21.01.2022 It's great to see that newer "biological" therapies are showing the same promise in head and neck cancer as they have already demonstrated in melanoma. Hopefully in the near future they will be available to all our patients as part of their routine treatment. This preliminary trial looked at patients with advanced head and neck cancer that had not responded to standard treatments, generally a death sentence. There was a 30% reduction in the risk of death if you took the new drug which may not sound like much but bear in mind this is very late stage disease and would be generally considered incurable. The results might be much more impressive if it is used in earlier stage disease. The side effects of these "targeted" drugs tend to be much more bearable than traditional chemotherapy drugs which are often pretty brutal.

15.01.2022 As a follow up to the HPV article I mentioned before. These are disappointing figures out of the US which show the resistance to vaccinations there. The figures for Australia are better with 70% of eligible girls vaccinated in 2012 , which was right at the start of the vaccination campaign , now extended to include boys and girls. Those figures are not great though. And guess who had the lowest rates, the good old Eastern Suburbs of Sydney! - 59%!! There is a lot of work still to do.
15.01.2022 Less pain is always good news. A regular problem of ours is what painkillers to give to children after tonsillectomy? It seemed for a while like we were running out of options. Which is a real problem. It is still one of the most common operations performed on young children ( from 2 years up). It's a painful operation, over 50% of children had to return to their family doctor or emergency dept for more pain relief in the first week after their surgery in a recent study. Ther...e are a lot of medicines you cannot give to small kids. Aspirin ....no it causes paralysis. Morphine......no it causes constipation, confusion, respiratory depression. Codeine ...probably shouldn't as it can cause breathing difficulties. Even Ibuprofen known as "sweety medicine" in my house gets the stink face. Many of my colleagues won't even operate on you if you take Ibuprofen (Nurofen or Advil) in the 2 weeks before surgery because they are worried it may increase your risk of bleeding. Many ENT surgeons in Australia won't prescribe Ibuprofen to their paediatric patients despite there being strong evidence that it is the most effective post-operative pain relief and no evidence that it causes bleeding. Which leaves only panadol (paracetomol or acetomiphen) as the lonely survivor in the competition to be the "perfect" pain-killer. Well even it's not perfect , not even close. For a start it doesn't really work that well, it is easy to overdose on and even small excess doses can cause horrific cases of liver damage. At least it was readily available and, taken at the right doses, was OK. Until everyone became worried that it exacerbated asthma. That's a heck of a lot of kids with no pain relief options. This paper though should reassure us that Paracetomol (and Ibuprofen) are pretty safe painkillers to use even in moderate asthmatics. Which is good news for my patients and good news for me because I won't have to change all my advice leaflets. Ibuprofen and paracetomol combined will stay as my first line recommendations for post operative pain relief after paediatric surgery. Oh and I forgot to mention ice cream! Plenty of ice cream. 15.01.2022 In an era where we seem happy to part with our hard earned cash for virtually any old junk. Fancy a Gatorade Organic anyone? Its good to hear that there are some things that are definitely worth forking out for. Fissure sealants are plastic coatings that dentists apply to your children's adult molar teeth. They are invisible, painless and guess what they work. Really well! They cut your child's risk of cavities in half. That has to be a sound investment.
13.01.2022 This is an important point. Treatment for cancer is not great. Treatment for Head and neck Cancer is possibly some of the worst. The ability to prevent cancers is the holy grail of cancer medicine. HPV is a sexually transmitted virus that causes cervix cancer in women and also tonsil and throat cancers in men and women. It is preventable with a single vaccination. That vaccination only works if you give it to kids before the virus gets into their bodies. That means vaccinati...ng before they start having sex at the age of 12 or 13. Obviously that raises tricky political questions about how best to get parents to vaccinate their kids and conversations some people would rather not have. This paper suggests that giving parents the option of opting out actually increases their likelihood of opting in. Opting in means more children get vaccinated, if enough children get vaccinated then we get "herd immunity" in other words the disease itself disappears in the community because there is nowhere for it to live. That way we reverse the epidemic of HPV caused tonsil and tongue cancers we are seeing in our clinics. See more

04.01.2022 The Pandora's box of cancer screening. It's important to bear in mind when you read this beautifully written article that the author does not have cancer. She has a risk of cancer, but the uncertainty of that risk sounds almost worse than cancer itself.
