25.01.2022 It is an absolute privilege to officially announce that @h2h_physiotherapy are the proud physiotherapists for Oakhill College in Season 2020. Since graduating in 2010, Director of H2H, Ben Eisenhuth, has been involved as a Strength and Conditioning coach, First Aider and now the Head Physiotherapist for Oakhill College. This Saturday marks the return to Damien Oval for the 1st XV, the return to CSC for our 1st Netball, and the return to Lawrence Oval for our 1st XI!... Given the crazy year we have all endured, H2H cannot wait to be back home and I am sure all of Oakhill College feel the same way! 20.01.2022 To ICE or NOT TO ICE that is the question! So should I Ice my injury? Quite simply! YES, but!! yes there is a but! (see below) ... 1 What does ICE actually do? Reduces secondary damage - ICE reduces tissue temperature by about 5-15 degrees which in turn decreases cell metabolism in the area surrounding the injury. In short, one of the roles of cell metabolism is that it creates heat which can further damage tissue at a cellular level. So by cooling the skin you are reducing the chance of further damage to underlying tissues and surrounding structures. Reduces Pain - Reducing the temperature of the skin decreases nerve conduction to the site of aggravation and/or damage, which basically means the body sends less pain signals to the site of injury! gets us moving - When you decrease pain you allow yourself the ability to get moving with a lower pain rating! By allowing this pain limited movement you will help reduce swelling and in turn speed up recovery both at a cellular level as well as functional level allowing you to return to what you love! 2 How long should i ICE my injury for? - Use the trial & check method of pinching and grabbing your skin and being able to distinguish between sharp and blunt sensations every 1,5,10,15,20mins. - This will reduce the likelihood of ICE BURN! - Simply, never exceed 20 minutes and allow your skin to return to the same temperature as the opposite side before reapplying more ice. 3 How many times should I ice my injury today and over the weekend? - Regularly for 3 days! - The acute inflammatory period lasts roughly 72 hours so here @H2H we suggest apply ICE hourly for the first 24 hours and then 2hourly 25-48 hours and 3 hourly 49-72 hours. NEVER ICE during sleep! - By the end of day 3 your REHAB PLAN should be in place! 17.01.2022 Very pleasing to hear no internal damage for Luke Keary overnight. Great friend of H2H was seen tackled with an elevated shoulder and open rib cage. From here Keary went down wincing in pain and was unable to complete the simplest of tasks, passing and running. Diagnosis - No internal damage, Diagnosed Non-Displaced Rib fracture(s) Prognosis - Depending on pain and function loss. Usually between 2-6 weeks depending on location and significance of fractures... Assessment - 1. Great from Roosters Medicos to call for an Ambulance to rule out internal damage. Luke was in immense pain and to rule this out immediately is of extremely high importance as it can be life threatening. Hence, weekend sporting events and competitions should always invest in health professionals for sports coverage! 2. Pain with autonomic tasks - sneezing, coughing cause pain locally 3. Limited AROM - Poor thoracic ROM, esp. rotation. Pain with shoulder elevation, patient will want to keep arms lowered. 4. Pin point tenderness - obvious, localised pin point tenderness when palpating over the ribs, usually the angle of the rib is fractured (most prominent to the skins surface) 5. Scans - Usually X-Rays distinguish any noticeable fractures, however most specialists will require either a CT or MRI scan to rule out damage to the bone integrity as well as internal damage. An MRA is a fantastic tool to rule out or confirm internal bleeding. Treatment - Manage pain and gradually introduce functional movements and gradually load patient when pain reduces and function returns. Early 2 weeks very limited treatment. Breathing exercises, Pain medications, Ice (controversial), equalising internal with external pressure with a pillow or towel (sneezing, coughing) all can speed up recovery.
