18.01.2022 This patient presented with a broken lower molar (tooth 37). The tooth needs an indirect restoration, however it may need RCT first judging by how deep the existing restoration is on the radiograph. There are also several other teeth needing attention. In these type of cases, I prefer to restore a tooth like this with a direct composite, which serves as a medium term temporary restoration. A well bonded composite even of this size on a lower 7, can last 2-5 years easily in my hands. It allows us me to 1- build trust with my patient over several visits, 2- fix the other teeth that need fixing and 3- monitor the vitality status of this tooth over the next 3-6 months. If the tooth remains asymptomatic and vital, we can proceed with an indirect restoration.
17.01.2022 This lovely patient had a post and core crown on tooth 11 done many years ago. The restoration debonded and part of the post was broken inside the root. After discussing all restorative options, the patient decided to get an implant retained crown to have it replaced. A direct temporary bridge was made till the ptn can get in to see the specialist for an extraction and implant placement

16.01.2022 Cracked teeth make me nervous. Specially upper premolars which can go from being completely asymptomatic to being cactus very unexpectedly. So when I got a message at night from a colleague that their premolar was very painful and unable to take any pressure on the palatal cusp, I started fearing the worst... Luckily, the crack had not split the tooth and we were able to save the tooth :D

It is very important to me to be able to do restorations which 1- last a long time and 2- give the patient minimal post-op discomfort. 14.01.2022 This lovely patient wanted to improve the look of her front teeth as she is getting married in less than 2 months. This is an ideal case to be treated with Orthodontics, but given the timeframe, we were left with very few options before the big day and direct resin veneers were done to help make the teeth look straighter. The positioning of the teeth made this very challenging to treat. Even then, only very minor tooth preparation was done on tooth 11 mesial aspect to achieve this result. It's not perfect, however the patient knew this before starting as we had done a mockup for her to preview. In all aesthetic cases, a mock up is essential to preview what the proposed outcome will look like.

11.01.2022 I'm not a fan of direct composite resin bridges, mainly because their life span is shorter than the other alternatives. But they are a cost effective and minimally invasive solution for the short-medium term in some cases. As with everything in dentistry, case selection is very important. This gentleman had a RPD which he had for over 30 years and wanted to have it replaced with a fixed solution, but his budget was limited. A fibre reinforced composite resin bridge was done and the occlusion on the lower incisors was adjusted to ensure no contacts on the bridge in centric and excursive movements

09.01.2022 Tricky to treat sometimes. Patient needs several indirect restorations and is waiting till next year to have this tooth restored with an indirect restoration. So in the meantime, a direct composite onlay is a good medium term temporary. Till next year!

07.01.2022 The start of every new smile starts with a wax up. This is the first crucial step that creates the blueprint for a new smile and allows the patient to see what their teeth will looks like after completion of the treatment. It also gives them a chance to provide feedback and input into the shape and size of their teeth once they can visualise the mock up in their own mouth. In the end, it translates to the patient getting exactly what they want and there are no surprises on the day of cementation. A tip to reduce flash around the gingiva when trying in the mock up- take a small round bur and inscribe a margin in the stone around the gingival margin, before taking a putty impression of the wax up.

05.01.2022 Clinical tip- If the clamp comes in the way when you're trying to seat a sectional band, you can open the clamp and push the sectional band down to let the clamp sit on the sectional band. In this case, the band was adapted against the tooth by the help of the clamp alone.

03.01.2022 One of the occupational hazards of being a dentist is that you start to constantly analyse people's teeth and smiles, how they look when they talk etc. I would have done countless smile designs in my head. The waitress at the restaurant with the rotated centrals, the check out lady at Coles with the peg laterals, the barber with the gummy smile... All the things I wish I could fix... I had noticed this patients teeth for a while, and one day she asked me what I would recommend for her teeth. The answer was simple to me, but it's wise to never impose your aesthetic viewpoint on the patient, rather let them tell you which aspect they are unhappy with. And so, we began planning and designing a new smile for this patient.
02.01.2022 Tooth 26 showed extensive cracks on the Disto-palatal aspect. Tooth 36 is missing but tooth 37 has occlusal contacts in the distal area of tooth 26. Tooth 37 was cracked and restored with an eMax crown at the same appointment. While the crown was in the furnace, tooth 26 was restored with direct composite. Total time taken for the crown on tooth 37 and composite on 26 was 2.5 hours.
02.01.2022 Tooth 26 has been endodontically treated by someone else previously. Now, we do not treat radiographs but treat patients, but I'm not happy with the way the endo looks on the radiograph. So I recommended getting a second opinion from an endodontist and perhaps getting the endo re-done prior to having it restored with a crown. The patient declined. My philosophy is, where possible, be certain that the treatment I provide will last at least 5 years, specially for the more cost...ly treatments. If I'm in doubt, I would not proceed with the treatment. So I declined doing a crown on the tooth as the endo may possibly flare up in the next 5 years. In this case, the distal contact of the 6 was an open contact leading to a food trap and needed treatment. The patient and I settled on a compromise, to restore the tooth with a composite overlay. A cheaper alternative in a compromised situation. If the tooth does play up, it can be extracted and the patient isn't out of pocket too much. See more
