24.01.2022 Another email that has brightened up my day! What a smile Baby-E has! The news of Baby-E’s (who is now 4 months old!!) is quite special to me as I remember celebrating hearing his heart beat on his pregnancy scan last year with some champagne (I posted about this champagne in September)! Not all fertility journeys are easy. Mummy-A and Daddy- L have been trying for pregnancy for 4 years. After 11 embryos transferred in IVF cycles and 3 miscarriages later, this little boy mad...e a much anticipated appearance! Mummy-A tells me that they are already starting to think about a plan for trying for number 2! 22.01.2022 Introducing Baby-H, whose feet are just as cute as his button nose! Baby H's story is a little different to his big sister, whose pregnancy happened straight away for Mummy-T and Daddy-C. 8 months after trying for their second baby, Mummy-T had an ultrasound that revealed severe endometriosis. I performed a laparoscopy procedure to treat the Stage 4 endometriosis and found both fallopian tubes were blocked at the time of procedure.... So we proceeded to IVF and little Baby-H is the beautiful result of our second embryo transferred. I’m also so pleased to hear that they had a wonderful experience with their delightful obstetrician @drAlexPolyakov who looked after them during their pregnancy! 19.01.2022 It is the current national recommendation for all couples before trying for pregnancy to consider the Triple Reproductive Genetic Carrier Screening, screening for 3 common genetic conditions (Cystic Fibrosis, Spinal Muscular Atrophy, and Fragile X Syndrome), to see if you are at risk of having a baby with these 3 conditions. 19.01.2022 We have all been living under very strict social distancing restrictions that 6 months ago would have been unimaginable. Following the Road Map to COVID Recovery yesterday, I would like to tell my patients that there are no changes to my current practice. I will still see most of my patients through telehealth. I will still see patients that need examinations and colposcopies in my clinic with extra infection precautions. All fertility treatments are still proceeding. We are ...still awaiting further information about elective surgeries. Currently some non-urgent gynaecological surgeries are delayed. There are many well established controls in place at my clinic to minimise risks of infection, including: . - masks for all staff and patients. . - Staggered appointment booking. . - Symptoms and travel history screening. . - Temperature checks. . - Physical distancing where appropriate in clinic. . - Strict hygiene and disinfection practices. . - Dedicated solo practice waiting rooms to avoid coming into contact with other doctors’ patients. 18.01.2022 What a delight it is to hear from Mummy-A and Daddy-J about the arrival of Baby-M! Baby-M decided to make an entrance 3 weeks earlier than anticipated, and as Mummy-A told me, had a little rough start with jaundice and hypothermia. I’m so glad to see them all safely home. They had to make the most being in lockdown with a new baby! What a beautiful family, and their fertility journey was not an easy one! I first met Mummy-A in 2016 when she started trying for pregnancy.... She came to see me due to increasingly severe period pain which she needed to go to the emergency department for. I performed a laparoscopy and extensive endometriosis was treated. Mummy-A’s pain significantly improved. Then in subsequent testing, it was revealed they had both male and female infertility factors that were impacting on their chances of pregnancy so 3 months after laparoscopy we proceeded to IVF. They fell pregnant on their second IVF cycle, but unfortunately suffered a miscarriage. We tested the miscarriage which showed an abnormal number of chromosomes. This is a very common reason for miscarriage and is usually a random event in nature, and the same abnormality is not likely to repeat itself. Beautiful Big-Sister-E resulted from their next IVF transfer cycle! Just before Big-Sister-E turned 2 years old, Mummy-A and Daddy-J returned to see me, planning to transfer their one remaining frozen embryo. And this little embryo became gorgeous Baby-M. I’m so pleased for you all and thank you for sending me your photos! 17.01.2022 As some of you know, I come from Taiwan and grew up in Australia. I was taught to embrace my Aussie background and remember my Asian heritage. I try to teach my kids the same, but keeping balance between Eastern and Western traditions is not always easy! My son just lost 2 lower baby teeth. We have kept the Western tradition of the Tooth Fairy and he is now $5 richer. But I couldn’t resist a Taiwanese tradition my mother taught me when I was a little girl, of throwing... the lower baby teeth onto the roof to help the adult teeth grow straight. . However... as far as my son knows... his teeth are now with the Tooth Fairy. I couldn’t very well give him back his teeth to throw onto the roof! So I had the roof-throwing-duty today. One of the things I love about what I do is the continuity of care with my patients. I first met Mummy-A in 2016 not for fertility issues, but for abnormal cervical (PAP) smears that required treatment. I performed a cervical procedure call LLETZ (large loop excision of the transformation of the cervix) to remove the abnormal cells on her cervix. She started trying for... pregnancy off and on in the following 2 years and we then proceeded to IVF when she hadn’t conceived. In our first IVF cycle I collected 14 eggs, but none of the eggs fertilised to form an embryo. So we tried again, adjusting our fertilisation method in the laboratory. This time, 12 eggs were collected and Baby-C was the first embryo we transferred back! 02.01.2022 What do you know about UTERINE TRANSPLANT? Uterine transplant is a potential future fertility option for women who were born without a uterus or has had a hysterectomy. Currently, women in these situations, who still have intact ovaries can have eggs harvested and undergo IVF to store embryos. However, they will require a gestational carrier (a surrogate) to carry the baby. Or they can pursue adoption. A recent publication in RANZJOG (Royal Australia and New Zealand Journa...l of Obstetrics and Gynaecology) surveyed over 50 women without a uterus to assess their views and explore their attitudes towards uterine transplantation. Uterine transplant has been performed under clinical trials for a few years now worldwide, and offers these women a potential option to carry their own baby. Uterus from a donor will first need to be removed in a complex operation with its blood vessels intact (usually 6-10 hours to perform) and then transplanted into the recipient, where the blood supply is re-attached (usually takes 4-6 hours to perform). Then the recipient will need medication to suppress her immune system to prevent her body rejecting the uterus. 12-18 months later, an embryo that was previously stored can be transferred into the uterus. After pregnancy, once the woman has completed her family, the uterus will be surgically removed, so she is able to cease immunosuppressant medication. The first baby born from a transplanted uterus was in 2014 in a Swedish trial. Since then 18 more live births have been reported. It is a treatment that is still in the clinical research phase, and more than 15 centres worldwide are pursuing this research.
