20.01.2022 CONNECTING CARE COMMUNIQUE 8 SEPTEMBER 2020 Connecting Care Central Coast provided feedback to the local community about outcomes from the 18-month Anticipatory Care Project at a lunchtime community forum held in the Gnomon Room, Ulverstone Wharf precinct on Tuesday 25 August. Thank you to those who came. We are pleased to be able to share videos from the forum which can be found on the Connecting Care Portal www.connectingcaretas.com.au or via Facebook.... We proudly showcased the lasting legacy the Project leaves to the Central Coast Community in the form of the Connecting Care Portal (including the Local Resource Directory) and an innovative Community Wellbeing Governance Model developed locally for Central Coast in partnership with Collective ed Ulverstone and Central Coast Council. Connecting Care Portal If you haven’t checked out the Portal recently please do! We continue to improve functionality and content and work will begin soon on incorporating additional enhancements to ensure sustainability of the Local Resource Directory and Portal once they are gifted to Central Coast Council at the end of the year. Please encourage your organisation and other local service providers, community groups and other relevant organisations to list on this FREE Directory. Without your help we can’t provide the community with the means to connect to support when and where it’s needed. Central Coast Community Wellbeing Governance Model The Collective ed Ulverstone and Connecting Care partnership is driving the transition to this new community-driven governance model. Maree Gleeson and Judi Walker are working with Collective ed to manage this smoothly and ensure that it works collaboratively with groups and organisation across Central Coast. A joint meeting of the Connecting Care Roundtable and the Collective ed working group will be held on 27 October to move this forward. Check out the short videos on the Portal and Facebook page. 10.01.2022 CONNECTING CARE COMMUNIQUE 18 MAY 2020 Here is our latest short Communique as we gear ourselves for the last few weeks of the funded project. The good news is that we are making significant advances with Connecting Care’s ongoing sustainability into the future. Community Governance ... A small Working Group has overseen the development of an independent community governance model for Central Coast which we hope will enable continuation of the important work of funded projects like Connecting Care and Collective ed Ulverstone, grow wellbeing initiatives across the municipality, and avoid unnecessary duplication. Connecting Care Portal and Facebook Page www.connectingcaretas.com.au www.facebook.com/connectingcaretas Entries on the Local Resource Directory continue to be extended and more Central Coast organisations contacted to ensure the Directory captures information for our community. This is even more important now as community moves into COVID-19 recovery. It looks like we will be able to continue the Connecting Care Portal, including the Local Resource Directory until the end of the year and are currently negotiating to make sure they continue into the future. Please encourage your organisation and other local service providers, community groups and other relevant organisations to list on this FREE Directory. Without your help we can’t provide the community with the means to connect to support when and where it’s needed. Community Health Literacy Training The first Community Health Literacy Training session was held last week via Zoom. We intend to run another Zoom session on Friday 5 June. These sessions are aimed at local front line staff and volunteers in local businesses, services and community groups and build on the pilots run last November. 06.01.2022 COVID-19 TESTING | AVAILABLE FOR ALL NORTH WEST RESIDENTS Testing criteria for coronavirus is different in the North West because of the risk of community trans...mission. The criteria applies to anyone in the North West, or anyone who has spent time in the North West in the 14 days before their symptoms started. If you feel like you have a fever, runny nose, cough, sore/itchy throat or shortness of breath, call your GP or the Public Health Hotline on 1800 671 738 as you need to be tested for COVID-19. Testing is only available with a referral from a GP or the Public Health Hotline 1800 671 738. In the North West region, coronavirus respiratory clinics are located in Burnie and Latrobe. If there is not a testing clinic in your local area, you can travel to another town or outside your municipality for testing purposes. After being tested, you will need to quarantine at home until you receive your test results. For more information, please visit: www.coronavirus.tas.gov.au

05.01.2022 Thanks to everyone who was able to attend the Joint Meeting on 27th October 2020, of the Connecting Care Roundtable and Doors to Mentors Working Group (Collective ed. Central Coast). The engagement and input from everyone was incredible and we are very excited about the next steps our first Community Leadership Roundtable Meeting for December 2020! The main outcomes from this meeting include: Clear understanding of where we have been, where we are and where we are going.... Introduction of the Interim Chair of the Community Leadership Roundtable. Workshopping the role of the Community Leadership Roundtable and Action Groups. Refining membership categories. Commitment from groups/organisation and/or individuals in attendance to join the Community Leadership Roundtable and/or Action Groups. Progress towards a name for the Central Coast Community Wellbeing Governance model. Stay tuned for more info soon!
