Following ovarian stimulation, unfortunately not every egg collected will be mature. This is important as oocytes need to be mature to have a chance of fertilising. Simply put, immature oocytes do not fertilise regardless of how much sperm is... put with them or how long they are left as they are not genetically prepared to support the process of fertilisation. Additionally, even if they are mature, not every mature egg will fertilise. On average, between 50-70% of mature eggs will fertilise with conventional IVF, and between 70-80% for those that have undergone ICSI. This is because mother nature still has a hand in what we do!

Your embryos are most definitely safe in culture! We know that your embryos are precious, and our dedicated embryology team care for them as if they were their own.... Incubators are the specialised pieces of laboratory equipment where the embryos are safely housed whilst under our care in the laboratory, allowing them to develop and grow under perfect conditions. To protect these incubators from the impact of any power failure all of our incubators including the embryoscopes have a backup power source connected to them at all times. This means they are guaranteed a power supply to function as normal in all conditions. In addition, our incubators are closely monitored by our team and a 24/7 system which alarms in response to any deviation from these perfect culture conditions, both for their internal temperature (37C, body temperature) and for their gas concentration (to allow the pH of the culture media we use to be optimum for embryo development), as well as for the power outage mentioned previously.

19.01.2022 Fertility Question Time with Sydney fertility specialist Dr Helen Peric! Today we will be exploring whether IVF causes early menopause due to using up more of your eggs. Read more below! .... No, IVF does not result in women using up more of their eggs and cause earlier menopause. Women are born with all their eggs and will continue to lose them throughout their lives until they reach menopause. Follicles which contain the eggs are continually recruited from this pool of eggs.... Every month when women ovulate they dont lose just one egg, they lose a cohort of eggs. How many does depend on their overall ovarian reserve. When women have a pelvic ultrasound we look at their antral follicle count (AFC). This is a marker of their ovarian reserve. The higher your AFC, the higher your ovarian reserve. From those antral follicles, usually one will become dominant, and this is the one women will ovulate from. The rest of the antral follicles are lost that month. When we do IVF, the extra FSH hormone women inject gives you a cohort of dominant follicles. This means IVF does not make more eggs than you are producing, it is instead rescuing follicles which would have been lost. This is how you end up with more eggs from the IVF process. Genetic testing is important because abnormal chromosomes can stop an embryo from implanting on the uterine lining, or result in a miscarriage or the birth of a child with physical and/or mental disabilities. Testing can help prevent these outcomes by identifying the affected embryos during their development before they are... transferred back into the uterus during an IVF cycle. 14.01.2022 Do you know how embryos are frozen? Vitrification is a fast cryopreservation technique that is 7000 times quicker than slow freezing and has resulted in a dramatic increase in pregnancy rates because of the excellent quality of the embryos when thawed. This is largely because it greatly reduces the risk of rupturing or damaging the embryo as there is minimal chance for ice crystals to form within the cell. Vitrification involves blastocysts (embryos) being suspended in a drop of fluid on a tool with a tiny hook on the end. The droplet is lowered onto a metal block that has been cooled by liquid nitrogen, where it hardens into a glass-like bead. The frozen embryos are then stored individually in a labelled cane in a liquid nitrogen Dewar (storage vessel).

Cheryl Ayres, special projects officer and senior embryologist will be answering your question today! ... By definition, an ovarian follicle is a fluid-filled sac that forms on the ovary during the first stages of a menstrual cycle, and each follicle is expected to contain an egg. This follicle will then increase in size, and ...the egg inside matures, under the influence and changes in different hormones occurring over the following days. This ovarian response is simulated in an IVF cycle using medications which encourage more than one or two follicles to develop, with a trigger injection given to assist the eggs to mature, ready for egg collection. Follicles will often differ in size, they dont all grow at the same rate, but you will be carefully monitored to ensure that the optimum time for egg collection is pinpointed. It is important to understand that it is not always true though that one follicle equals one egg at the time of egg collection, and as such, it wouldnt be uncommon to find that for every follicle seen on ultrasound that some didnt contain an egg. As a general guide, around 90% of follicles will contain an egg, and thus it is common that follicles are aspirated and there is no oocyte inside.

On day 3 we assess embryos based on the number of cells and the level of fragmentation within the embryo. By this stage, we expect the embryo to have divided into between 6-10 cells, with 8-cells being ideal.

But how and why does it affect sperm and male fertility health? ... Smoking not only affects the number of sperm, and the speed at which they swim, but also the quality of the genetic material inside the sperm cells. For these reasons, we encourage smokers to quit well in advance of any fer...tility treatment. In addition to this, publications have indicated male smokers have an increased risk of having offspring with childhood cancers such as leukaemia. The good news is that the impact of smoking on sperm damage is reversible. The benefits of quitting smoking will be evident within a couple of months, meaning the sperm two months forward will be significantly improved as they have not had exposure to the chemicals cigarettes contain. ... 06.01.2022 Day Two of Embryo Development This is the day following fertilisation and we ideally expect to see between 2-4 cells at this stage. The quality of the embryo will be assessed by our scientists through determining how even the cells are and evaluating the presence of minimal fragments. This is important as fragmentation occurs when the cell division is inefficient.

03.01.2022 To start the week we have a video of one of our brilliant embryologists catching sperm in "sperm slow". Sperm slow is a media used which contains hyaluronic acid (naturally found in the zona/shell of the egg) and helps us select the most mature and genetically viable sperm for injection via ICSI. The "super sperm" will bind to the media causing it to slow down (hence the name). This has a dual purpose of selecting the best sperm but also slowing the sperm down for the embryologist to "swipe" it's tail to initiate a series of chemical changes that enable the sperm (once injected) to initiate the fertilisation process.
