25.01.2022 Bungree Aboriginal Association is a preferred provider of the government's HCP Program, providing coordinated packages of care to people who have complex needs. The Central Coast Chronic Health team recently referred a socially-isolated 56-year-old male to our program. This man has complex care needs and was frequently presenting to hospital after falls in the home associated with malnutrition and non-compliance in maintaining health care needs.... Support staff were scheduled to visit five days per week to assist with reducing isolation, heating meals and grocery shopping. In the five months Bungree has been visiting the home, the client has had no admissions to hospital, has gained weight and has now reached out to family with whom he previously had no contact.
