Q: Why should I see a Credentialled Diabetes Educator (CDE)? A: Diabetes Educators are an extension to your health care team (GP and Practice Nurses) and can provide expert advice to help build your self- management strategies of your Diabetes. Research has identified that CDE’s provide three main values: empowerment,... education and support. A CDE can help you develop a person- centred healthcare regimen and develop problem- solving skills to achieve improved health outcomes. Q: Who is a CDE? A: The Australian Diabetes Educators Association (ADEA) grants status as a CDE to health care professionals, in recognition of demonstrated knowledge and expertise in diabetes education and commitment to professional development and ongoing learning. Q: How can I access a CDE consultation? A: CDE consultations can be funded via Team Care Arrangement (TCA) referrals via your General Practitioner Management Plan (GPMP or more commonly known as ‘Care Plan’). CDE consultations may be bulk- billed or subsidised by Medicare through this process. Eligible Medicare card holders can access up to five (5) consultations per calendar year to allied health practitioners. This advice is general in nature and always seek advice from your Medical Practitioner or other qualified health professional with any questions you may have regarding your health or medical condition.
