25.01.2022 Most women CAN and SHOULD exercise in pregnancy. However, exceptions do exist. Certain medical conditions can mean a reduction, modification or cessation of activity due to potential health risks. Conditions where exercise can cause harm, either to mum or bub include:... severe cardiorespiratory disease, placental abruption, vasa previa, uncontrolled type 1 diabetes, intrauterine growth restriction, active preterm labour, severe pre-eclampsia and cervical insufficiency Your obstetrician will be right on to these conditions and providing advice as to what is safe for you. If we are treating women with medical conditions of concern, we are always in close contact with the treating obstetrician to make sure exercise is managed safely and appropriately. Week after week, especially at the moment in Victoria, we are speaking to and trying to help so many women who have had reduced or not evidence based postpartum care due to healthcare service restrictions and a lack of understanding of the importance of pelvic floor health by some providers. Making pelvic floor physiotherapy part of standard postpartum care would improve the physical and mental health of so many, and caring fo...r one generation of women well means they will be able to more easily care for the next. 23.01.2022 Oestrogen levels decline in menopause as well as in the first few months after giving birth. This drop in oestrogen can result in vaginal atrophy. Symptoms include: vaginal dryness vaginal walls become thin and frail ... irritation or itching dyspareunia (painful sex) Recent research shows us that topical oestrogen can be effective to help improve atrophy symptoms, as well as showing positive changes in other pelvic floor disorders, such as: improvements in urinary incontinence and decreases in urinary urgency If we find vaginal atrophy on examination we may recommend a review with your GP, as topical oestrogen is available via prescription only. 23.01.2022 INTIMATE PARTNER VIOLENCE Almost one in four (23.9%) respondents reported some form of intimate partner violence from a current or ex-partner. 11.1% (1 in 10 women) reported having a controlling partner. The figure was higher among women with a disability (18.9%) and those reporting a worse financial situation than before COVID-19 (15.3%).... 19.5% of women experienced emotional abuse. Physical (4.3%) and sexual violence (5.5%) was highest among women aged 18-24. "Stereotypically we think that all violence is the physical act, whereas the most common forms of violence experienced by women are emotional abuse and controlling behaviours. These are very harmful things to experience in intimate relationships and as the survey has shown, these behaviours are much more common than has been generally believed. 21.01.2022 Research shows that 36% of elite female athletes experience breast pain associated with contact sport yet only 10% of them report these injuries and less than half use any prevention strategies. Womens health physiotherapists are well placed to help ensure breast injuries are well managed. It is critical we normalise conversations around breast health so that everyone (whether athlete or social sport participant) can be encouraged to report and, when necessary, receive trea...tment for breast injuries. We encourage women to ensure they are wearing a good fitting bra that is suitable for the level of impact required in each individual sports participation. This may need to differ between sports. 20.01.2022 This week is womens health week and also happens to be birth trauma awareness week at the same time, which is fitting as they tend to go hand in hand. Our individual responses to birth can be extremely complex, regardless of whether it was a vaginal birth, vaginal birth with intervention, emergency cesarean or planned cesarean. Trauma is very personal, and unique. Some women find certain interventions more difficult to deal with than others. Mastitis is usually the result of a blocked duct (see yesterdays post) that hasnt cleared The milk build up can cause inflammation in the breast tissue, resulting in a red, swollen, hot and/or tender lump. Infection may or may not be present Early symptoms include feeling unwell with flu-like symptoms of fever, chills and aches. If you think you have mastitis, you should see your GP immediately How can I avoid getting mastitis? Feed your baby regularly and ensure a good latch Avoid skipping feeds Avoid tight tops or bras that put pressure on your breasts Rest as much as you can Eat well and stay healthy What should I do if I get mastitis? Start treatment ASAP See your GP immediately if you begin to feel unwell or develop a fever Get as much rest as you can - recruit your support crew now Drain the breast often and gently Check your latch and that the baby is feeding easily Feed from the affected side first, when the baby is sucking strongly Try gentle massage during the feed Change feeding positions to effectively empty the breast Before a feed: have a hot shower or apply warmth to help with milk flow After a feed: apply a cold pack to relieve pain and inflammation Being proactive can help prevent progression of mastitis and decrease your risk of developing a breast abscess. Get in to see your GP as soon as possible and consider physiotherapy for further assessment and other treatment options including therapeutic ultrasound. 20.01.2022 Todays topic: EXERCISE AND BREASTFEEDING Welcome back to our World Breastfeeding Week series where we discuss common issues and questions from our breastfeeding mums who come to the clinic for lactation support. Is it safe for me to exercise while breastfeeding?... Yes. Its important to focus on your health as well as your baby Exercise improves your fitness and does not affect infant growth Will exercise decrease my milk supply? No. Studies show moderate to high intensity exercise does NOT have a negative effect on milk volume OR composition Recommendations for exercise while breastfeeding: Make sure you are eating enough (caloric intake should not fall below ~20-25% of output). For most people this means ~1500kcal/day Drink water before, during and after exercise to keep hydrated Drink enough water that you urine is pale yellow. Darker urine is a sign that you need to drink more. 19.01.2022 Does this happen to you?? Ever wondered why? Well, back in 1998, Rao and colleagues investigated the activity in the gut after consuming coffee to try and figure out what might be happening... 12 people wore a probe in their colon to measure the motor activity of the gut ... Over 10 hours they consumed (in random order): caffeinated coffee, decaffeinated coffee, hot water and a 1000kcal meal They found that caffeinated coffee DOES increase the activity of the muscles of the gut The effect of caffeinated coffee was similar to eating a meal, 60% stronger than water and 23% stronger than decaffeinated coffee. What does this mean? Caffeinated coffee stimulates the muscles of the gut which in turn can help to move your stools down and out! Lots of our patients have noticed this effect in their bodies and use it as part of a healthy bowel routine to get things moving easily in the morning So if youve been looking for another reason to justify that morning coffee, consider this it! 14.01.2022 Pregnancy can be rough and sometimes it does feel like there must be seven babies in there (pregnant lady in this pic, we have all felt like you at some point. I see your face, I feel you). Lockdown has everyone spending a lot of time indoors and this can mean less incidental exercise and a lot of extra sitting which doesnt help a pregnant pelvis or spine. Some tips:... Try to limit sitting to 30min at a time. If working from home, there are computer programs to remind you to stand up and take a quick break Use your exercise time each day to go outdoors and get moving Consider a pregnancy pillow at night Stretches can be really helpful to prevent back pain. Here are some that require no equipment: Childs Pose (avoid if you have pelvic pain as well). Ducts carry the milk from the breast to the nipple If these ducts become blocked, milk can build up behind the blockage This causes a lump to form in the breast and your breast may feel tender How can I avoid them? Feed your baby regularly Avoid tight tops or bras that put pressure on your breasts Rest as much as you can Eat well and stay healthy What should I do if I get a blocked duct? Less water = constipation Constipation = pelvic floor problems ... So how do you make wearing a mask and drinking water work? 09.01.2022 Todays Topic: SPORTS BRAS AND EXERCISE GEAR FOR BREASTFEEDING A well fitted supportive sports bra is important to stabilise and compress the breasts against the chest. Matching your bra support to the level of activity impact is crucial. Especially where the upper body is in motion (i.e. jogging, dancing, etc).... Comfortable and loose-fitting shorts and tops are recommended if exercising indoors or in a warm environment. A breastfeeding mums metabolism may be higher than usual, and this may cause body temperature to increase rapidly during exercise. So choose fabric that allows for the easy dissipation of heat. In a cold environment, layering of clothing becomes important as the layers will trap warm air. If you get too hot, you can take off one or more of the layers. Weve tagged them before and we will tag them again... @shescience are skilled at fitting your bra to match your breastfeeding exercise needs. 09.01.2022 Vulval health... do you know how to care for down there? Did you know? Estrogen in the vaginal tissues promotes the growth of lactobacilli bacteria. These helpful bacteria create a slightly acidic pH of the vagina, reducing the likelihood of infections.... Many products that are marketed as cleansing products actually cause harm. They upset the vulvovaginal pH, which in turn affects the normal vulvovaginal microbiota needed for protection against infection. So if feminine cleansing products arent necessary, what should you do to look after your vulva? Keep it simple... Tips for vulval care: The vagina is self cleansing - wash with cool-lukewarm water only Do not use soaps, sprays or deodorants Wear cotton underwear Remove wet bathing suits and sweaty activewear promptly It is a good idea to become familiar with what is NORMAL for YOUR OWN vulva. 07.01.2022 Todays topic: ENGORGEMENT Welcome to our World Breastfeeding Week series where we discuss common issues and questions from our breastfeeding mums who come to the clinic for lactation support. What is engorgement?... Engorgement is when there is a build up of milk, blood and other fluids in the breast. This can cause your breasts to become hard, swollen and tender and your nipples to become flattened and taut. It commonly occurs 2-6 days after your baby is born but can also happen any time your babys feeding patterns change How can I avoid it? Feed often from birth and avoid other fluids unless medically needed Wake your baby for a feed if your breasts become full and uncomfortable Check your position and latch to ensure good drainage of the breast What to I do if it happens to me? Time for a history lesson #localpride #womenpower @yarraville_village @cityofmaribyrnong In 1919 the Spanish Flu epidemic swept through Melbourne. Yarraville in particular was hard hit but responded well to the challenge. Mrs Emma Newton and Mrs Jerram formed the Yarraville Influenza Relief Committee and set up 1...5 teams of women to distribute donated food and supplies. Their organisational centre was in Fels Park where a soup kitchen and laundry were established under large canvas tents. Households indicated they needed assistance from the teams by displaying a red SOS in their front window. It is important to understand breast kinetics because the bra-breast forces generated during physical activity directly affect the physical ability and comfort for women when exercising. Drinking more alcohol since COVID-19 was highest among women aged 25-44 (24.4%). One in three respondents (33.6%) reported that their health was worse than before COVID-19. This week we are not only highlighting the importance of #WomensHealthWeek, #worldphysiotherapyday2020 and #birthtraumaweek2020 but it is also Cancer Rehabilitation Awareness Week (#CRAW2020). Most people have either been diagnosed with or have a close friend or family member who has or had #cancer. What most people dont know is the importance of rehabilitation following a cancer diagnosis. Entering the post-treatment phase of a cancer journey often does not me...an that the physical and emotional recovery is complete. Most healthcare systems do not have the means to effectively rehabilitate post treatment and there is often a lack of referral on to those who can provide assistance with rehabilitation. Rehabilitation focuses on enabling those with a cancer diagnosis to live well and live fully. Our physiotherapy team has a special interest in functional recovery following #breast, #gynaecological and #prostate cancer diagnosis. 01.01.2022 Mask up and look down is about face masks and the increased risk of falling in our older population... we think this applies to our pregnant ladies and anyone wearing glasses With a mask on, your lower peripheral vision (the things you can see without moving your head to look down) is restricted when you wear a mask. This means you may not be able to see the ground and your feet without looking down when you have a face covering on. ... If you are wearing glasses, masks are also renowned for causing fogging which makes it difficult to see anything! So what should you do?? look down regularly when walking, and always when you are walking on an uneven surface or approaching a curb or step; when going up or down a curb or step, look down and use a support such as a handrail to assist with balance if available; If wearing glasses, try the following to help prevent them fogging up: ensure firm fit over your nose using wire or tape wear glasses over the top of the fabric place a tissue on the bridge of your nose between the mask and your face clean glasses with a small amount of dishwashing liquid use single lens glasses (in preference to bifocal or multifocal glasses) for walking outside if you have them.
