25.01.2022 Community . The last 103 days in Lockdown 2.0 have been undeniably hard for Metro Melbourne. But despite our dislike for living under restrictions, Victorians have strived to do what is required to protect our community. #actoflovenotfear . So there was cause for celebrations today as low infection rates were maintained; resulting in the welcomed announcement that some restrictions can be eased from midnight .... . This little town of Spoonville sprouted a few months ago, and it’s been delightful to watch it slowly grow. As we start our first week of Stage 4 Restrictions, Im choosing not to bemoan the temporary loss of freedoms, but to focus on the reason for locking down - as a demonstration of love - for our community and our families. . 25.01.2022 World Orthodontic Health Day . Its been lovely to see all the posts celebrating smiles and good dental health during #WOHD. . I have the good fortune of working with many amazing orthodontists, including sharing care for one of my favourite operations - orthognathic surgery (aka corrective jaw surgery).... . Not everyone with a bad bite needs to have surgery but it is often undertaken when there is an underlying skeletal abnormality. This combined treatment plan requires good coordination and communication between orthodontists and surgeons, and theres a lot of behind-the-scene discussions including during 3D surgical planning (video of one shared with permission). . 24.01.2022 Innovation and collaboration . Masks are one-size fits all; and I often find those with ear loops either too loose (therefore ineffective) or too tight (thereby hurting my ears after a long session). . And commercial face shields are often incompatible with surgical loupes and the bulkier #ppe (personal protective equipment); as there is insufficient space between the face and the shield.... . So Im thankful for innovative companies like Lyca Smith. Even though they normally customise surgical stents and plates, they sent these great ear guards and different configuration of face shield frames for me to trial. But whilst Im still in planning mode to ensure we are prepared for a second wave of infection, Im really praying these extra #ppe will not be required! . 19.01.2022 Foggy glasses . Fogging is, unfortunately, a common phenomenon for masked, bespectacled folks. . Fogging is caused when the warm exhaled breath meets the cooler lens, so can be eliminated by a well fitting mask:... mould the wire over the nose tighten your mask (easy to do with ties but still achievable with ear loop masks - either with an ear guard, or put a twist/figure-of-8 into the loop before placing over ears). . Other touted - but personally untested - methods include: placing tissues under the top edge to seal the gap, using commercial anti-fog products, or washing glasses with soapy water and allow to air dry overnight (surfactant action) N.B. any lens treatment may damage special coating. . My personal anti-fog remedy is to tape the upper edge of the mask down with medical tape. This not only helps with eliminating fogging, but also reduces movement of the mask when I speak (i.e. from opening and closing my jaw), and it stops me taking off my mask unnecessarily as I dont like taking the tape off my skin. . You can also reduce fogging by loosening the lower ties on a mask, or with a valved mask - but that would totally defeat the purpose of community masking. . 17.01.2022 Oasis . It was so amazing to overlook this courtyard for the The Royal Melbourne Hospital medical records training this morning! I must admit that the thought of seven hours staring at a computer screen had initially filled me with dread. But it was a great day . On top of the useful IT skills acquired, we even got a lunch break (a bit of a luxury for a surgeon); caught some sun; listened to the cascading water and the bird song, and indulged in (quite!) a few photos .... . I had actually completed part of my medical training at Royal Park (only 6km from the CBD!) but I dont think this building was opened then. . 16.01.2022 Mask truths . There are so many myths out there about mask wearing - one of which is that you are only going to get carbon dioxide toxicity from rebreathing your exhaled breath. . Today, Bec, Sally, and I scrubbed out of our theatre list and checked our oxygen level - after over four hours masked up. None of us have major medical issues so unsurprisingly, our oxygen levels measured between 98-100%. And if you look closely, youll see that each of us have two masks on -... a tight fitting N95 AND a surgical mask over the top. . So unless you have a pre-existing medical condition (and if you do, you should consult your GP) - you should be okay with a mask too! 15.01.2022 26 Jun 1498 . Celebrating 522 years since the first bristle toothbrush was invented in China! These were made of coarse boar hairs attached to handles of bamboo or bone. . In 1938, nylon bristles were introduced by Dupont de Nemours, followed closely by the first electric toothbrush in 1939. Modern toothbrushes are usually made with plastic moulded handles, but many are moving back to more environmentally friendly alternatives such as bamboo.... . It is important that patients continue toothbrushing following their procedures, and all our patients are given an extra soft bristled toothbrush to aid this. And whilst we have made the move back to bamboo handles, I promise that our brushes do not have bristles! . 14.01.2022 Single use masks . Its been heartening to see so many adopting masks before Melbournes mandatory requirement from midnight tonight; but many masked crusaders are wearing them incorrectly. So, here are a few tips on single use masks from a frequent user: wear the coloured side outward (and pleats facing away from you), mould the metallic strip to the bridge of your nose to improve the fit; this also reduces fogging for those with glasses. I often bend the wire in th...e middle as my first step, after securing the mask, pull the bottom of the mask over your chin whilst firmly holding your mask over your nose (to attempt to form a seal around your chin, mouth, and nose). . It is worth taking your time to ensure the fit is as comfortable as possible because once on, you should refrain from touching your mask again. (i.e. assume the mask is doing its job and is hence contaminated). If you must adjust your mask; sanitise your hands before, and after any adjustment. . Please dispose of your masks safely and responsibly. Take them off by the straps (the least contaminated portion of the mask) and put it straight into the bin. Then clean your hands. . And please remember that the key to reducing transmission is still to: #gettested if symptomatic #stayathome if possible #physicaldistance at 1.5m apart #washyourhands frequently, for 20 secs each time. . But do mask up properly so that we can get the most benefit from our collective efforts as a community. Our eager med student then showed me these beautiful stencils she had completed, to help her understand the movements of the jaws before she saw the procedure for the first time today. What a !... . 10.01.2022 RecycleReduceReuse . We may have just closed out ‘National Recycling Week’, but everyday is a good day for recycling . Health care is notoriously for the amount of ‘waste’ it generates; so we had consciously designed our practice to enhance our ability to ‘recycle, reduce, and reuse’.... . And thanks to the addition of the purple bin (for glass recycling) last week - we can now separate our landfill and recycling into seven different bins . . 85% of health care waste is general and non hazardous (source: WHO), including a lot of clean soft plastic packaging required to maintain sterility. But soft plastics (i.e. scrunchable plastics) are actually readily recyclable via REDcycle bins (Woolworths and Coles). . And whilst many larger organisations do not yet recycle soft plastics in-house, our team always try to reduce our environmental footprint, even if this means collecting the clean packaging from our theatre lists to recycle externally. . 09.01.2022 #dentalhealthweek 3-9 Aug . Instead of celebrating Dental Health, we had a week of constant changes as Melbourne headed into Stage 4 restrictions and its first ever curfew; and dental practices received a mid-week order to wind down with immediate effect. . But this does not mean that you are not allowed to visit your dentist. Many practices are staying open to provide urgent care so you can still access treatment if you have an emergency.... . Towards the end of Victorias first lockdown earlier this year, we had a much greater number of patients presenting with dental infections to the tertiary hospital where I was oncall. Some of the patients had delayed presentation and required multiple operations, and one was even admitted to ICU. So please dont neglect your dental (medical, nor mental) health, especially during restrictions! . 09.01.2022 Covid Testing Take II . So we were back at the Respiratory clinic earlier than expected. . Mr R’s cold symptoms came at an especially inconvenient time, and I must admit that I was momentarily tempted to ignore it and just carry on with life! I reasoned that the chances of it being Covid was close to zero - coming hot on the heels of Ms R’s and my own negative tests; with likely transmission of a common cold within a family; hayfever season; and Victoria’s recent low tra...nsmissions etc... . BUT . we all know that rigorous testing is vital to maintain viral suppression... So off we went to add another swab to the 200+ test processed by the The Royal Children's Hospital, Melbourne yesterday (go Victorian kids and parents ). . It was great to receive confirmation today that Mr R is Covid free, but it’s still home time until symptoms resolve. So we’ll just have to order in some s to celebrate the eleventh consecutive day of Double Zeros for Victoria! . 07.01.2022 Happy Friday . Victorian hospital networks set up their timetable on a four week rotating cycle, so most surgeons operate on a similar schedule. . Ive consultant appointments at two public hospitals and it so happens that the commitments fall heavily in one week - this week! Add to that a concurrent week of oncall, late night call-ins, and resumption of normal private practice post-restrictions - its certainly been one busy week.... . I must admit I was a little tired and slightly grumpy when I left work, but this beautiful sight reminded me I have so much to be grateful for, including a job that I love. #bestjobintheworld . 07.01.2022 Ear guards . Our team have been using ear guards with our ear loop masks. Not only do they improve the fit of the one-size-fits-some masks, they also relieve the pressure on ears when you have the mask on for any length of time. . There are many open source files to print your own ear guards, and whilst not everyone will have access to a 3D printer - there are everyday items you can use which do the same job! I am not a great seamstress ( my forte lies in suturing soft... tissue), but even I could put a couple of buttons on a hat or headband. . 06.01.2022 Post-operative care . Scrunchy, pictured immediately following surgery, was well on her way to recovery when I checked in with her mum two days after her procedure. . It’s my routine to call each patient a few days following surgery. Patients usually do not remember our discussions in the recovery bay so the call provides an opportunity to answer questions, reinforce instructions, and to check on recovery.... . Some of my post-operative patients were surprised when I called them yesterday, at the beginning of the grand final long weekend One was sweetly concerned that I was ‘working’ on a public holiday, but I reassured her that it was for my benefit - I relax better after I have spoken with everyone . . Then it was off for adventures; including the first face to face meeting with grandparents for months! . 06.01.2022 Back at school . The kids are finally back at school today And I don’t know who is more excited - them or me?!? . They skipped off this morning with their hand sanitisers and face masks. And despite using various commercial ear savers, their favourite turns out to be recycled plastic ( milk bottles in this instance).... . The kids prefer this as: it is flat (fitting easily under bicycle helmets) it catches less in my daughter’s hair, and it is comfortable (as it is easy to customise for length). . And if your child(ren) are wearing masks - do make sure it is adapted properly to reduce the need for adjustments (and contamination). You can use the above hack, hair clips, hair ties, around pigtails, monkey pick ups.... the possibilities are endless! . As working parents, life has always been a delicate balancing act; but this difficulty has increased in the Covid era - with restrictions, remote working, and concurrent schooling from home. . This morning was a classic example - it was manageable whilst attending a multidisciplinary zoom meeting and providing IT support for two students multiple meetings. An out of field oncall phone call came in, and all off a sudden - too much was happening all at ...once and I couldnt keep up. . This is not unique though - according to the Washington Post, the average length of an uninterrupted stretch of work time for parents during COVID-19 was three minutes, 24 seconds." . And please dont think that you cannot be infectious if you are not symptomatic; most cases have been spread by individuals before they realise they were infected.... . A special shout out to Mr M who modelled an oversized valved mask for me. This also serves as a good reminder that one-size-fits-some sizing are usually designed on an adult male mannikin, and not suitable for everyone. So please make sure you choose a mask which covers your mouth, nose and chin, with no gaps on the sides. . And it was even more poignant when I learned of the back story. . Paralyzed is inspired by Deltas experience of nerve injury following salivary gland surgery resulting in paralysis of her tongue. And whilst I do not perform this procedure, there are similarities with all surgical risks:... No surgery is risk-free. And whilst risks can be mitigated and complications can often be managed, the only way to remove risk is to avoid surgery. Complications will happen to every surgeon. I have had my fair share. The impact of a specific complication are very individual (e.g. loss of tongue movement for a singer), hence the need for personalised treatment plans, and informed consent. It is important to identify and rectify a complication early, as there is often recovery of function with time +/- therapy. . We were one step closer today when we welcomed Dr Suhaib Alqudah (Melbourne Oral Medicine and Facial Pain Centre) to Queens Parade Specialists. . Suhaib worked as a senior dentist in Newcastle for a number of years before completing the clinical training of his Doctor of Clinical Dentistry in Oral Medicine (Medicine, Dentistry & ...Health Sciences at UniMelb). He is now finishing his PhD, so when he's not busy seeing patients with Oral Medicine needs, he's in the lab researching complex mucosal disease. . Today, between patients - and between bites ( thanks Cavallini Bakery & Patisserie) - we discussed patient care and compared notes on home schooling . So glad school returned to normal this week!
