22.01.2022 The collapse of the Teton Dam, is considered the failure that changed the shape of the industry. In 1976, over the space of the morning, a muddy leak appeared, suggesting sediment was in the water, but engineers did not believe there was a problem. By 9:30 a.m. the downstream face of the dam had developed a wet spot which began to discharge water at a rapid rate before the embankment material began to wash out. ... Crews with bulldozers were sent to plug the leak, but were unsuccessful, with work crews were forced to flee on foot as the widening gap, now larger than a swimming pool, swallowed their equipment. The fundamental cause of failure may be regarded as a combination of geological factors and design decisions that, taken together, permitted the failure to develop. Despite spending over $100 million in the development, there were significant details that were overlooked throughout the construction of the project. The collapse of the dam resulted in the deaths of 11 people and 13,000 cattle, with total damage estimates have ranged up to $2 billion. 06.01.2022 One of the most notorious cases of product failure was Bridgestone/Firestone tires. In 1994, they were the first tire manufacturer to achieve ISO 9000 certification. Six years later, In 2000, they voluntarily recalled over 6 million tires after a federal investigation revealed tire tread separation was being attributed to deaths on the road.... During the investigation, former employees testified the rubber used for tire manufacturing was expired and other parts were compromised due to improper controls and hasty inspections. Poor QMS systems and culture allowed for this to happen, which resulted in $79 million in the proposed settlement. If quality fails can happen to some of the biggest companies in the world, they can happen to any organisation. All it takes; One chink in the armour One finding that is ignored One conversation that is swept under the rug. Whats more disastrous is the fact that over 271 people died in traffic due to the failed tires, thats 271 people that never got to see their families again because of an organisational quality flaw. 03.01.2022 People are quick to blame their employees, or pieces of software for their inefficiencies and quality issues. Throwing it away to human error. While human error does exist, we must recognise that workers are typically doing what management are telling them to do, and what is laid out in a position description, and employee handbook....
