23.01.2022 On Monday morning, I will be seeing a patient (in the lovely gardens out the front of St Vincent's, not in my office). That will mark the last outpatient I see face to face for a little while, unless medically urgent. Telehealth will keep us going and connected. Keep your distance, stay safe and avoid risk of transmission and we'll be back to normality in just a little while. We can do it, together.

21.01.2022 https://jamanetwork.com//jamaotol/article-abstract/2737712 The rates of tongue tie release have been increasing dramatically in recent years and of concern, it seems to have become the go-to solution for many new mums struggling with breastfeeding. Not surprising really - stressed, tired and vulnerable mums struggling with breastfeeding are bombarded with this advice, quite often the very first suggestion given by others in social media forums, and even many professionals... - some speech pathologists, nurses/midwives, lactation consultants and dentists. This little study showed that the majority (~62%) of young infants referred to a specialist clinic in the USA, for consideration of frenotomy (snipping of tonuge tie) did not require the procedure but were helped instead by a careful assessment of their feeding and offering mothers alternate techniques to help. Im still surprised that almost 40% of this cohort were deemed to require surgery but the take home message is that the majority of bubs did NOT require it and that although frenotomy MAY be required from time to time, one should be careful about jumping straight into looking to have this procedure done before considering other options and treatments. Although not a high risk procedure, there may be complications (and I have seen these not infrequently - bleeding, scarring, infection), not to mention potentially quite significant cost depending on who does it or where its done.

10.01.2022 After nearly 8 years of this being my entrance to work, I have handed in my keys! Darling Downs Paediatrics has moved to 86 Alderley St, Rangeville (still in Toowoomba, for our out of town patients!). Ill still get to enter here though, when going to see inpatients and attend deliveries! In some ways its a little sad....but its mostly exciting to be moving on and able to make my new rooms how I want them. Oh, and did I mention?... free and easy parking at the new location! Thats a BIG positive for our patients :-)

07.01.2022 Great to hear a local QLD paediatric service lending a hand abroad. Sad that this opportunity arose though, a clear result of poor vaccination rates. What Samoa has unfortunately experienced of late gives a glimpse of what used to happen, in communities worldwide, prior to vaccination. Luckily its preventable.

07.01.2022 https://dontforgetthebubbles.com/skindeep/ Another great idea from the DFTB team - a collection (still developing I believe) of skin conditions and rashes etc, in paediatric patients. You may recognise some, and others may even help you work out what's affecting your child. But be aware - some might find some of the photos alarming and of course please still take your child for medical review if concerned about a skin problem or rash. 'Googling it' isn't suggested!

Some old mates are launching an online, after hours (evenings only at the moment, hours may change depending on how things go and demand) 'virtual paediatric consultation' with ongoing follow up as able and required. Expert acute paediatric care and advice (not to mention ability to prescribe, order imaging, pathology tests etc), remotely, in your home. Nice work guys

04.01.2022 Great timing! Only this morning I discharged a toddler, luckily well and happy following possible accidental ingestion of a potentially toxic substance. Then I spy this from the DFTB team and its a timely reminder that commonly kept items around the house can be exceedingly harmful. Put it away. Curiousness in our little ones is a delight but prevention and vigilance is key to keeping them safe.
