17.01.2022 Flight Logics on recent findings in relation to 2014 G-IV accident... G-IV Runway Accident, N121JM: Release of Report Synopsis Provides Key Learnings Regarding Compliance and Warning Systems - In September, the National Transport Safety Board (NTSB) released a synopsis (with the final report subject to final edits) in relation to the runway overrun of a G-IV during a rejected takeoff in Massachusetts 2014. The incident resulted in a loss of control whereby the aircraft rolle...d and collided with airport equipment and fencing, killing all on board - two pilots, a flight attendant and four passengers. Impact forces and a post-crash fire deemed the aircraft a wreck. The interim synoptic report has revealed that habitual noncompliance contributed to the accident, in particular failing to conduct a flight control check. Whilst a challenge-verification-response checklist task might have avoided this accident there were a number of safety barriers that were either overridden, procedurally not followed or delayed, which increased the likeliness of such an accident. The report highlights some important lessons to be seriously considered by Flight Crew, Trainers, Auditors, Engineers, those responsible for airport infrastructure/fittings and those that can support technology retrofit for the gust lock warning system in the G-IV model. The latter being concerned with limiting the operation of the aircraft when the gust lock is engaged, whereby pilots receive an unmistakable warning to prevent takeoff, from the beginning of the takeoff phase. It is worthwhile for all Operators to consider how they might measure Flight Crews compliance to checklists and SOPs so that compliance rates are available and meeting standards.

13.01.2022 What are your thoughts on confidential reporting within aviation? Some of the benefits include protecting the identity of reporters and upholding values of Just Culture. Just Culture is a culture some organisations adopt which allows for a non-punitive system which addresses how and why an incident occurred without pointing the finger at individuals, thus enabling the organisation to learn from errors and mistakes. Confidential reporting is also thought to improve the likelihood of individuals reporting thus strengthening the benefits achieved through any reporting system. 03.01.2022 The Australian Airports Association (AAA) recently updated their species information fact sheets following the release of statistics (by the ATSB) which indicated species of Kites and Bat/Flying Foxes had the largest overall number of strikes during 2012-2013 and hence presented an evidential risk to flight operations. The fact sheets are used by aerodrome operators as a source of information which includes how to identify species, species distribution/population patterns, h...abitat information; as well as food, behaviour and breeding information. The fact sheets provide guidance on specie specific risk management techniques for those species that pose the greatest threats to operations in Australia. The information incorporates waste, water and grass management key attractions for wildlife food, shelter, nesting etc. THE AAA also iterates the important role of correct identification of species and the accurate reporting of strikes. It is through identification and reporting that trends can be monitored to ascertain effectiveness of mitigation/management activities and allow for continuous improvement in managing risks associated with wildlife strikes. 01.01.2022 Ever heard the term green aircraft? Maintaining the integrity of an aircrafts metal is paramount, as deterioration to its smooth surfaces or weakening in the airframe or within the airframes parts can have catastrophic consequences. These planes have just had their anti-corrosion treatment a coat of corrosion resistant green paint. All aluminium aircraft will have this, or a similar treatment applied, under their final paint job. ... Some common applications are: catalysed polyure-thane enamel, waterborne polyurethane enamel, and two-part epoxy paint. Initial prevention coupled with robust cleaning practices are key to protecting the metal beneath. Areas that can be more prone to corrosion include (but are certainly not limited to) landing gear, wing flaps and the frontal engine. Helicopters endure similar corrosion challenges and parts such as rotor heads and gearboxes should be inspected on a regular basis.
