Morning, Afternoon or overnight shifts to choose from! About the role: We are seeking enthusiastic, passionate and caring individuals to join our team supporting people with disabilities. Your work will support people achieve their individual lifestyle goals, and as we support people of all ages and interests, each work day will be different and innovative. You will work with clients in the Community, providing a 1:1 service. Responsibilities and Duties: Provide 1:1 direct care assistance to each client in accordance with their care plan and as directed Monitor the safety and well being of the clients and report any concerns to supervisor Support clients to access activities and outings in the community Manage accurate and detailed shift reports, incident reports and progress notes Work with client, family/carer, relevant case manager and other professional to support the client’s individual goals Actively contribute to a team environment through open communication and participation in regular staff meetings Participate in performance development meeting to identifying ongoing training and support opportunities. To be considered for this role, you must provide the following: Minimum of 1 year paid work experience in supporting people with disabilities Satisfactory National Police Check for 'Employment in Disability' (no more than twelve months old) Satisfactory Working With Children's Check (Employee) Current First Aid & CPR Certificate Current Driver’s License & your own car A Certificate III or IV in one of the following or equivalent: Individual Support/ Home & Community Care/ Aged Care/ Disability/ Nursing If you are currently enrolled in one of these courses, or an equivalent course, we would love to hear from you!
