12.01.2022 Dry Needling is a topic that draws a lot of attention. But is it really all it is cracked up to be? There is a lot of hype but does it deliver the goods? And just how should it be done? At Bright Health Training we recognise that dry needling or acupuncture has its place, but we also recognise that there are significant risks and controversy about the topic, just ask an acupuncturist! One topic of contention is the eliciting of the local twitch response. This is a jumping or ...twitching of muscle when it is stimulated by a needle, often this is associated with a deep aching sensation. Many "old school" needlers will highlight the need for a local twitch response as essential and an indication that the point has been "hit'. It is our opinion at Bright Health Training that strong needle stimulation is not necessary, has increased risks, will create greater post needling soreness and no significant, or clinically relevant benefits over superficial dry needling. One recent study, demonstrated this clearly. It compared myofascial dry needling of the trapezius with or without local twitch response and found that there was clear superior benefit to no twitch response needling. It is wasn't just borderline superiority, the "twitchy' needling had a 30% pain reduction (6 to 4 in terms of VAS) but the no twitchy needling was on average 6.5 to 2.5 for VAS in post treatment response. The functional improvements were also superior to non twitchy needling, with an average superior increase of more than 10 degrees greater compared to twitch response needling. So the moral to the story? In this case at least, there was a clear benefit to not eliciting a twitch response, being gentle and being safer. https://content.iospress.com//journal-of-back-an/bmr181286

Yoga, Taichi and Motor Control Exercise faired well, along with acupuncture. Couple that with guidelines recommending non-opioid based treatment for lower back pain and this makes for some interesting conclusions. Unfortunately massage didn't fair so well this time along with other manual therapies.
