24.01.2022 Dr Courtney has kindly shared the results of his latest research below....... Researching Postoperative Tonsillectomy Pain Management with OxyNorm in Children Dr Courtney has recently completed research on pain management. Along with his colleagues their article was published in 2015 in the International Journal of Pediatric Otorhinolaryngology with the full reference below.... Dr Courtney and his team evaluated postoperative tonsillectomy pain management, where an information sheet on OxyNorm was included with a standard postoperative information versus the situation where only the standard postoperative information was given. It was discovered that a postoperative analgesia information sheet provides for a higher satisfaction and knowledge for parents and children. Patients with the information sheet had improved postoperative pain control, most significantly at day 5. Parent’s assessment of pain control was also superior, particularly at day 3 and day 7. The primary recommendation to come out of the study was that a postoperative information sheet on analgesia be given to all parents to provide for improved analgesia control and safe management of children in the postoperative period. All of Dr Courtney’s postoperative tonsillectomy patients now receive an OxyNorm analgesia information sheet. 19.01.2022 What symptoms do vocal cord nodules cause? Vocal cord nodules cause symptoms of a husky voice and problems with projection of the voice. The symptoms include: Roughness to the voice Reduced endurance of the voice [my voice becomes husky quite easily]... Increased effort associated with talking Difficulty singing high notes Chronic hoarseness Sometimes patients with vocal cord nodules may have a normal speaking voice but the hoarseness of the voice only becomes apparent when they raise their voice. In other patients the voice is always husky. The only way to truly diagnose vocal cord nodules is to go and see your ENT Surgeon and have a nasoendoscopy which will allow for a direct examination of the vocal cords. Following this diagnosis patients can then proceed to treatment. 05.01.2022 Our resident rhinologist (Dr Dan Robinson) has kindly offered some interesting insight into the value of a CT for a nasal obstruction..... A very interesting article was published recently in Laryngoscope, which looked at whether you can determine accurately whether you have a blocked nose with a CT Scan. What the study looked at was patients who were having a CT Scan of their nose and sinus’s. The patients where then asked to fill out a questionnaire called the NOSE Question...naire. The NOSE Questionnaire is used by Dr Dan Robinson pre and post operatively as well as many other surgeons to determine the degree to which a patient’s nose is blocked. The CT Scans where then looked at by a radiologist as well as an ENT Surgeon and scaled according to the degree of septal deviation the patient may have. The degree of septal deviation was then correlated with the patients NOSE Score to see if there was any correlation between the CT Scan findings and the NOSE Score. The results of the study are very interesting; there is no correlation between the degree of nasal obstruction and the findings on a CT Scan in relation to having a bent septum. The bottom line from this study is something which ENT Surgeons have known about for years. The best determinant as to whether a patient has a blocked nose is to take a history from a patient and examine their nose. Therefore, if you feel as though you have a blocked nose, but have a normal CT Scan of your nose, there is every possibility that your symptoms are being caused by something which wasn’t found on the CT Scan and can only be found by seeing an ENT Surgeon.
