08.01.2022 This lovely 65 year old lady was referred for restoration of her front tooth after the crown had come off while eating. The post/core crown was made over 15 years ago. Fortunately, she did not fracture the root in the process, however the remaining tooth structure was minimal - approximately 0.5mm supra-gingival. This situation can be unpredictable to restore and in most patients, an extraction followed by an implant or bridge is the safest option in the long term. The pati...ent on the other hand wanted to do everything possible to try and save the tooth. Fortunately in her case, the factors were favourable to consider this as a viable treatment option..with some out of the box strategies. I did my best to delay the implant for another day. Treatment performed: - Cast gold post core on tooth 21 - PFM crowns on 11 and 21. - 360 degree fine gold margin on 21 to allow a minimal marginal prep and maintain as much cross sectional thickness to the tooth. Margin approximately 1-1.5mm sub-gingival. - Porcelain buccal margin on tooth 11. - PFM Cantilevered bridge 23-22 Technical work by Ricky Lee @ilkirickylee . @ South Coast Smiles

08.01.2022 Salvaging a case that was referred for restoration of a front tooth on a young patient. The implant in this case was placed too deep and too far labial resulting in deficiency of soft tissue. The patient was not interested in attempting soft tissue grafting or removing the implant and starting again. A porcelain fused to metal crown was made using an angulated screw to move the screw access palatally, and the soft tissue was replaced with prosthetic pink. Nice work by Carolyn Kyle @fusionoralart on recreating the gingival pigmentation. Fortunately the patient had a low lip line so the join line is not noticeable.

Digital smile design helped communication with the ceramist and patient to ensure the patient knew what she was getting. A diagnostic wax up and direct mock-up was also used. @ South Coast Smiles
