23.01.2022 This case is a reminder that what we do for our patients can drastically affect their psycho-social well-being. Dental work that doesn’t go to plan may affect aesthetics, self-esteem, function and be a financial burden. Patient care and well being should be the ultimate objective of a health care clinician. All on Four gone wrong. ... Unfortunately the patient received an all on four procedure 8 years previously that started to fail within the first year. He functioned with the broken prosthesis that consisted of four maxillary teeth for 6 year before seeing me. Unfortunately the treating dentist was deregistered for poor treatment of this case and many others. The main problems were due to the poor positioning of implants leading to severe cantilever forces and breakdown of the maxillary prosthesis. Coupled with that was the poor design of the prosthesis leading to inability to clean and peri-implantitis. The patient did not want to undergo further bone grafting procedures and did not want the existing implants removed, hence the unique solution of a stable base denture with non-rigid attachments that allowed the maxillary removable prosthesis to slide passively on the attachments. It was a life changing transformation for the patient allowing him optimal function and providing the ability to socially interact once again. We can do so much for patients!
