24.01.2022 I'm so glad to see that the conversation around obesity is changing. Let's get rid of the fat shaming and bias in healthcare and look at some real world solutions. "Obesity is not defined by ones weight or size, but by whether a persons body fat impairs their health, according to new clinical guidelines for managing the condition." 23.01.2022 In a nutshell... yes. Strength training is not only safe in pregnancy but actually recommended! Lets face it, motherhood involves a heck of a lot of lifting and carrying, so being strong is of great benefit! However there are some caveats. 1. You should get clearance from your medical team and check for any contraindications 2. Unless you are new to lifting, you should anticipate that the amount there you can life comfortably will decrease as your pregnancy progresses.... 3. You should avoid the Valsalva manoeuvre when lifting (Due to increase in blood pressure as well as strain on the pelvic floor). If you have to hold your breath, its too heavy! 4. Signs and symptoms to watch out for include heaviness in the perineum, bulging in the middle of the abdomen and pain. 5. Standard precautions associated with exercise in pregnancy apply- check out the 2019 Canadian guidelines. If you need help designing your exercise program, drop me a pm! 21.01.2022 Freedom to me is being able to get out in nature and exercise with friends and family. Its having trust in my body... to know that I will not break. To be confident that the niggles that I feel will resolve with time (and a bit of TLC). To know that with each passing year, although I cant jump as high or run as fast as I used to, I can still do all the things I want to be able to do. To know that I can fly down a mountain on my snowboard or kayak on the ocean and enjoy wa...tching the seals bask in the sun. I can jump on a bike with my kids and enjoy the beautiful scenery that Perth has to offer or throw on a backpack and go hiking through the forest with little preparation. These are the reasons I do mobility and strength work almost every day. To me, movement means freedom. 21.01.2022 Great job by Grainne, Emma and Tom to get these guidelines up and running (pun intended...). Anyone working regularly with new mums know how little information there is on when it is appropriate for women to return to running after childbirth. Most guidelines have about 10 pages on exercise in pregnancy and then maybe 1-2 paragraphs on the postnatal period (if you're lucky!!). This team spent MONTHS trawling through the literature to find the best evidence available (sadly th...ere isn't a lot, but hopefully this will encourage more research!) as well as speaking to physiotherapists around the globe to get an idea of how we are working with new mums to support them in their return to running. I was thrilled to be involved in a small part (reviewing the content and adding my thoughts/ clinical experience), as I do think this is a fantastic document that has already helped so many women around the world on their postnatal journey. Having it published in the BJSM also helps increase awareness about pelvic health and the important role that pelvic health physiotherapists play.

12.01.2022 We all have different fitness goals ...and our goals do tend to change over time. 10 years ago, I was recovering from childbirth and wanting to get back into regular physical activity. 7 years ago I was passionate about strength training as I was recovering from significant low back pain. 4 years ago I was training for my 3rd Dan Black Belt. 3 years ago I was training to compete in the World Cup in TKD.... Now, I'm training to compete in a sprint distance triathlon. Yet its not quite that simple. - Erica Smith (@ericasmith.sex.ed) In Ericas GGS article, Creating Welcoming Gym Environments for Trans and Gender Nonconforming Athletes, she interviews and shares the experiences of several trans and gender nonconforming (GNC) athletes who experience anxiety and fear around going to the gym due to the judgment, harassment, and discrimination they may face in those spaces. Want to create a more welcoming environment for trans and GNC athletes? According to the athletes Erica interviewed, heres how: 1Educate yourself and your staff on the discrimination trans and gender nonconforming folks face. 2Provide single stall or gender neutral bathrooms, locker rooms, and changing areas. At least one private changing area can go a long way. 3Avoid delineating between mens and womens workouts and weights. 4Have a zero-acceptance policy against harassment including harassment based on gender identity. State this policy explicitly on your gyms website. 5Learn from your mistakes. Your gym doesnt have to be perfect, but you must be willing to learn and be humble when you mess up. If we limit the access of trans and gender non conforming members, we are essentially denying them the right to health, wellness, and fitness. 10.01.2022 DO YOU HAVE PELVIC PAIN? Some of my physiotherapy colleagues are completing a research project involving persistent pelvic pain (PPP) and associated psychological factors. PPP can include diagnoses such as vulvodynia, vaginismus, chronic prostatitis, bladder pain syndrome, pudendal neuralgia and so on, or undiagnosed pain perceived anywhere in the pelvis. We know there are negative cognitive, behavioural, sexual and emotional consequences associated with PPP. Last year, a scr...eening tool titled the Pelvic Pain Psychological Screening Questionnaire (3PSQ) was developed via a panel of experts. It is a short questionnaire designed to detect the presence of psychological factors in individuals with PPP. This is the first screening tool specific to a PPP population that combines various aspects of psychological welfare. It has excellent potential to allow clinicians to make more informed decisions regarding pain management, guide appropriate referral to other healthcare providers and ultimately impact management outcomes in a positive way. Each year, we had an international trip which was the highlight of the year. This year, we were going to Manchester to help the Australian Diamonds prepare for the Comm Games. The week before the team was flying out, I did my ankle. I tore all 3 ligaments and ruptured the joint captule. My leg was purple up to my knee. My whole ankle was filled with fluid. I missed the trip. Of course. But since I was at the AIS, I had physio twice a day for the next month. One physio would spend an hour wrapping cotton wool around each toe and all the way up my calf, working the swelling up my leg. I was given the best treatment in the world using the latest research and equipment. In 2013 I gave birth to my daughter. Two weeks later, I was in tears because it felt like my vagina was falling out. There is no other way to describe it. I went straight to the hospital, where I was told that it all looked normal. NORMAL! At my six week check-up, I was diagnosed with a prolapse and recommend to see a Womens health Physio. Since that initial appointment, Ive spent probably over $10,000 trying to heal my body. And together with @lyzevansphysio, weve invested much more to create the @empoweredmother_ Program, so that other women dont have to go through what I did. I share this story to highlight the huge difference in the way we treat birth injuries and athletic injuries. If we could invest just a tiny percent of what we invest into athletes, we could save future generations of women from a life time of pelvic issues and the mental health issues that arise from these conditions. The ongoing cost to the economy of these conditions is huge. The cost of prevention, so much less. Amy Dawes from @birthtrauma.org.au has created a petition to include pelvic health physiotherapists in pregnancy and postnatal care for all women.
