21.01.2022 Can the human spine be out of alignment? Is spinal misalignment a diagnosis and explanation for back or neck pain? Lets get clear on some definitions.... Alignment is a noun meaning to be in correct position. There is no medically accepted definition for ideal spinal position. This is because the human spine is a dynamic tissue. A true vertebral misalignment in medical terminology is common and is often the result of genetic or unexplained bony variations. An example of this is scoliosis. It is well documented that those with vertebral misalignment have little to no symptoms and are often high functioning, capable individuals. A spinal subluxation is an orthopedic term describing the misplacement of the vertebrae beyond their normal range of movement. It is often the result of traumatic force or hypermobility. It can, but not always, be associated with pain and negative symptoms. A spinal joint mobility dysfunction (SJMD) defines a restriction in a spinal joint preventing the joint from accessing its full range of movement. Spinal joints move in four planes and when a joint is restricted in one or all of these planes, it can be considered to be a SJMD. 4 SJMD's can be used as a medical diagnosis and are a common explanation for back and neck pain. When joint movement is limited it creates dysfunctional compensation in the local and global muscles firing. Tissues surrounding the area begin to seize up in response to the immobile joint. Local venous, arterial and lymphatic flow decreases secondary to the reduction in movement. Rather than viewing the spine as "out of alignment" and needing to be re-algined it can be viewed in terms of having functional or dysfunctional mobility. 20.01.2022 ENVISION PLAN COMITT EXECUTE REFLECT. That summarises how to create proficiency with movement. For the longer version, read on.... To cultivate proficiency in any movement pattern you have to give your brain the information it needs to execute the movement. Proficiency looks like the parts moving as they should. Without excessive strain, effort or rigidity. Assuming the movement is first pain free. You can't move well with pain. Not for very long at any rate. Pain interferes with the brain and the movement program the nervous system runs. Like a virus in a computer, it prevents us from acting out what we set out to achieve. Before you begin your next exercise. No matter what it is. Ask yourself first: "What am I prescicely trying to feel in each body part during the moment?" Then ask yourself what that looks like. Envision it. Plan it. Comitt to it. Then act it out. Slowly at first. Reflect on how it went. Repeat this. Over and over. Then you're on your way to building proficiency. Human movement relies on the flow of information between the nervous system and the structures of the body The more information coming from the body to the nervous system, the better informed the movement Joint centration refers to how well centered and congruent a joint is positioned at its point of contact Optimal joint centration enables co-activation of muscles surrounding the joint Co-activation of the active muscular system allows for sufficient control of the joint and surrounding structures. 12.01.2022 CORE STABILITY What is it? How does one improve core stability? Do unstable surfaces matter?... We know that activation of the core musculature appears to occur by lifting a heavier weight overhead rather than by lifting a lighter weight overhead with an unstable load, or on an unstable surface. Does bracing help? Bracing of the core has been shown to increase stability of the spine. However, bracing only involves tightening the outside of the core. What role does breathing play? The diaphragm has two functions, to ascend or descend. It has been shown to activate before other trunk muscles in response to rising on toes or external perturbations. Making it an integral muscle for postural control. Contraction of the diaphragm has been shown to increase intra abdominal pressure (IAP). IAP is key for reducing spinal loads. Air flow into the lower lungs via contraction of the diaphragm increases IAP. A raised IAP relieves spinal compression and improves the ability to lift heaiver loads via tension of abdominal cavity muscles. KEY POINT: Regulation of IAP plays a vital role in stabilising the spine during lifting.
