My advice for EVERY woman and couple planning on conceiving in the next 12 months is to see an obstetrician BEFORE starting to try for a baby. In particular, a pre-pregnancy counselling session is especially important for women who have any of the following: - Difficulty conceiving (generally more than 12 months for women under 35 years of age or more than 6 months for women 35 years or older) - A history... of a complication in a previous pregnancy including miscarriage, stillbirth, preterm birth, pre-eclampsia, fetal growth restriction, fetal abnormality or traumatic birth - Gynaecological issues including PCOS, endometriosis, uterine abnormalities (including bicornuate and septate uterus) and irregular or long menstrual cycles - Underlying medical conditions such as epilepsy, diabetes, kidney disease, autoimmune disease, thyroid disease or hypertension - A family history of any significant genetic abnormality - Mental health concerns, especially significant anxiety or depression This appointment is the perfect opportunity for us to: 1. 04.01.2022 Yesterday was world prematurity day. Each year, approximately 8% of babies are born prematurely in Australia. For some babies this has little impact, but for many others it can affect the chance of survival and cause a number of significant medical complications including cerebral palsy, brain haemorrhage and chronic lung disease. I have a particular interest in preterm birth prevention, working in the Preterm Labour Clinic at the Royal Women’s Hospital and offering the same ...expert care to women in my private practice. Evidence has shown that a dedicated preterm birth service is critical in managing women at high risk of preterm birth. Women with the following risk factors should seek specialist advice: * Previous preterm birth or second trimester delivery * Prior cervical surgery (cone biopsy, two or more LLETZ/LEEP procedures) * Uterine anomalies (including bicornuate, didelphys, septate uterus) * Multiple procedures requiring dilatation of the cervix (three or more curettes for miscarriage or termination of pregnancy) * Obstetric cervical trauma Management strategies include: - Cervical surveillance, where I perform fortnightly ultrasounds from 14-26 weeks to assess the length of the cervix and look for other factors that may increase the risk of premature birth.
