25.01.2022 WHY THE CONTROVERSY OVER TMD? An extract from Dr Spahls guest editorial for Cranio At the heart of all the controversy over what constitutes correct diagnosis and treatment of that certain pathophysiologically degenerative orthopedic, neurologically manifest global mosaic dynamic flux referred to (sometimes) as temporomandibular joint-pain-dysfunction disorder (TMJD), is surely the central notion of the actual significance of functional condylar position. There is one sch...ool of thought which holds that it has everything to do with the dysfunctionally induced TMJD symptomatic picture. An opposing school of thought basically contends that it does not. (Both are actually right.) One thing however is certain. Both sides feel compelled to cite the literature to act as a source of support for the veracity of their position at the expense of the repudiation of the others. Does this mean that somebodys literature is lousy? Yes, probably so. But if so,why? Could it have anything to do with the type of measuring rod being used? In brief, orthodontics has to accept the fact that, like it or not, it is intimately one with TMJD therapeutics on an orthopedic and myofunctional level. Similarly, the "GMDF (Global Mosaic Dynamic Flux) of the field of TMJD diagnosis and therapy is intimately entwined with not only occlusion, but also necessarily, with that subsection of the discipline of orthodontics that is known as Functional Jaw Orthopedics. The two subdivisions of ortho and TMJ are symbiotic fraternal twins (~I the mothering super-science of Global Maxillofacial Orthopedics - plain and simple - and the literature agrees. 05.01.2022 PAIN EXPLAINED FROM CAUSE TO CURE What is TMJ? TMJ is a serious and painful degenerative jaw joint condition that goes by many names: TMJ syndrome, temporomandibular joint syndrome, temporomandiublar joint pain dysfunction syndrome, temporomandibular disorder (TMD) etc. The initials stand for temporomandibular joint, the anatomical name for the jaw joint back by the ear where the lower jaw attaches to the base of the skull.... How do you diagnose it? Patients suffering from TMJ pain-dysfunction syndrome, or TMJ, exhibit jaw joint clicking during chewing, sore jaw joints, sore jaw muscles, limited opening of the mouth, clenching or grinding of the teeth, especially during sleeping, neck aches, earaches, tooth pain with no dental cause, ringing in the ears or a stuffy feeling in the ears, and many other unpleasant symptoms. But the BIG thing TMJ patients suffer from is chronic recurrent headaches and the big finding of modern times is that the worst of these headaches, the migraine, is the end stage of the long degenerative pathway of this condition! How do you treat it? You can treat the symptom i.e. the pain, using various medicines, pills, or shots, in conjunction with a variety of other pain relieving and/or relaxation treatments. But this is just a band-aid solution. You need to treat the cause and heal the damaged temporomandibular joint itself. Fixing TMJ Healing seriously damaged temporomandibular joints (TMJs) and relieving the serious problems associated with it involves realigning the bite of a lower jaw that is trapped too far back during chewing by its own upper jaw, (malocclusion). A bite that is back too far drives the lower jaw too hard into the back of its own socket (where all those nerves and arteries course through). Correcting this with proper treatments delivers that offending lower jaw a bit more forward permanently so that, during biting, the back ends of the lower jaw (condyles) are no longer jammed into their own sockets. This stops the repetitive compressing and injury to those same nerves and arteries by the lower jaw at full biting force. That means using FUNCTIONAL APPLIANCES!!! There are many kinds of TMJ-damaging bad bites and many ways of doing this. Biting back too hard causes the problem and realigning the bite more forward cures it.

03.01.2022 The ABC of TMD The temporomandibular joint (TMJ) and the trigeminal nerves are some of the most important structures of the face. When the TMJ is out of alignment or compromised in other ways, it can affect many other areas of the head and neck. Because the trigeminal nerve, which innervates the TMJ, also innervates so much of the face and neck, pain all over the head and face and eventually migraines, may result when there is any sort of disorder in this important structure ...(TMD). The TMJ is by far, the most active joint in the body, involved in over 2000 swallowing actions per day, every day of your life. The TMJ is also instrumental in many other facial functions including; chewing, speaking, and breathing. Thats why this joint is such a busy and important one in every person every day. It is the only joint structure in the body with two sides that affect each other. It is often considered to be the last vertebrae because of the enormous effect the lower jaw has as a counterbalance to the head. This means that if one side of the jaw is out of alignment, it puts the mandible out of balance, which can cause numerous problems to the head and face. It has been shown in research that the condyles of the mandible almost exactly duplicate the condyles of the atlas that articulate with the skull. If the TMJ isnt working right, its almost inevitable pain will follow, and often the end of that continuum is the migraine headache. This has been thoroughly researched and proven by neurologists, and as Dr Spahl explains in his lectures, TMD can place pressure on the Trigeminal nerve and result in chronic orofacial pain and migraine. This is what Dr Spahl specialises in teaching in his lectures, and why it is so important for all medical professionals not just dentists to have an understanding of in order to diagnose and seek the appropriate treatment.
