They do not have to be official photos. 3. A copy of birth certificate, birth extract or passport. 4. Payment of fees to SQRCHDI via cheque, cash or direct debit. One year full fee is $25.00 which covers registration and other levies. 5. Please include a stamped self-addressed envelope with above paperwork. 6.
