18.01.2022 What is Iselins Disease? Inflammed growth plate in Iselins disease in a child. Iselins Disease is pain around the base of the little toe (5th metatarsal). The pain may radiate down the lateral (outside) of the foot. It is commonly described as a sharp/aching or bruised type of pain. This is often associated with localised swelling and redness. Pain typically gets worse with activity occasionally resulting in a limp.... Who gets Iselins Disease? Common in children between 8-16 years old especially those that are physically active. Adults cannot get Iselins disease as the growth plate closes over and turns into bone with skeletal maturity at approximately 16 years old. Adults can get pain in the same place as this is the point of attachment of a muscle. What causes Iselins Disease ? In children up to the age of about 16 years, the bones in the feet still have growth plates present. These growth plates are made up of soft cartilage. Eventually this hardens and fuses once growth/development has slowed or been completed. There is an epiphysis (growth plate) that is situated on the base of the 5th metatarsal. This is half way down the outside edge of the foot. Around this area is also an important attachment area for the peroneal tendons. These tendons runs all the way from just below the lateral knee down the lateral (outside) of the shin. They run down and attach into the bones in the area we are discussing. Over-loading of this tendon with regular sports/activities causes the tendon to become acutely inflamed and irritated. At the same time in a growing child there are typically phases of rapid bone growth. The surrounding muscles and tendons then have to play catch up to this growth. At times, they may be shorter and tighter than normal. The combination of acute inflammation from repetitive stress, with a short and tight peroneal tendons due to rapid growth, results in excess tension and inflammation on the growth plate (epiphysis). The end result is pain, swelling and occasionally bone or cartilage over growth around the base of the little toe (5th metatarsal). How do you treat Iselins Disease? Iselins disease should initially be treated by the RICE (Rest Ice Compression Elevation) method. A reduction in sports/activities is necessary to reduce repetitive stress on the growth plate. Time is needed to allow repair of the growth plate to take place. A Podiatrist can also play a vital role in the rehabilitation process of Iselins disease. Over-activity or repetitive stress of the peroneal tendon may be caused by biomechanical abnormalities within the leg or foot. This can cause the peroneal tendons to work harder than normal to stabilise the foot. A Podiatrist can assess the lower limb mechanics and discuss the use of orthotics to manage this. 16.01.2022 The term pronation refers to the excessive rolling in of the foot while standing, walking or running. Some pronation is normal in every step, but too much can cause agreat range of problems throughout the body. Below are diagrams of the foot and body showing some (but not all) of the most common places that foot pronation can cause pain. Be aware that these problems will affect 40% of the population at some time in their lives, while only around 5% of the population seek trea...tment. This means that a vast number of people are suffering leg, hip or back pain that does not respond well to symptomatic treatment of the sore spot because the cause of the problem is never addressed. Think of your body in terms of a multi-storey building. How can the 5th floor window frames be fixed if the crumbling foundations are never repaired? Treatments such as rest, massage or anti inflammatory medication will help to relieve the pain until the activity that causes the pain occurs again. Unfortunately, when this activity is standing or walking, relief is short lived. Visit the podiatrist for a comprehensive examination of your complaints. A blister is a fluid filled pocket that emerges on the outer layer of the skin due to injury or infection, in response you body develops the blister to cushion the damaged area to prevent any further harm. Having blisters on your feet can be very uncomfortable or painful whilst having to walk throughout the day. Causes and Symptoms The main cause of blisters on the feet is due to a mixture of repeated friction and pressure against the skin. Although, tissue... or blood vessel damage to the outer layer of the skin can create a blister. When the skin of the feet is continually rubbed against a shoe, sock, or rough surface it can begin to cause irritation and inflammation . This will result in pain, swelling, and redness. Before the blister itself has developed, a red sore will begin to emerge on the area of inflammation on the foot. If the irritation continues, it will then lead to shearing of the skin. The continued inflammation will cause shearing, this is where small tears are created in the skin. The bodys response will send fluids to fill the small tears and help cushion and protect the underlying tissue layers. When your feet hurt, it is obvious that you might have a foot problem. A podiatrist is the logical place to go for help. What many people dont realise is that poor foot function can cause widespread pain across your body. Think of your skeleton like a multi-storey building. e wouldnt expect the window frames of the top floor to be straight if the foundations are crumbling and the whole structure is out of alignment. Treatments including rest, massage and anti-inflammatory medication will probably help to relieve most types of pain until the movements that cause the pain occur again. Unfortunately, when that is just simply standing or walking, relief will be short lived. Where might I get pain do I need orthotics? All soft tissue shrinks when cold and needs time to warm up with movement. This is just like an athlete would do to prevent injury before working hard.
