25.01.2022 Totally the wrong message here. If its a suspected stroke, STRAIGHT to CT and CT angiogram. This is the best test, even in kids. Not only is it more accurate, but it is faster to access and faster to perform, and usually doesnt need a general anaesthetic. 20.01.2022 Not 100% accurate but it is definitely not properly resourced. The comment that they have no sympathy for teams that dont recruit is ridiculous. We have lost applicants because conditions and resources interstate are better, and we interviewed a great candidate 18 months ago who we still cant appoint (hes a professor in Germany) because we cant get over all the bureaucratic hurdles (visas, AHPRA etc) and we arent getting any help at all with that from NSW Health. Highl...y trained INRs dont grow on trees either: if you advertise - as we have done - you may get one suitable applicant if youre lucky. Even if we do appoint, they wont give new appointees operating time - we just have to share the 1 paid day a week between two hospitals that we have. Weve been trying to get trainees on board for ages too, but (a) until recently we couldnt get a position funded and (b) no-one wants to train when the conditions are poor - theyd rather do something else. None of this is the fault of one hospital, one administrator or even one bureaucrat in ministry. There is a deep level of political ignorance about the scale and cost of this problem. What money there is gets thrown at outdated or irrelevant issues by well-meaning but ill-informed bureaucrats. And dont get me started on the stadiums. Remember FAST: get straight to the nearest emergency department if you or someone you know develops sudden lopsidedness of the FACE; if there is difficulty in lifting one ARM; if there is difficulty in understanding or generating SPEECH; and that TIME is critical - the sooner you get help, the better chance of recovery. #stroke #ECR

11.01.2022 It is surprising that there is any controversy about this at all, but the implementation of training standards and minimum case loads for skills maintenance for INR practitioners in Australia has met with unexpected resistance from some quarters, including hospital executives. Thankfully, medicine as a profession has historically done a pretty good job of self-regulating and ensuring that our patients are safe. The Conjoint Committee for Recognition of Training in Interventio...nal Neuroradiology (CCINR - www.ccinr.org.au) is a collaborative group of radiologists, neurologists and neurosurgeons who work to ensure that training standards for INR are fair but sufficient to ensure a good basic skillset, and to set minimum case numbers for practising INRs to ensure that they are doing enough work to keep their skills current and fresh. 09.01.2022 A real problem. More people with less training doing more complex cases. Regulators, Colleges and hospital administrators unwilling to address the issue. One (partial) solution: create "devices for dummies" - tools that anyone with a bit of training can use and get familiar with for most of their cases.

03.01.2022 Well its been a while since I posted on here. A combination of being super busy and dealing with some personal issues has kept it that way, but its been year another year of rewarding care of literally hundreds of patients. While it is fantastic that most have done very well and returned to normal or near normal life after their aneurysm, stroke, AVM or other illness, it is always sad and sobering to reflect on those patients who didnt fare so well, along with their families. I wish all of my patients, their families, their carers and my extraordinary teams at Prince of Wales and Liverpool Hospitals, along with our patient and caring secretaries a very peaceful and happy holiday season and best wishes for the New Year.
