25.01.2022 Can we fix it? We create a lot of change doing Manual Therapies. Joint sprains can create Positional faults Mulligan. Joint dysfunction creates Motor Amnesia and diminished muscle function McGill. We can mobilise these joints and help to create a better scenario within the body for better movement. We can treat muscles and fascia to increase range of motion. But can we ‘fix’ pain? The body is a system that responds to stimulus and regulates itself. With manual ther...apies we can affect a change that the body responds to. Inherently though, the body fixes itself. Our job as manual therapists is to help get the body moving as well as we can, or prescribe exercises if the body isn’t doing what it should. Every ‘Body’ has an optimum potential timeframe for healing for each individual injury or scenario. It is extremely hard to help that body reach its optimum potential timeframe. It is really easy to inhibit that process and slow it down. Don’t over treat. Let the body do its thing. . . 17.01.2022 I got asked today, "am I a glutes or Pelvis guy, which is more important for function". Important question. In the clinic, the answer is always specific to the patient in question. Assess, treat, reassess. But(no pun intended), gluteal dysfunction can contribute to pelvic, leg or knee problems, where as pelvic joint immobility( public symphysis, SIJ) will directly inhibit gluteal action and functional capability. .... 17.01.2022 It's been a tough year so far and we're beginning to see the light at the end of the tunnel. Not quite there yet. Sorry to say the Melbourne and Brisbane workshops were cancelled and also the @massmyoaus conference in Adelaide too. I had a great new topic on assessment specifics and application. We'll be back next year though. As it stands at the moment I hope to continue shortly but I don't feel we can safely offer workshops yet and maintain all social distancing requirem...ents. 14.01.2022 I am seeing a lot of hype against Manual therapies. It doesn’t work, it a waste of time. This is not a statement FOR the use of MT’s only. This is not a statement against other modalities. A level of understanding needs to be reached to see where each modality fits within the treatment spectrum. There is room for everyone. We KNOW that soft tissue therapy provides temporary relief for aches and Pains. We know that specific Manual Therapy treatment options can provide a wi...ndow of opportunity for pain free motion. Is this a lasting effect? No. Does this mean it isn’t worth while doing these treatments? No. What it does provide is relief for the patient. It provides an opportunity for the body to return to normal pain free motion. This allows the body to repair within a quicker time frame. It allows us to then prescribe exercises and progressions that the patient can achieve more easily. You cannot make the body heal faster than it is physiologically capable of. You can only help it reach its potential. It is difficult to help a body achieve its potential. It is very easy to inhibit it with over treatment. Remember. No-one has all the answers. No modality is the perfect treatment option. Some are just more effective for each specific problem. 14.01.2022 It's out. Is it? Or is it just a poor descriptor for alignment issues. . Labeling something as 'out' creates perception that something is wrong and must be corrected. It creates fear and a dependance on the therapist to 'put it back in'. Sometimes, a singular facet joint lockup can be debilitating, but it is just a spinal segment that is unable to move through range painfree. . The term 'out' may have originally come from the term 'spine is out of alignment' which by itself ...doesn't really mean anything. In comparison, even a severe spinal curvature or scoliosis doesn't necessarily always create pain or limitation. Assess, treat, reassess. If you're not assessing, you're guessing. Explain to your patient your findings. They deserve to know the accurate truth not a lazy description. Do better. . . . . . . 12.01.2022 The spinal cord, nerve roots and some of the protective cord coverings. Note the size of the Corda equina (horses tail) fibres at the base of the spinal cord (that usually starts around T12) and also the nerve roots as they exit the central cord. Quite thin. The thicker, lateral cords are the protective coverings of the peripheral nerve as it exits the spinal column.. . This is why a disc bulge isn't the end of the world. There is plenty of room for the spinal nerve to exit ...the intervertebral foramen. Swelling, irritation of the local structures and localised dysfunctional joint and movement patterns all contribute to irritation of the nerve. All COMPLETELY CORRECTABLE with treatment and exercise! . Your patients pain is very real to them but it wont last forever. Very important concept to understand. . The upper fibres of infraspinatus has been mislabeled as supraspinatus (marked with the red X). The supraspinatus resides in the supraspinous fossa above the spine of the scapula (marked as ^supra) out of the screen shot. Honest mistake to make but these can become taken as fact and accepted as a truth. .... Remain critical. Search for facts. Form an educated opinion. Be prepared to rethink that opinion. Fact check. 04.01.2022 Improvised Orthopedic nail. Accident involving unsafe work practices and a Nail gun. . In Orthopedic repairs a Nail, Kirschner wire or intermedullary rod is placed within the shaft of a long bone after boring it out to provide stability after a fracture. Smaller bones like the fingers or thumb can be stabilised with wire in the same fashion. Many variations of wire placement occur depending on the fracture shape.... . Plenty of interesting scar tissue can be left after surgeries and traumatic fractures that limit movement for manual therapists to work on. We can't get rid of scar tissue but you can create an effect on the collagen matrix that increases the pliability and flexibility of the tissue. *Remember, most of the change is created by the body. There may be some initial mechanical changes that we elicit, but the majority of the change is the body remodelling the collagen matrix.
