15.01.2022 Gobbato L, Paniz G, Mazzocco F, Chierico A, Tsukiyama T, Levi P, Weisgold A. Significance of crown shape in the replacement of a single central incisor with a single implant-supported crown. Quintessence Int 2013;44:407413. The aim of this study was to analyse the significance of natural crown shape when replacing a central incisor with an implant-borne crown. Included in the study were 60 patients, who received a single implant-borne crown to replace a maxillary central inc...isor, and whose final aesthetic result had gingival symmetry, dental symmetry, no black triangles, and no diastema. Most (65%) of the teeth that were triangular in shape received an additional restoration on the adjacent central incisor, while some (40%) of the square-tapered group required an additional restoration, and few (13.3%) of the square group required an additional restoration. All teeth requiring additional restorations had their original shape changed to a square form. Additional restorations included composite resin restorations, porcelain veneers and crowns. When obtaining consent from a patient requiring the replacement of a maxillary central incisor with an implant-borne crown, the potential need and cost (financial and biological) for an adjacent restoration should be discussed. The shape of the natural tooth may be a predictor of the need for an adjacent restoration. The patient should also be advised about the potential need to change the original shape of the tooth in order to close black triangles, diastema or to achieve symmetry.

When this is all over and we are finally able to return to normal practice I foresee additional measures required of us to safeguard our patients, our staff and ourselves. 09.01.2022 It was an honour to be invited to Japan to attend Tokyo Medical and Dental University, where I had the opportunity to observe the use of optical coherence tomography in an intra-oral scanner on a live subject. The short video shows an all-ceramic crown being scanned and a bubble in the cement layer being discovered unexpectedly. Unlike current intra-oral scanners on the market this technology is able to produce a true 3-dimensional image with volume, much like micro CT (witho...ut the radiation). The scanner has sub 10 micron resolution and uses infra-red which can penetrate soft- and hard-tissue. Sub-gingival crown margins, marginal gaps, residual cement, cracks, periodontal pockets, sub-gingival calculus, caries, the dentino-enamel junction, and even the orientation of connective tissue fibres in the periodontium can be visualised. I was awed. This is the future of intra-oral scanning.
