25.01.2022 One of my fav procedure: Apicos. Traditionally labeled as The last resort or something to be done only where orthgrade treatment is not possible. I have a different take on it. Micro surgical retro root canals are not only treatment of choice at times but also saves unpredictable outcomes. This is true for treatments such as this where there is a fantastic custom post and core and crowns in good condition. We much rather treat the disease than try and remove sound restor...ations even though they are not the most esthetic. This is also a single visit procedure opposed to months of caoh2 dressings in multiple visits. Nothing is wrong or right. The decision is based on patient centered care and shared decision making. I would usually keep my submarginal flap more conservative but treating two teeth together both with reasonable root resorption with lesion extending distally especially for #11 made me go a bit wider than usual. I’ve gone back to silk sutures 4-0 here but would prefer 5-0. 24hours check today and patients is comfortable with no pain and minimal discomfort. External inflammatory root resorption with non healing granulation tissue. Pathology sent to lab, not always needed but I like to know the histology of the pathology assoc if possible. 24.01.2022 Distal crack 37 leading to SAP. Luckily not deep. Super long roots with 27.5 WL in mesials, S shaped. All restorations removed, crack explored and sealed, distal wall buildup. 23.01.2022 Symptomatic lower left 2nd molar with AP. Conservative/minimally invasive endo, both access and prep. The idea and rationale of the new age endodontics as I like to call it is minimal intervention and invasion of the root canal system preserving the dentine and keeping the tooth and root anatomy as close to the original. Are we ready to restore such teeth with more conservative overlays?... Mesials 20 04, distal 25 04. Two visits. Eddy sonic activation of irritants. Sealer Endosequence BC high flow with WVC. 21.01.2022 SIP 16 with a large beautiful amalgam. I always expects cracks when there is no caries associated. Before access is made, best to deconstruct. We found not one but two cuspal cracks (horizontal), can be seen in the image with the yellow arrow. The disto-palatal cusp had to be removed, actually came off almost by itself. Such cuspal cracks/fractures occur due to many years of cuspal flexure of unsupported cusps. Here, you want a restoration that bonds and flexes with the too...th/cusps. Obviously all this is more theoretical but there is some level of evidence available. This wasn’t an mb2 day (very rare). Obturated and core built with one of my favourate composites by voco admira fusion extra. The ormocer technology claims lowest polymerization shrinkage but who knows. 20.01.2022 Interesting and deceptive! Emergency access opening with my RD as patient was in severe pain. Even though all the canals seem patent, the mesials had excessive calcification right from the orifice to the apex. A slow and tedious struggle negotiating with no 6-8 k files. Besides the mesials were necrotic and both distals were vital and bled excessively. Typical SIP/SAP showing different stages of disease and pathophysiology in mesial and distal canals. 19.01.2022 Lifecycle of a carious tooth. #36. The initial bitewing and PA is from a year ago. Tooth diagnosed with SIP/SAP and initial endo/pulp extirpation with their dentist. The pain disappeared/life got busy and hence didn’t follow-up. 2nd visit 7 months ago, re pulp extirpation with a different dentist. Advised possible crack and ortho band placed. Finally, temp restoration chipped off and the band wasn’t fitting well. Ortho band removed, all previous temp restorations removed, n...o crack found under the scope. Full service endo with composite core, 2 visits. 25 04, WVC. Not a perfect margin as I would have liked to see on PA/BW but I couldn’t find any catch or loss of marginal integrity clinically. Hopefully can be polished off during the bonded onlay prep by my RD. Composites used- flowable/ Giomers and Packable SDI. How many people take PA/BW following proximal restorations? 18.01.2022 Restorative driven endodontics. All teeth/roots can be endodontically treated but not all teeth can be coronally restored. While we plan root canals, it is more important to plan the the restoration and the restorative material first. This planning itself will guide us though our endodontic treatment. Mandibular premolars or I should say all premolars can have high variation in their root canal system. This one split mid root. I though about a fiber post but then I would ...have to prepare the coronal root a bit more hence a composite post/deep restoration 4-5mm in to the canal was good enough. I still prefer the the convention method of removing overextended GPs by engaging the H file. No fancy gadgets or instruments needed here. 08.01.2022 Saving a tooth opposed to loosing a tooth! That is all what basic dentistry is about. There are plenty of replacement options but would you rather loose a part of you for an artificial replacement? Endodontics is not fancy because the work isn’t visible, isn’t cosmetic or smile enhancing work rather it’s the foundation work. We all know what happens to structures with poor foundations. ... Tooth 17. Large and deep carious lesion with defective restoration, symptomatic, all 3 roots going their own way and on top of that a disto-buccal angulation of the tooth. I find such upper 2nd molars hardest to treat right from placing the rubberdam clamp to final restoration. Completed in 2 visits with final core composite. 90mins each appointment. Asymptomatic now, fully functional besides a happy patient who didn’t want to loose this tooth. 07.01.2022 Caries driven buccal access #45. Extensive buccal caries leading to pulp exposure and subsequent AP. The occlusal surface has no caries and I had no reason to drill through it. Steps undertaken: . Debried and remove all buccal caries. ... . Isolate . Access the chamber and the canal. . Made a small notch on the occlusal wall of the buccal cavity just on enamel to achieve near straight line access. . Prep and disinfect. . Caoh2 for 3-4 weeks . Disinfect again and obturate, partial WVC with flowable and packable composite on the buccal. Will this maintain the fracture resistance of this tooth? It probably will. Occlusion plays a big role in decision making especially when large buccal restorations are present. 06.01.2022 Life cycle of carious teeth. #reactionarydentistry 2 years ago, 45 and 46 were diagnosed with extensive caries, 45 with pulpal involvement. 1 year ago, both 45 and 46 with pulpal involvement and 45 pulp extirpated as it got symptomatic. 6 months ago, 46 also got symptomatic and an initial endo was done. Symptoms disappeared but the bacteria continued to do what they do best. Eventually, symptoms reappeared. ... 2 visits, 20-25 04. Full service with composite core ready for a bonded onlay for both. A bonded onlay in such teeth can work well as the composite can be repaired distally for 46 in a few years if needed. 47 is missing and there will be some further bone loss distal to 46 in time.
