20.01.2022 Achilles Tendinopathy and the Role of the Soleus There are large plantar flexor torque and endurance deficits between subjects with and without AT. These ...deficits are bilateral suggesting that the non-symptomatic limb should not be used as a healthy limb or to provide between limb comparisons with the symptomatic limb in future studies or during clinical work. Weakness of the soleus appears to be responsible for the majority of the deficits observed in participants with AT. Further work needs to determine how current clinical interventions alter these deficits and whether they link to clinical outcome and whether these changes exist prior to or as a consequence of AT. https://www.sciencedirect.com///abs/pii/S1466853X18305017

18.01.2022 How Stress Changes Within the Knee During Deep Squats . I made this post to get people thinking about the stress they're placing themselves and others into ...when performing deep squats. I'm not saying that deep squats are bad on the knees and in studies of elite level weightlifters (average 17 years lifting) they generally have the same rate of cartilage degeneration as the average population (despite so much squatting).However, you'll also find studies where they have increased arthritis (PF joint) compared to average people. . However, the postero-medial (back inside) meniscus is more prone to degenerative changes over time and deep knee bending is a risk factor for developing tibiofemoral arthritis. . What this study showed is that compressive forces on the tibiofemoral joint increased with increasing depth of squatting (155 degrees of knee flexion), partially due to decreased surface area in the joint to help distribute forces. The other interesting component here was that the lateral portion of the joint becomes displaced in a deep squat and the medial (inside) portion of the joint is responsible for handling all of the load. . Study in participants at the Ultra-Trail du Mont Blanc Race Parameters ...= 171 kilometres (106 mi) and total elevation gain of 10,040 metres (32,940 ft). The race was finished by 3 of the 6 runners with a total hip arthroplasty (THA), all of the 8 runners with a hip resurfacing arthroplasty (HRA) and 3 of the 4 runners with a total knee arthroplasty (TKA). Conclusion : Running ultra-trail is possible after a hip or knee joint replacement. All of the 8 runners with HRA finished the race. Instead of proximal migration, the inf...ection typically spreads dorsally through a sinus in the superficial transverse metacarpal ligament; the proximal spread is prevented by the palmar aponeurosis. Classically, patients present with pain and tenderness in the webspace with abduction of the adjacent digits (V-sign). Treatment is always surgical and involves both a dorsal and volar incision with optional placement of a Penrose drain; avoidance of the webspace is critical to avoid scar contracture and loss of abduction.
