12.01.2022 ! Heterogeneity is not an RCT pitfall if the sample sizes are big; rather, it is where RCT comes into its strength. Randomization of large samples adjusts for known and unknown confounders. In this situation, adjustment with regression analysis is an optional extra.... Homeopathy already has a method for identifying variables and predictive factors for effect. It is, of course, the simillimum principle. If a simillimum or close proxy is prescribed, then one expects an effect. If homeopathy has the strong effects that homeopathic practitioners believe it has, then this should be detectable by large sample RCTs. The ability to supply clinically indistinguishable medicines and controls makes homeopathy the therapy par excellence for using the RCT, much more so than pharmaceuticals. Before getting distracted by esoteric and hitherto unproven study designs, homeopaths should consider using a proven design that produces decisive mathematically rigorous results the large sample size double-blind RCT. They could consider using what in their experience is a proven method of remedy selection, the simillimum principle, which is the cornerstone of prescribing and admit only those patients who are good examples of this principle and not cases that are speculative. In 200 years this has not been undertaken. A large network of practitioners, say, 1400+ or 0.5% of practitioners in India, under no pressure to prescribe, could admit only those patients with clear homeopathic indications randomized by central randomization. For simplicity the study could be conducted over a short time span; e.g., one month, so as to be minimally inconvenient to study participants and practitioners, and allow rapid data collection. With a large research network large sample sizes in rigorous design could be collected rapidly, studies replicated, and the effect of homeopathy tested serially in different conditions: anxiety/depression, asthma, rheumatoid arthritis, migraine, etc. If homeopathy has the effects practitioners believe, then it has the potential to be an extremely validated therapeutic system.
