20.01.2022 As Queensland borders open and Victoria goes into lockdown, I thought I’d post an update on what we know about COVID-19 in pregnancy. A recent UK study shows 0.5% of pregnant women have required hospital admission with COVID-19 (NB: this is UK data). Most of these women were in the later stages of pregnancy, suggesting that women in late pregnancy are more severely affected by COVID-19. However, outcomes for pregnant women are similar to that of the general population. Chance...s of a preterm birth in this study were 25%. The most common reason for this was for the benefit of mum’s health. About 2% of babies tested positive within 12hours of birth, suggesting a possible but low chance of transmission to babies during pregnancy. 19.01.2022 Fibroids Part One: Gynae Issues Fibroids are benign overgrowths of the muscle of the wall of the womb. They are very common, affecting up to 70% of women. In most cases, they are small and do not cause any symptoms. Sometimes, they can grow larger and cause problems such as pain, bleeding, urinary and bowel symptoms.... Fibroids tend to shrink after the menopause. Other management options include non-hormonal or hormonal treatments, uterine artery embolisation and surgery (including hysterectomy).

It’s important to be aware of your options and the possible things that can happen during birth. It’s great to consider what your preferences are. It’s part of being prepared, informed and empowered. It’s also essential to discuss any preferences with your care provider, so you are all on the same page. However, you don’t HAVE to have a birth plan. You can also have a bir...th plan in your head rather than one on paper. Also (and this is the most important bit), don’t see your birth plan as something to measure yourself against. 15.01.2022 HEAD BANDS ON BABIES Sarah here (paediatric nurse & CPR Kids founder). Let me tell you a (true) story. I was shopping at the supermarket a little while ago (pre...COVID) when I saw a mum shopping with her young bub. All I could see was bub's legs and body, the hood of the pram was covering her face. I noticed that she was kicking her legs furiously, which I thought was a little strange so I ducked down a bit to look at her face, from an acceptable distance that a stranger should maintain from someone else's baby! What I saw sent a shock of adrenaline through me - I jumped straight over and grabbed mum's arm (she was just browsing the shelves, one hand on the pram) and at the same time flipped back the hood - the head band that the baby was wearing had slipped down over her nose and mouth, and she was turning a dusky purple colour. Mum and I seemed to simultaneously reach down and pulled the head band off her nose and mouth, and bub immediately let out a huge cry. It's a bit of a blur, but I remember mum being stunned about what was happening. She picked up her baby who continued to cry for a bit then settled down, perfectly fine after the ordeal. Mum wasn't so fine. Mum was crying, saying that she knew not to let bub sleep with the headband on, but didn't realise there would be any danger from having the headband on whilst awake. However, just head movement or rubbing can move a headband down over the face. They are fine for a photo or while being held by someone, but please don't leave a headband on a baby if they aren't close, awake and in direct active supervision. And of course, NEVER when sleeping. In pregnancy, fibroids can cause some specific issues. In most cases, they are small and do not cause any problems at all. However, if the fibroid is large, it can increase the risk of things like: miscarriage, having a breech baby, going into labour early, having a small baby and heavy bleeding after birth. ... Sometimes fibroids can grow during the pregnancy and this can cause pain. This pain is usually self-limiting and managed with simple pain relief. Sometimes a Caesarean section will be required due to the fibroid but, in most cases, a vaginal delivery is possible.

13.01.2022 I gave a talk to the Greenslopes Private Hospital junior doctors and The University of Queensland medical students yesterday about #ovarianhyperstimulationsyndrome. Simply put, women receiving treatment for infertility are sometimes given medications to get the ovaries to work harder to produce more eggs. Some ovaries are more sensitive to this than others. This may lead to an overproduction of eggs as well as certain hormones that cause you to swell up. This can make you quite unwell. Fortunately, this condition is not very common and it does resolve by itself with time. It is a deeply personal journey that mothers and babies take together. Sometimes that journey might not go how you hoped. Sometimes it might look very different from what you envisioned. It may be harder, it may be shorter then you wanted. Or conversely it might last much longer than you expected. But the value of breastfeeding journeys are not defined by how long they last, or whether they're exclusive breastfeeding, or whether you pumped and fed, or supplemented, or tube fed, or used a supplemental nursing system..... All journeys, even the ones that go off road, are valuable, important and worthy of celebration. Every feed, even if it was a single syringe of colostrum that was expressed antenatally, is valuable, important and worthy of celebration. And that is not failure.

These micronutrients are essential for baby’s brain development and help prevent conditions such as spina bifida. Iron is also important for mums to prevent anaemia. Levels can also be depleted during breastfeeding. Who else loves spinach?!

Genetic Carrier Screening has been around since 2018. You and your partner can have this testing done before you fall pregnant. It doesn’t tell you anything about your baby. What it tests for are genetic mutations that you might carry that don’t affect you (ie, you’re an asymptomatic carrier). However, if both you and your partner both carry the same mutation, there is a 1 in 4 chance that your baby will actually have that disease and be symptomatic. The 3 main genetic conditions tested are: cystic fibrosis, spinal muscular atrophy and fragile X syndrome.
