25.01.2022 LUMINOSITY 2019 . Exercise is brilliant for anyone, but for those living with Endo it can be a really important part of your pain management . Exercise releases chemicals within our body which act as analgesics, down-regulating the pain pathway (and without the side effects of medications!)... . The difficulty with Endo is that when you have a flare, the fatigue and pain can make exercise the last thing you want to do! 24.01.2022 LUMINOSITY 2019 . There is some research that links endometriosis with increased levels of depression and anxiety - which makes sense! Endo can cause symptoms of chronic pelvic pain, menstrual irregularity, dyspareunia (pain with sex), bowel dysfunction and issues with fertility. Its human nature that you would worry about how this impacts your work, family and relationships . We do know that anxiety and depression can have a negative impact on pain levels, which then inc...reases worry and the cycle continues. So the stress and worry is normal, but not helpful. So what can we do to break this cycle? . Exercise has proven benefits for mental health via release of feel good chemicals. Its also brilliant for keeping us in the current moment, and these moments of mindfulness can help tame the mind when it starts to go wild with worry . 24.01.2022 Pain and your brain . A really important concept in long term Pelvic pain is not just the structural problem (eg Endo lesions), but also the changes to the nervous system and brain. . Everyone processes pain via their brain. So, for example if I cut my finger, the nerve from my finger (an Afferent nerve) travels to nerves in my spinal cord and makes a connection with other nerves. These nerves then travel up to the brain with the message. The brain processes the information, ...then sends information via a messenger nerve (an Efferent) down the spinal cord (making connections along the way) to the finger to say PAIN . Now, this nervous system pathway in someone who hasnt had pain before, is like a little dirt track. The nerves steadily send the message up, find the right part of the brain, which steadily sends the message back down. BUT our bodies are AMAZING! if you have had lots of pain in this area before (like in your pelvis with endometriosis) the nervous system adjusts. Your little dirt track starts to build new lanes and becomes a superhighway. Its connections become faster, and the part of your brain processing this information grows. Your body becomes on high alert, becuase this is important! Your superpowered nervous system is ready to go! Your nervous system may have sprouted new nerves and have more neurstransmitter chemicals to make connections quicker in response to your pain . So nervous system changes are real, and so is your pain. Working with a great medical and allied health team can help you with the structural problem (like Endo) but also help with the nervous system changes, becuase the treatment for both can be very different. 21.01.2022 WHAT TO EXPECT AT MY POSTNATAL PHYSIO APPOINTMENT . Firstly well have a good debrief on your pregnancy and birth story. This is a really important time to reflect on such a significant moment in your life. This will be followed up by queries around current symptoms (pelvic heaviness, urinary or fecal incontinence, back pain etc) and your goals for rehabiliation . We will then assess the resting tone, co-ordination and strength of the muscles of your pelvic floor to determin...e how they are progressing in the postnatal period. The ligaments which support your pelvic organs will be checked and from here a plan for return to exercise will be established. The mobility of perineal or caesarean scars will be reviewed and scar management techniques implemented if required . Strength and conditioning following pregnancy and delivery is not only about the pelvic floor. The rest of your core (particularly the abdomen) has been stretched and weakened and assessment of abdominal separation and global strength will aid your recovery. Your physio will work with you to determine the what and why of your exercise goals (whether it be walking/ crossfit/ yoga or running) and outline the rehab steps required for you to reach your goals. Regardless if you enjoy high intensity exercise or more gentle forms, babies get heavy quickly, so regaining your strength is important . 15.01.2022 A critical evaluation of the trigger point phenomenon "The theory of myofascial pain syndrome (MPS) caused by trigger points (TrPs) seeks to explain the phenome...na of muscle pain and tenderness in the absence of evidence for local nociception. Although it lacks external validity, many practitioners have uncritically accepted the diagnosis of MPS and its system of treatment. Furthermore, rheumatologists have implicated TrPs in the pathogenesis of chronic widespread pain (FM syndrome). We have critically examined the evidence for the existence of myofascial TrPs as putative pathological entities and for the vicious cycles that are said to maintain them. We find that both are inventions that have no scientific basis, whether from experimental approaches that inter- rogate the suspect tissue or empirical approaches that assess the outcome of treatments predicated on presumed pathology. " "The construct of MPS caused by TrPs remains conjecture. All working hypotheses derived from this conjecture have been refuted and therefore the theory can be discarded. In contrast, evolving insights into the neurobiology of noci- ception and pain suggest plausible hypotheses that form a basis for advancing knowledge and therapeutics in this challenging area." "In our opinion, current neuroscientific hypotheses can form the basis for collaborative scientific investigation to explain the clinical phenomena. We offer two for consid- eration, neither of which relies on local pathophysiology." "Quintner and Cohen [25] hypothesized that the TrP was an area of what was then called secondary hyperalgesia occurring in muscles that are structurally and physiologically unimpaired. Noting the remarkable proximity of TrPs to known peripheral nerves, these authors argued that sensitization of the axons within the nerves, possibly by inflammation, may inform the underlying mechanism. 14.01.2022 LUMINOSITY 2019 . Fatigue is one of the most common symptoms of endometriosis, with a recent study finding women with endo experience fatigue at twice the rate of women without endo (Ramin-Wright et al 2018) . All types of exercise can help fatigue, and high intensity and strength training can be particularly helpful. The hard thing with endo is that sometimes your fatigue and pain is at a level where pushing yourself isnt possible. This is where breaking your exercise in...to snacks through the day (eg 3 blocks of 10mins) can be helpful . 09.01.2022 SHOULDERS CAN BE A PAIN..... . Shoulder pain is really common, and often debilitating. A recent study (Imagama et al 2019) showed that shoulder pain, more than knee and low back pain had the most impact on physical and mental quality of life . Treatment can be confounded by the fact that structual pathology can just as freqeuently be found on scans of pain free shoulders as painful ones (Barreto et al 2019)... . One thing that is universally accepted with shoulder pain is that progressive loading to improve strength should be the goal of physio treatment. So make sure you have a physio who will work with you (not just on you) to progressively build strength and improve your function . 07.01.2022 I had the privilege to be included on panel last Thursday for @unsw and @endometriosisaustralia where I got to hear @sophiedillman speak eloquently about her experience with endo. Sadly her story is a common one, with a delay in diagnosis and having healthcare providers downplay or dismiss her symptoms. To the women with endo - if you are seeing someone and you can sense the internal eye roll when you discuss experience, calmly walk out the door and leave. You des...erve to be heard and believed. To health care providers - its ok to not know how to help someone or to refer on. Its NOT ok to not listen, downplay or belittle someones experience. . Endo is complicated. There will be peaks and troughs. If someone tries to sell you a magic cure call bullshit. Stand up for yourself and assemble a team who will celebrate the wins AND be in the trenches when times get tough. Im pissed for women with Endo. We can and should do better. 05.01.2022 Obturator Internus is a pelvic wall muscle that starts inside your pelvis and exits to join onto your hip. It works to help rotate your knee away from you. . The cool thing about this muscle is it has a little canal that houses the pudendal nerve, which branches off to supply parts of your perineum . In some instances of pelvic pain this muscle can become dysfunctional and contribute to symptoms from the pudendal nerve (eg pelvic pain with sitting, bladder or bowel changes ...or pain in the genitalia) . Here is a little sequence designed to reconnect your brain to this muscle and help it gain its full range of contract and relax. . Ex 1 -push your left knee out into the wall for 3 sec, then relax across your body for 3 sec. repeat x 3 Ex 2 - gently rotate the left knee and hip into the midline of your body, hold for 5 sec and repeat x 5 (avoid hip pain or jamming!) Ex 3 - deep hip stretch bringing knee across to opposite shoulder. Hold for 5 breaths. To celebrate the end of my year long professional certificate in Pessary management (Prof cert in the Conservative management of Pelvic Organ Prolapse at Unisa), this week Im going to post a few updates on physios and pessaries . Pessaries are a really great way to manage the symptoms of prolapse or incontinence by supporting the vaginal walls - kindve like a splint or a brace. They are made of medical grade silico...ne, and are a conservative, cost-effective and relatively simple thing to manage. They can be a brilliant way to help you do the exercise you want without symptoms . Some pessaries need to be taken out every day (e.g. the cube) and others can stay in for days at a time. How you choose to use your pessary is up to you. Some women just like to use them for sport, whilst others will wear them every day. . 04.01.2022 Prolapse 101 The word Prolapse can induce a lot of fear and worry, when in reality it is an extremely common condition that affects up to 50% of women . Pelvic Organ Prolapse (POP) is where the pelvic organs sit lower in the pelvis than they used to. Your bladder, uterus and bowel are suspended inside the pelvis by an amazing array of connective tissue and are supported underneath by the deep pelvic floor muscles .... I often describe this support system like a suspension bridge the pelvic floor muscles are the road base, and the ligaments/ fascia are like the suspension cables. Prolapse can happen when either the muscles or ligaments of the pelvic floor become weaker, more flexible or damaged . What can you do? Pelvic physio of course! 02.01.2022 LUMINOSITY 2019 . This is a nice movement that opens the front of the hips and mobilises the nervous system. It can be really relieving after a flare or with ovulation pain when you feel all scrunched up through the hips and abdomen . Concentrate on tucking the pelvis under first, then add in gentle head nods which should increase the tightness through the front of the thigh. The side bend increases the stretch up into the abdomen... . The second part of this exercise can be intense on the knee, and is a really strong stretch to the quad, so ease into it or avoid it if there is any pain . 02.01.2022 TAMING THE MONKEY MIND . I have been loving the lectures from Jetsunma Tenzin Palmo on @yogaanytime where she describes the importance of training the mind with both calmness and candour . Building a calm mind takes discipline, but it is the foundation of everything we do. Our mind is always with us, even when we dream, so it is common sense to train it to be content... . Focusing on exercise and a healthy diet is important, but whats the point if we are feeding our mind with junk every day?
