EVA is a shank dependent material and requires to be in contact with the supporting surface of the shoes to provide resistance to load. An EVA orthotic will be thicker in the medial and lateral arch area compared to a polypropylene orthotic. This thickness can cause challenges when fitting orthotics to patients footwear.... OrthoTips: If you dont have access to a grinder give your lab as much information about the footwear as possible so they can pre-grind the orthotic to size. Use a template to indicate: -dorsal surface dimensions -plantar surface dimensions Plantar surface dimensions are very important for EVA fitting as this is the size of the flat surface the orthotic will rest on within the shoe. This client needed a custom EVA orthotic with a Poron Mortons Extension for a patient with hyperkeratosis PMA1 and medial hallux, due to tissue stress created by their toe-off pattern. ... They selected a 3.2mm poron mortons extension from the finishing additions options on the DOLA Custom Prescription form. Key features of this selection include: -an addition of the poron material under the 1st Metatarsal head, crossing the MPJ extending the full length of the hallux and beyond. - addition to match the width of the hallux following the anatomical medial flare - Poron thickness of mortons extension as scripted Orthotip: Aim of this addition is to provide material cushioning under the 1st MPJ and hallux. The poron material will not block or reduce 1st MPJ dorsiflexion unlike a polypropylene mortons extension. Localised poron was selected to keep the orthotic forefoot volume to a minimum and concentrate on the area of tissue stress. They selected an extrinsic polypropylene rearfoot post from the rearfoot postings options on the DOLA Custom Prescription form. Orthotip: A rearfoot post provides stability to an orthotic shell and allows for angulation in a varus or valgus direction. Modern CADCAM allows Rearfoot posts to be milled directly into the shell creating stability without additional material. Extrinsic poly rearfoot post (while not actually an extrinsic piece of poly) resembles the traditional extrinsic EVA post shape, with a large flat plantar surface for maximum stability & contact area within the shoe. The Forefoot remains stable and balanced even at high Rearfoot post angles, removing the annoying rocking problem. Choose this option when wanting the most stable rearfoot post but ensure space is available in the footwear. A poly rearfoot post will have a longer lifespan compared to an extrinsic EVA rearfoot post, which may need replacing or refurbishing. Traditionally an EVA post was applied after the shell was pressed onto a positive cast as the heel shape would be round and unstable.... Modern CADCAM allows rearfoot posts to be milled directly into the shell creating stability without additional material. Intrinsic rearfoot posts in a polypropylene shell creates a stable flat plantar surface without adding bulk to the device. The anterior orthotic edge remains stable and balanced even at high Rearfoot post angles, removing the annoying rocking problem. A varus or valgus angle can be applied to create either medial or lateral ORF. Choose this options when wanting a stable rearfoot post in footwear with space limitations or when wanting a low bulk option. Health professions and orthotic labs can have variations in terminology and definitions for orthotic design features. Our interpretation of this question, is a difference in the required positioning of the metatarsal bar addition.... A metatarsal bar is an orthotic addition characterised by its shape and placement location. A metatarsal bar gradually rises proximally to distally and is placed along the metatarsal shafts, at a width to influence the selected metatarsals, 1-5, 2-4 etc. They can be custom made of poron or EVA or purchased as prefabricated additions in poron. The difference in terminology reflects the positioning of the metatarsal bar pad. A pre-metatarsal bar will be placed in a more proximal position, influencing the metatarsal shafts and ending before the metatarsal heads. A traditional metatarsal bar placement is more distal with material located under the metatarsal heads. This provides attributes of the material (poron) under the metatarsal heads. When prescribing a metatarsal bar, remember to tell you lab the placement you require. The orthotic prescription forms part of legal clinical notes on your patient and is required to be completed if a device is to be manufactured. Parts of a prescription form commonly include: -Patient / clinic / order information / dispense date -Orthotic shell material -Plaster or CAD design instructions -Orthotic shell shape / dimension info -Additions -Covering -Instructions section -Diagram or foot skeleton Your lab will often have a manual that outlines the variables on the form and gives terms or standard definitions. It is challenging to fit all on a 1 page form or a short online form so the instructions section is used by practitioners for additional information not included in the tick box sections. If it doesnt appear on the form it doesnt mean your orthotic lab doesnt provide the service or variable you are after. Use the instructions section, send them an email or give them a call. The addition of thickness to an extrinsic polypropylene rearfoot post to form a heel raise will elevate the heel with respect to the forefoot. It will function in the same way as an EVA rearfoot post and heel raise but will not wear out or need replacing.... In turn it will require grinding to reduce or remove, and can be increased with the addition of an EVA heel raise.
