22.01.2022 Good evening everyone. I just wanted to post a warm fuzzy. Someone made me very happy tonight with some great feedback on my new online education sessions. I thanked her, saying she made me very happy . She said this world could do with more , making each other happier. ... so it reminded me of this old story...... They brought balloons to a school. One was given to every student, who had to inflate it, write their name on it and throw it in the hallway. The professors then mixed all the balloons. The students were given 5 minutes to find their own balloon. Despite a hectic search, no one found their balloon. At that point the professors told the students to take the first balloon that they found and hand it to the person whose name was written on it. Within 5 minutes everyone had their own balloon. The professors said to the students: These ballons are like happiness. We will never find it if everyone is looking for their own. But if we care about other peoples happiness... well find ours too." 21.01.2022 Interesting morning read summary: Most efficacious face masks: N95, surgical, polypropylene, and handmade cotton face masks all appear efficacious for blocking respiratory droplets, while bandanas and fleeces seem to offer little protection, according to a study in Science Advances. Researchers tested the efficacy of 14 different mask types as follows: A male wearing each mask repeatedly spoke the words "Stay healthy, people" toward a laser beam inside a dark box. Droplets th...at passed through the laser scattered light, which was recorded with a cell phone camera, and a computer program then counted the droplets in the video. For reference, the speaker did the same while not wearing a mask. N95 masks were most efficacious, followed by surgical masks, then polypropylene masks and handmade cotton face coverings. Other masks were less efficacious. In particular, gaiter-type fleece coverings seemed to spread larger respiratory droplets into numerous smaller droplets. Of this, the researchers write, "Considering that smaller particles are airborne longer than large droplets ... the use of such a mask might be counterproductive." Longer incubation period? The incubation period for SARS-CoV-2 may be longer than previously assumed, a study in Science Advances suggests. Researchers studied nearly 1100 people who developed COVID-19 outside Hubei province but had a history of travel or residency in Wuhan; all had a known departure date from Wuhan and a known date of symptom onset. The median incubation period was estimated at 7.8 days whereas the generally accepted incubation period is close to 5 days. The 90th percentile was 14.3 days. The researchers note that, according to their estimates, some 10% of COVID-19 patients may develop symptoms more than 14 days after infection. They write, "This may be a public health concern in regard to the current 14-day quarantine period." Multisystem inflammatory syndrome in children (MIS-C): As of late July, 570 cases of MIS-C had been reported to the CDC. Patients ranged in age from 2 weeks to 20 years. The most common underlying condition was obesity. Most were tested for SARS-CoV-2, all of whom had positive results on PCR or serology. Some 86% of patients had at least four organ systems involved most often gastrointestinal, cardiovascular, and dermatologic. The researchers identified three classes of MIS-C. Class one (median age, 9 years) was usually accompanied by shock, cardiac dysfunction, abdominal pain, and elevated inflammatory markers. Class two (median age, 10) was characterized by respiratory symptoms typically seen with COVID-19. Class three (median age, 6) tended to be less severe and was often marked by dermatologic conditions. The mortality rate was 0.5% with class one, 5.3% with class two, and 0.0% with class three. 03.01.2022 What is a normal BGL? Well it depends on who you ask, when, they trained / studied, and what patient we are talking about. Almost definitely the BGL or blood sugar level of 4-8 mmol is a commonly touted / spruked / reflected upon range. But its lies. 4-8 mmol is not normal. In fact it is dangerous to rely on this old school approach. So where to start.... Lets review three different people . One with no diagnosis of Diabetes. we will call these Non-diabetics One with Type 2, and one with Type 1. I know there are other dysglycaemic syndromes, but realistically they all fall into these two big super groups. You need to attend my one day ( or 5 part Online Diabetes Series (CPD for nurses and paramedics) to let me fully unpack this can of worms), but I digress. Non-Diabetics ( dont get precious about the non person-centered terminology) If fasting, their normal BSL should be 3.9-5.4mmol. That is it. There is your baseline fasting Glucose. But usually we dont catch the patient in a fasted state. We take a random finger stick BSL. Normal here is 4-7.8mmol. Pretty close to the old 4-8 that you may have been taught. But Wait there is more. In primary health areas routine random BSL screening for dysglycaemia, would suggest that if you had a random BSL over 7.8 then a fasting Glucose is taken. Should this be elevated above 6.1, then a glucose tolerance test should be ordered. Now the Diabetics. With DM there exists either a deficit in Insulin (T1DM) or impaired insulin regulation and/or insulin resistance (T2DM). These patients dont have a normal BSL, rather a target range that their diabetes clinical support team (doctor, Diabetes educator, nutrionist, endocrinologist, mates at the pup or gym, Reiki master, facebook experts, yoga teacher and Tupperware lady) mandate. For T1DM the Fasting target is 4-8mmol and <10mmol 2 hours post eating. For T2DM the fasting target is 6-8mmol, and 6-10mmol 2 hours post eating. There it is We are done.
