23.01.2022 I keep seeing posts like these playing down the seriousness of this problem, and I find it infuriating! I want to explain how WRONG this is All these other diseases are in equilibrium- that is, they’re relatively stable and ‘mature’ in our population. COVID-19 on the other hand is a newborn! It’s 5 months old, but it’s growing quickly!! To see how it might look when it’s matured, we should consider what would happen if 70% of the global population gets it. (At which point... you will see some herd immunity) If 70% of 7 billion people gets sick, that’s 4.9 BILLION people infected worldwide, of whom roughly 980 MILLION people would require hospitalisation, and 98 MILLION people would die, assuming 2% mortality rate. But if the mortality rate is 5% (as it is in Italy, and as it would be if the healthcare system is overloaded), then 245 MILLION people would die. This doesn’t include indirect deaths attributed to heart attacks, car accidents etc that couldn’t be cared for in an overwhelmed healthcare system. If we consider ONLY Australia, then 17.5 MILLION would become ill, of which 3.5 MILLION would require a hospital bed, likely within a 12 month period. At 2%, AT LEAST 350,000 Australians would die, BUT if it’s 5%, then 875,000 would die. NOT COUNTING INDIRECT DEATHS. In BOTH World Wars, a total of 100,000 Australians were killed. TOTAL. ANYONE who could trivialize the deaths of 875,000 Australians, is by definition, unAustralian. I’m looking at you Alan Jones Guys, this is not a drill. This is not a scare campaign. This is not a conspiracy. This is Mother Nature at her most terrifying. Stay home. Don’t visit. Please.

Because WE all act...ed swiftly and decisively, Australia now has one of the lowest rates of transmission in the world and we have had time to improve PPE levels, prepare our hospital system and save lives. A true testament to our power when we work together! But COVID-19 is still in the community, and further outbreaks are inevitable without ongoing action. To prevent us from going in and out of lockdown, we need to maintain our precautions (hand hygiene, masks, social distancing) and we need the ability to test widely, test quickly and isolate any cases or potential cases very quickly. Google, Apple, F@cböok, Wh@ts@pp, the Banks, and even the games on your phone collect intimate details on our whereabouts and activities all the time, and most of us don’t give it a second thought. The COVIDSafe app does not collect location data and does not access your contacts or any other personal information; it simply keeps a log of who you’ve come into prolonged close contact with. The data is encrypted and only accessible under specific circumstances. 13.01.2022 Just want to get the message out the that not all disinfectant wipes will work against Coronavirus. Check the small print to see the active ingredient. The following ingredients DO NOT WORK to kill coronavirus: - Benzalkonium chloride... - Chlorhexidine - Acetic acid (vinegar) - Citric acid The following DOES kill the Coronavirus: - Soap or detergent and water - 70% ethanol (alcohol) - isopropyl alcohol (rubbing alcohol) - 0.1% hypochlorite (but 0.2% is better- faster acting; this is dilute bleach- handle with gloves) - 0.5% hydrogen peroxide (can be used as hand sanitizer) Follow the South Eastern Gastroenterology FB page as we will be publishing recipes on how to make these solutions at home

13.01.2022 How many infectious people are walking around with COVID-19? The answer is MUCH MORE than you think! NSW is at 2032 official cases as of today, and Australia is at 4359. But we know that 20% of people have few or NO symptoms , and those people aren’t getting tested and aren’t included in official figures (**watch out for loss of smell and taste- this could be the only symptom!**) So adding the missing 20% tells us there are about 2542 infections in NSW, and 5448 in Australia.... BUT, it takes about 10 days to go from exposure to a positive test result, and during most of that time, patients are infectious. So if you want to know how many people are INFECTIOUS right now, look about 9 days in the future. To keep it simple, we’ll use a doubling time of 3 days (around the world its between 2-4 days) In 9 days, you would have three doubling times, so you need to multiply today’s number by EIGHT! (x 2 x2 x 2 = 8 ) That means that right now, in NSW, we can expect that there are approximately 20,336 people who are INFECTIOUS. In Australia, that number is 43,584!! Stay away from people, especially the ones you love. Wash and sanitize your hands. Consider wearing a simple mask if you’re in public. After all, how sure are you that you’re not infected?! 13.01.2022 I would like to point out at this point, that with a doubling time of 4 days (conservative), and 1050 confirmed cases as of tonight, within 20 days we will have at least 33,600 infected of which we can reasonably expect 1680 to need ICU... In a country of 2000 ICU beds. Our only effective treatment is to starve the virus of new hosts (ie you, me and our loved ones) My advice:... - Self isolate now. Protect yourself and your family- this means no one in or out of the house. 12.01.2022 Not sure how many people are aware that COVID-19 is faecally excreted! + !! This means people with the virus pass it into the toilet with their poo! The official line is we don’t know whether this represents a risk of infection, but I say take precautions because none of us want to be the ones that find out the hard way!! Some precautions you and your family can take:... - Where possible, anyone with suspected/confirmed COVID-19 infection should isolate to their own bathroom - Avoid public bathrooms if at all possible (my patients will IBD can’t all do this- keep detergent wipes and hand sanitizer with you if you’re not already self isolating) - Flush buttons, taps and door handles should be disinfected after using the toilet, and then wash your hands - When washing hands, be careful to not contaminate your hands again by turning off the now-contaminated tap bare-handed. (You potentially contaminated it with your hands when you turned it on!) - Disinfect/wipe down surfaces (sink, benchtop etc) once or twice a day - Remove Toothbrushes and other personal hygiene products from any exposed position (put them in cupboards, drawers or in another room) - Hang towels at least 3m from the toilet - CLOSE the lid of the toilet BEFORE you flush Why are these things necessary? Because toilet plume!

12.01.2022 The commonest misconception I hear? Bright red bleeding means it’s nothing serious- WRONG! Don’t write off symptoms without knowing what is causing it- especially bleeding! Unfortunately this scenario is becoming more common- my own brother narrowly avoided being diagnosed with bowel cancer in his mid-30s- if he had waited another year for his colonoscopy, it’s likely the aggressive polyp he had would’ve become malignant. If you have symptoms, see your doctor. If you see blood when you go to the toilet, ask to see a gastroenterologist and get it checked out once and for all- early intervention can prevent cancers and saves lives!

I’ve seen sooo many people at the shops and in photos on social media misusing their PPE, so our amazing nurse at The BMI Clinic has made this video to help inform about the proper use of hand sanitizer, gloves and masks! Mistakes I’ve seen: ... - Shopping with gloves on (only contaminates your produce and groceries- cleanse the trolley handle instead!) - Driving with gloves on (now you’ve contaminated your car interior- take off gloves and sanitize hands before you get in the car) - Drinking (or eating) with gloves on (now you’re drinking with a contaminated cup!!) Some general tips: - Decide what areas should be CLEAN- eg household, car, office or desk. DO NOT use gloves in these places- keep them for dirty places, and REMOVE and wash hands when entering a clean space -ALWAYS wash hands BEFORE putting on PPE (gloves, masks, etc), and again AFTER taking them off - Gloves should be considered contaminated as soon as you put them on- ONLY touch DIRTY surfaces once on. Remove if you need to touch a clean surface - Take gloves OFF before touching yourself or anything you eat or drink - Masks are considered CONTAMINATED once used. 05.01.2022 To the Government having claimed little community transmission- the only way we can be seeing rises like this is if community transmission has been underway for at least 2 weeks. Stop lying to yourselves and the public and open up testing! Not knowing the true state of play stops us being able to plan At current numbers and rates of diagnosis we will have 10,000 infected in 10 days. If I’m right about community transmission, then it may actually be double that. About 15-20%... will need a hospital bed and 5% or more will need ICU Time for some advanced planning and mobilization The government needs to begin: - Mobilising military resources and medical supplies - The process of closing day surgical facilities and some private hospital theatres and commandeering anaesthetic machines, ventilators, infusion pumps and other equipment - Redeploying anaesthetists and anaesthetic nurses to care for intubated patients - Setup a network to allow ICU specialists to oversee the satellite units & anaesthetists for more complete intensive care support - Tell the public you’re taking these steps so faith is restored in your management of this crisis This all takes time and needs to start now to be ready for the tsunami
