25.01.2022 I am often asked about whether we should put vinegar on a box jelly-fish sting. The answer is YES! . A few years ago James Cook University did a study on how vinegar effects the nemotocysts and whether it is any benefit in preventing them from further firing. The study is called An in-vitro examination of the effect of vinegar on discharged nematocysts of Chironex fleckeri (Box Jellyfish).... The ARC (Australian Resuscitation Council) put out a statement saying....."The ARC is Australia’s peak resuscitation body and is responsible for developing guidelines for resuscitation and related emergencies. It does this within a framework consistent with the National Health and Medical Research Council recommendations for developing and reviewing clinical guidelines. As part of the ARCs ongoing guideline development process the findings of this study will be critically evaluated by the Council to determine if changes to the current ARC recommendations are required. Until this review process is complete, the ARC continues to recommend the use of vinegar for the treatment of Box Jellyfish stings." So the steps to take are 1) remove the patient from the water 2) call the ambulance early 3) restrain the patient from rubbing the stung area 4) observe airway and breathing and follow DRSABCD 5) FLOOD the area with VINEGAR for 30 seconds to prevent from further stinging and pick off remaining tentacles with your thumb and forefinger 6) if you DONT have vinegar pick off the tentacles and rinse the stung area with seawater. 7) DO NOT use fresh water as this may cause discharge of undischarged nematocysts.
