is the only option after an ACL injury. ... Many patients feel pressured to have surgery, or pressured into making an immediate decision, and the longer they take to decide, the worse their outcomes. Many patients are unaware that they have a choice as to whether they want surgery or not, or that they are allowed to trial conservative therapy first! Its time for a different approach to Anterior Cruciate Ligament injuries (Zadro and Pappas, 2019). We need to educate patients on both the risks and rewards of surgical vs non-surgical management. 17.01.2022 A very important systematic review released this week through the British Journal of Sports Medicine by 2 research orthopaedic surgeons and 2 research physiothe...rapists, highlighting the common misconception that good-quality evidence shows ACL injuries require reconstruction to prevent additional meniscal tears over time, compared to physiotherapy and exercise without surgery. Traditional reasoning is that ACL reconstruction is necessary to provide stability to the joint in the short and long-term, stopping meniscal tear rates from increasing, under the assumption that the ACL cannot heal. New evidence suggests that through intense, graded strengthening and rehabilitation the neuromuscular system can provide dynamic stability to the joint, prevent further meniscal damage and allow for healing of the joint tissues (including potentially the ACL) for many patients. This information needs to be communicated to patients as a part of a shared-decision making process following an ACL injury. 01.01.2022 NO SURGERY OR DELAYED SURGERY IS BETTER THAN EARLY ACL SURGERY It is commonly thought that getting the best surgeon, and having surgery as early as possi...ble following an ACL injury is the key to early return to sport and prevention of further injury such as meniscal damage or osteoarthritis. This is a false belief, not only is early reconstructive surgery worse than delayed surgery, it is worse than no-surgery at all!! Filbay (2015) performed an exploratory analysis of the KANON trial (Frobell, 2010). They found at 5 year follow-up, patients who underwent early reconstruction (within 10 weeks of injury) had significantly worse knee symptoms than those who had exercise therapy alone. Shelbourne (1991) found patients who underwent ACLR within 3 weeks of injury were at significantly more risk of arthrofibrosis and loss of knee range of motion which required revision surgery.
